Midterm Outcome of Bone-Ligament-Bone Graft and Dorsal Capsulodesis for Chronic Scapholunate Instability.

PURPOSE: To assess midterm outcomes of our bone-ligament-bone (BLB) grafts for chronic scapholunate (SL) instability and better define criteria for their use. METHODS: We conducted a retrospective review of 26 patients treated with BLB grafts and dorsal capsulodesis between 1997 and 2009. Twenty-four patients were reviewed. Mean follow-up was 8.2 years. Two patients had dynamic lesions, 7 had SL dissociation, 14 had a dorsal intercalated segment instability lesion, and 1 had SL advanced collapse stage 1. Mean age at surgery was 46 years. All patients presented with pain and 14 had lack of str

Bilateral transcapholunate dislocation.

An uncommon case has been reported of bilateral transscaphoid-perilunate dislocation fracture following trauma. A 35-year-old male with bilateral transscapholunate dislocation was treated by closed reduction and internal fixation. The case was examined in detail, and compared to the findings in the literature; observations regarding fracture prognosis were also made. Two years post-surgery, the patient remained asymptomatic.

Perilunate Dislocation Reduction Technique and Results.

BACKGROUND: Perilunate dislocations are severe upper extremity dislocations to the wrist involving ligamentous injury. Previous literature cites closed reduction of perilunate injuries at 25%. Herein, the technique and results of closed reductions for perilunate dislocations are described. METHODS: Chart reviews were conducted of perilunate injuries treated from 2017 to 2022. Imaging was evaluated to categorize by Herzberg stage. Reduction was performed in the emergency department (ED) and involved traction, relaxation, and manipulation. Reduction attempts and success rates were recorded as w

Comparison of arthroscopy-assisted versus open reduction and fixation for treating scaphoid fractures.

OBJECTIVES: Scaphoid fractures represent the most common type of carpal fracture. This study aimed to retrospectively evaluate and compare the therapeutic efficacy of wrist arthroscopy-assisted fixation versus conventional open reduction with fixation for scaphoid fractures. METHODS: We conducted a retrospective cohort study involving 159 patients with acute scaphoid fractures. Participants were stratified into two groups: the open reduction with fixation group (underwent open reduction and fixation) and the Arthroscopy group (received wrist arthroscopy-assisted fixation). Comparative analyse

Is the Repair of Acute Scapholunate Injuries Associated With Distal Radius Fractures Necessary at the Time of Osteosynthesis? A Systematic Review and Meta-Analysis.

BACKGROUND: Acute scapholunate ligament injuries (SLIs) can occur in distal radial fractures (DRFs). This systematic review compares patient-reported outcomes and range of motion (ROM) between operative and nonoperative treatment of acute SLIs in association with surgical fixation of DRFs. We hypothesize that there is no clinical difference. METHODS: A meta-analysis was used to evaluate the effectiveness of SLI repair versus no repair occurring with DRF with Disabilities of the Arm, Shoulder, and Hand (DASH) scores. We identified 154 articles of which 14 were eligible for review. Only 7 studi

The Diagnostic Utility and Clinical Implications of Wrist MRI in the Pediatric Population.

BACKGROUND: Unexplained wrist pain is a common presentation in children. To our knowledge, no studies have explored the clinical utility of magnetic resonance imaging (MRI) in the diagnostic workup of pediatric patients. METHODS: We retrospectively reviewed 307 consecutive wrist MRIs ordered at a tertiary-care pediatric hospital. Demographic data and the indication for imaging were recorded and grouped into admitting categories. The final impression of each MRI was scored with regard to potential impact on future treatment (0 = normal, 1 = minimal, 2 = moderate, 3 = high). Patients who went o

The Palpable Scaphoid Surface Area in Various Wrist Positions.

PURPOSE: To determine the theoretical amount of surface area available for palpation of the scaphoid in various wrist positions and to provide a guide depicting which wrist position will expose proximal pole, waist, and distal pole fractures. METHODS: Using 3 fresh-frozen male cadaver wrists, we digitized palpable surface areas (dorsal, volar, and snuffbox) of the scaphoid in several wrist positions. The entire scaphoid was then excised and a digitized 3-dimensional reconstruction of the entire scaphoid was obtained. The 2 images were superimposed and the surface area was calculated RESULTS:

Incidence estimates and demographics of scaphoid fracture in the U.S. population.

