Meningeal malignant solitary fibrous tumor with multiple recurrence, extracranial extension, cervical lymph node metastases: case report and review of the literature.

INTRODUCTION: Solitary fibrous tumor (SFT) of the central nervous system (CNS) is a spindle cell neoplasm originating from mesenchymal tissue. SFT is prone to local recurrence or distant metastasis. The main sites of metastasis include the liver, lung, and bone. However, the tumor direct extracranial extension via natural skull base foramina to cervical region is rare. CASE PRESENTATION: A 43-year-old male presented with an incidentally discovered left temporo-occipital region mass spanning the tentorium cerebelli identified during head and neck CT angiography, initially suspected as meningio

Long-term survival outcomes and recurrence patterns of olfactory neuroblastoma: A 13-year experience at a single institution.

OBJECTIVES: Olfactory neuroblastoma (ONB), also known as esthesioneuroblastoma, is a rare malignant neoplasm of the nasal vault and anterior skull base. The results of treatment for ONB are relatively good; however, regional and distant metastases can develop several years after definitive treatment. This study aimed to validate the treatment modality of ONB for oncological outcomes, especially for regional recurrence. METHODS: We retrospectively reviewed the medical records of 22 patients diagnosed with ONB at Kyoto University Hospital between 2009 and 2020. Descriptive statistics were calcu

Preauricular transzygomatic anterior infratemporal fossa approach for tumors in or around infratemporal fossa lesions.

Various surgical approaches to the infratemporal fossa (ITF) have been reported. Among them, the preauricular transzygomatic anterior ITF approach (anterior ITF approach) has been used for exposure of the antero-superior part of the ITF. The purpose of this article is to show anatomical dissections using the anterior ITF approach and to evaluate our surgical experience using this approach. An anatomical study of the anterior ITF approach was performed using six sides of three cadaveric heads. Clinical course was retrospectively reviewed for 34 patients who underwent microsurgical resection of

Fractionated stereotactic radiation therapy improves cranial neuropathies in patients with skull base meningiomas: a retrospective cohort study.

BACKGROUND: Skull base meningiomas commonly present with cranial neuropathies. Fractionated stereotactic radiation therapy (FSRT) has been used to treat these tumors with excellent local control, but rates of improvement in cranial neuropathies have not been well defined. We review the experience at Thomas Jefferson University using FSRT in the management of these patients with a focus on symptom outcomes. METHODS: We identified 225 cases of skull base meningiomas treated with FSRT at Thomas Jefferson University from 1994 through 2009. The target volume was the enhancing tumor, treated to a s

[Extensive lower clivus chordomas. Removal using the unilateral transmandibular approach].

BACKGROUND AND PURPOSE: Clival chordomas are rare skull-base tumors with local malignant behavior. Their control and removal remain difficult because of their anatomical location and because of their extensions. The goal of the treatment is complete surgical removal in a single stage if possible, with minimal deficits, followed by proton therapy. If the tumor remains extradural for a while, it finally progresses through the dura backwards to reach and displace the brain stem and upper cervical cord. Its anterior extension in the retropharyngeal space offers a logical opportunity and many advan

Acquisition of surgical skills for endonasal skull base surgery: a training program.

INTRODUCTION: Endonasal surgery represents a radical change in the practice of cranial base surgery and requires the acquisition of new knowledge and surgical skills. The optimal training program for surgeons has not been established. METHODS: We reviewed our experience with endonasal cranial base surgery from 1998 to 2006 to develop a training plan for the acquisition of surgical skills. It consists of a modular and incremental approach to endonasal skull base surgery that is designed to train surgeons to function as a team, learn endoscopic skull base anatomy, and develop fundamental endosc
