Percival Pott; Pott's fracture, Pott's disease of the spine, Pott's paraplegia.

There is a commonly held belief that Percival Pott sustained that ankle fracture that bears his name. This probably is not true; the injury was more likely a fracture of the tibial shaft, as suggested by careful reading of the account of the accident left to us by his son-in-law.

Multiple myeloma presenting initially with pleural effusion and a unique paraspinal tumor in the thorax.

We herein report an extremely rare case of a patient with IgD-lambda positive multiple myeloma presenting with myelomatous pleural effusion and ascites. A 58-year-old man visited our hospital with dyspnea as his initial symptom. His chest radiograph findings on admission revealed a left pleural effusion, and later, bilateral involvement. Computed tomography (CT) of the chest showed a paraspinal tumor with extension from the upper mediastinum to the abdomen. The cytological examination demonstrated myeloma cells in the pleural effusion and ascites, and histologically, in the pleura, an abdomina

[Pain and amyotrophy in upper members in a cirrhotic patient with Pott's disease].

We report the case of a 56 year old cirrhotic woman who presented during the course of a tuberculous spondylodiscitis affecting T9-T10, a clinical picture consistent with neuralgic amyotrophy affecting the right shoulder first, and later also the left one (Parsonage-Turner syndrome). This is an uncommonly diagnosed entity of unknown etiology and pathogenesis. Magnetic resonance images (MRI) include high signal intensity in supra and infraspinatus muscles and other muscles of the shoulder girdle, compatible with muscle oedema associated with denervation. These features, combined with the abilit

[Differences between tuberculous spondylitis and brucellar spondylitis].

OBJECTIVE: To identify potential differences in the clinical and laboratory characteristics between tuberculous spondylitis (TS) and brucellar spondylitis (BS). PATIENTS AND METHODS: Retrospective study of patients with TS and BS diagnosed in our hospital between january 1992 and december 1998. RESULTS: TS was diagnosed in 17 patients and BS in 10 patients. In our series, a higher delay in the diagnosis (27.9 +/- 24.6 vs. 16 +/- 5.6 weeks, p = 0.02) was found in TS. There was a higher frequency, but without stadistic significance, of immunosuppression, one or several paravertebral or epidura

Cervical tuberculous vertebral osteomyelitis: case report and discussion of the literature.

We report a case of tuberculous vertebral osteomyelitis of the first and second cervical vertebrae with extensive adjacent soft-tissue involvement and extension into the mediastinum and bilateral flanks. The clinical presentation of tuberculous vertebral osteomyelitis depends on the vertebrae involved. The characteristic syndrome, Pott's disease, reflects the consequence of infection of the lower thoracic and lumbar spine, the most common site of tuberculous vertebral osteomyelitis. Cervical involvement is unusual: tuberculosis affects the cervical vertebrae in approximately 0.03% of all cases

[Magnetic resonance tomography in the diagnosis of tuberculous spondylitis].

The examination included 35 patients with clinical and X-ray signs of tuberculous spondylitis. Magnetic resonance tomography (MRT) was performed on a Maganview apparatus (Finland). MRT helped to diagnose an active process in 5 patients. The most manifested changes were found on T2-suspended tomograms to indicate an inflammatory reaction of the bone marrow. MRT was effective in detecting bone marrow compression, its early atrophy, hydromyelia and postoperative changes. MRT can obviously fully replace pneumomyelography and myelography with contrast substances.

Emergency caesarean section in a poor risk patient in Nepal.

This report describes the management of a poor risk Nepalese patient (active pulmonary tuberculosis, haemoglobin concentration 4.6 g dl-1, weight 28.5 kg) requiring emergency Caesarean section for fetal distress. Subarachnoid blockade (0.5% bupivacaine plain 3.5 ml) was satisfactory; blood loss was around 400 ml and was replaced with Haemaccel and lactated Ringer's solution (blood was not available). Although unresponsive at birth, the child had an Apgar score of 10 at 15 min.

Vertebral column decancellation for the management of sharp angular spinal deformity.

The management goal of sharp angular spinal deformity is to realign the spinal deformity and safely decompress the neurological elements. However, some shortcomings related to current osteotomy treatment for these deformities are still evident. We have developed a new spinal osteotomy technique-vertebral column decancellation (VCD), including multilevel vertebral decancellation, removal of residual disc, osteoclasis of the concave cortex, compression of the convex cortex accompanied by posterior instrumentation with pedicle screws, with the expectation to decrease surgical-related complication

Tubercular atlantoaxial dislocation in children: an institutional experience.

OBJECT: In this paper the authors analyzed the clinical and imaging-documented profile of pediatric patients with tubercular atlantoaxial dislocation (AAD). METHODS: Seventeen children 16 years of age or younger with tubercular AAD were included in the study. Patients with reducible AAD underwent direct posterior stabilization. All patients were treated with a four-drug antituberculosis therapy (ATT) regimen (10-20 mg/kg/day rifampicin, 10-20 mg/kg/day isoniazid, and 15 mg/kg/day ethambutol in a single daily dose; and pyrazinamide 20-35 mg/kg/day in two divided doses) for 3 months. The pyrazi

[Value of different surgical techniques for sampling biopsy material in the diagnosis of tuberculous spondylitis].

Surgery biopsy was used to examine 133 patients admitted to hospital for suspected tuberculous spondylitis. The surgical biopsy diagnostic techniques involved biopsy of peripheral lymph nodes, paracentesis of abscesses, puncture and trephine biopsies of vertebral bodies, and open diagnostic vertebrotomy. Biopsy studies using surgical techniques verified the diagnoses of tuberculous spondylitis, vertebral tumors, and nonspecific vertebral osteomyelitis in 115, 8, and 10 patients, respectively. The patients' examination took 2 to 6 weeks. The use of surgical techniques for obtaining biopsy speci

Paleopathology in osseous remains from the 16th century. A survey of rheumatic diseases.

OBJECTIVE: To describe the rheumatic conditions found in skeletal remains of Amerindian ancestry disinterred from a 16th century Mexican cemetery. METHODS: A physical anthropologist and 2 rheumatologists surveyed the recovered skeletal remains. RESULTS: We examined the skeletal remains of 443 subjects. We found 19 cases of Pott's disease, 17 of osteoarthrosis in various anatomical locations, 2 spondyloarthropathies, probably ankylosing spondylitis or diffuse idiopathic skeletal hyperostosis, one probable septic arthritis in the stemoclavicular joint, and 2 compression fractures of the spine.