PURPOSE: The epidemiology of scaphoid fracture is based mostly on retrospective analyses of small population centers. The purpose of this study was to determine the incidence of scaphoid fractures in a large national population database. METHODS: The National Electronic Injury Surveillance System (NEISS) is a representative sample of patients that is weighted to provide estimates of various injuries in the United States presenting to emergency rooms. Data on wrist fractures obtained from NEISS were used to estimate the incidence of scaphoid fractures in the United States between 2002 and 2006

Comparison of percutaneous dorsal versus volar fixation of scaphoid waist fractures using a computer model in cadavers.

PURPOSE: Percutaneous screw fixation (PSF) is widely used to treat acute nondisplaced scaphoid waist fractures. PSF can be performed through a volar or dorsal approach. The aim of our study was to compare a dorsal versus volar surgical approach for PSF according to the sagittal orientation of the waist fracture (B1 or B2 in Herbert and Fisher's classification scheme, in which B1 and B2 designate, respectively, oblique and transverse nondisplaced scaphoid waist fractures) on computer modeling of cadaver wrists. METHODS: We used 12 upper limbs, and for each wrist we performed 3 computed tomogra

The clinical implications of scaphotrapezium-trapezoidal arthritis with associated carpal instability.

PURPOSE: Common causes of dorsal intercalated segment instability (DISI) include scapholunate dissociations and scaphoid fracture nonunions. Although less common than these, scaphotrapezium-trapezoidal (STT) osteoarthritis (OA) may also be associated with the development of a DISI deformity. The clinical implications of this form of carpal instability in cases of STT arthritis are still unknown. To study the radiographic progression and incidence of this entity, we reviewed our patients and report on 24 wrists with DISI in the presence of STT arthritis. METHODS: A retrospective chart and radi

[Navigation for placement of scaphoid screws : a new indication for intraoperative 3D navigation-a cadaver study].

INTRODUCTION: Up to now, the use of navigation systems for the placement of scaphoid screws has been impossible, mainly because there have been no ways of fixing the reference markers. Faulty placement rates in internal fixation of the scaphoid show there is a current need for a 3D image-based navigation system and intraoperative monitoring of how successful the procedure will be. For this reason, we have developed a new radiotransparent hand fixation device (Scaph-Splint), which allows reliable and accurate drilling of the scaphoid using 3D navigation. Tests of this device and the simultaneou

Ultrasound for the early diagnosis of clinically suspected scaphoid fracture.

PURPOSE: To test the ability of ultrasound to diagnose clinically suspected occult scaphoid fractures. METHODS: Eighteen wrists in 18 patients with an average age of 35 years (range, 10-77 years) were seen in the emergency room, each with a single traumatic wrist injury, snuffbox tenderness, swelling, and a negative wrist x-ray result. They were evaluated in this prospective, blind, controlled study by physical examination, x-ray, and high-resolution ultrasound. One hand surgeon performed the examination, and ultrasounds were read by a musculoskeletal radiologist. Patients were immobilized in

[Occult fractures of the carpal navicular. Detection by quantitative radioscintigraphy. Social and medico-legal repercussions].

Fractures of the scaphoid of the wrist are not easily diagnosed and when they are eventually recognized due to late complications, the prognosis is much poorer than it would have been at injury. Medical negligence and poorly-performed or interpreted x-rays are frequently cited as reasons for non-diagnosis. Often these fractures are trabecular, so-called occult, and they cannot be seen on a conventional exam, even if well-performed. Tomography and tomodensitometry have yielded poor results. Quantitative radioscintigraphy (QRS), the exam we are proposing, is a modification of scintigraphy, which

3D-Printed Patient-Specific Guides Versus Conventional Techniques for Scaphoid Reconstruction: A Comparative Study of 89 Cases.

Scaphoid non-union or malunion often produces a three-dimensional (3D) deformity that alters wrist biomechanics. Patient-specific instrumentation (PSI) has been introduced to improve reconstruction accuracy, but quantitative comparisons with conventional free-hand techniques remain limited. We retrospectively analyzed 115 scaphoid reconstructions performed between 2010 and 2019; 89 cases met inclusion criteria (45 PSI, 44 conventional). Distal-fragment rotation (flexion/extension, ulnar/radial inclination, pronation/supination) and translation were quantified using a Cartesian coordinate syste

Should we always perform scaphoid nonunion surgery in patients with minor preoperative symptoms?

The objective of this study was to assess the downsides of surgical treatment of scaphoid fracture nonunion in patients with minor preoperative symptoms. Patients were classified with minor symptoms based on the Patient-Rated Hand/Wrist Evaluation questionnaire. Of the 35 included patients, most patients encountered problems with patient-specific activities; 9% reported worse postoperative functional outcomes, 34% were not satisfied with the treatment and 9% were reoperated. The risk of a worse functional outcome after surgery with the need for further operations and the chance of developing w
