Secondary shoulder reconstruction in patients with brachial plexus injuries.

Restoration of shoulder stability in post-traumatic plexopathy patients is very important because more distal functions depend on a stable a … 6.87 ± 10.32 of external rotation. Secondary procedures such as pedicle and free muscles transfers, tendon transfers, and rotational humerus osteotomy augment shoulder stability and function in patients with irreparable paralysis.

Antibody-mediated disease remission in the mouse model of lyme borreliosis.

In the mouse model of Lyme borreliosis, the host immune response during infection with Borrelia burgdorferi results in the remission of carditis and arthritis, as well as global reduction of spirochete numbers in tissues, without elimination of infection. These events were recapitulated by passive transfer of immune serum from infected immunocompetent mice or T-cell-deficient mice to severe combined immunodeficient (SCID) mice. Previous studies have shown that immune serum is reactive against arthritis-related protein (Arp) and that Arp antiserum induces arthritis remission. However, although

Minimizing impairment in laborers with finger losses distal to the proximal interphalangeal joint by second toe transfer.

Traditionally, toe-to-hand transfers have been reserved for thumb amputations or for use after severe mutilating injuries. The authors report their experience with the use of second toe-for-finger amputations with preserved or reconstructible proximal interphalangeal joints in manual workers. The aim of the procedure was to reduce impairment and to upgrade the hand from a functional and cosmetic standpoint. Fifteen second-toe wrap-around or variations were carried out on 11 adults (18 to 41 years old). Four patients with two or more finger amputations received two sequential second toes; four

[The operative treatment of recurrent and simple traumatic dislocations of the elbow by the method of Osborne and Cotterill (author's transl)].

The essential pathological defect causing dislocation of the elbow is failure of the postero-lateral ligamentous and capsular structures. A … in our opinion it attacks the essential lesion. More complicated techniques such as bone blocks or tendon transfers, are believed to be less effective and unnecessary. Four patients with recurrent dislocation of the elbow joint were treated successfully by this method, based on an follow-ups of from 2 to 4 years. Encouraged by the results we transferred the meth …

Pronator Quadratus to Extensor Carpi Radialis Brevis Nerve Transfer in C5-C7 or C5-C8 Brachial Plexus Injuries for Independent Wrist Extension.

Background Patients with lesions affecting C7 and C8 roots (in addition to C56) demonstrate loss of independent wrist dorsiflexion in addition to loss of shoulder abduction and elbow flexion. Traditionally, this deficit has been addressed using tendon transfers after useful function at the shoulder and elbow has been restored by primary nerve surgery. Confidence with nerve transfer techniques has prompted attempts to replace this method by incorporating procedures for wrist dorsiflexion in the primary operation itself. Aim The objective of this study was to report the results of pronator quadr…

Vascularised toe proximal interphalangeal joint transfer in posttraumatic finger joint reconstruction: the effect of skin paddle design on extensor lag.

UNLABELLED: Vascularised joint transfer (VJT) from the toe proximal interphalangeal joint (PIPJ) has been criticised for providing a limited range of motion (ROM) and significant extensor lag in reconstructed digits. The original technique includes the dorsal skin of toe PIPJ as a marker of joint viability, while a modified technique uses the skin flap of the great toe and the dorsal foot. This systematic review aimed to assess vascularised toe-to-finger PIPJ transfers with respect to functional outcomes, and in particular, whether a modified skin paddle design can improve extensor lag. Biomechanical comparison of innovative suture techniques for repair of Achilles tendon rupture in a porcine flexor tendon model.

INTRODUCTION: In recent years, evidence-based progressive rehabilitation protocols following the surgical treatment of Achilles tendon rupture have been shown to be safe and appear superior to more restrictive treatment regimens. These protocols mainly rely on early mobilization and weight bearing. To enable this during postoperative rehabilitation, a biomechanically stable surgical treatment must be ensured. Different solutions for suturing ruptured Achilles tendons are currently favored by orthopedic surgeons and have been investigated in the past, including promising further developments of

Reconstruction of soft tissue defects of the Achilles tendon with rotation flaps, pedicled propeller flaps and free perforator flaps.

INTRODUCTION: Soft tissue defects exposing the Achilles tendon are challenging. Local perforator flaps represent a valuable option gaining increasing popularity. Despite preoperative planning an adequate perforator cannot always be found intraoperatively. The free peroneal artery perforator flap can serve as a back-up option limiting the donor site morbidity to the same extremity without sacrificing major vessels or nerves. METHODS: Nine patients with soft tissue defects exposing the Achilles tendon were treated with local perforator flaps, seven were scheduled for 180° propeller flap coverag

Outcome after Achilles tendon lengthening with a posterior capsulolysis according to Imhäuser in idiopathic congenital clubfoot.

BACKGROUND: The Ponseti method has radically reduced the need for extensive surgical treatment in idiopathic clubfoot. At present there are no universally accepted criteria for the use of the various surgical techniques. MATERIAL AND METHODS: 77 patients with idiopathic congenital clubfoot (111 affected feet) were treated according to Imhäuser. The operative release included z-shaped Achilles tendon lengthening and a posterior capsulolysis of the talocrural and talocalcaneal joint. In case of relapse in infancy and school age, a transfer of the tendon of the anterior tibial muscle (n=26) and

Endoscopic flexor hallucis longus transfer in the patients with chronic achilles rupture.

BACKGROUND: achillestendon rupture (ATR) is a widespread tendon injury that can be observed in middle-aged individuals after sporting activities or secondary to trauma. We aimed to retrospectively assess the clinical findings and radiological parameters of individuals treated with isolated endoscopic flexor hallucis longus tendon transfer (FHLT) due to chronic achillestendon ruptures (CATR) before and after surgical intervention. The working hypothesis was that, based on postoperative functional and clinical evaluation, FHLT was an effective and safe method in CATR treatment. METHODS: We anal…

Assessing an Online Patient Decision Aid about Upper Extremity Reconstructive Surgery for Cervical Spinal Cord Injury: Pilot Testing Knowledge, Decisional Conflict, and Acceptability.

UNLABELLED: Background. While nerve and tendon transfer surgery can restore upper extremity function and independence after midcervical spinal cord injury, few individuals (∼14%) undergo surgery. There is limited information regarding these complex and time-sensitive treatment options. Patient decision aids (PtDAs) convey complex health information and help individuals make informed, preference-consistent choices. The purpose of this study is to evaluate a newly created PtDA for people with spinal cord injury who are considering options to optimize upper extremity function. Methods. The PtDA w…
