Acromegaly has a substantial negative impact on quality of life (QoL). This review aims to discuss the impact of acromegaly on QoL from the clinical perspective as well as from the patient perspective. Furthermore, it aims to evaluate the use of patient-reported outcome measures (PROMs) in acromegaly and how PROMs aid decision-making. Here I review vertebral fractures (VFs) as an emerging complication of acromegaly through a pathway of key questions in order to help clinicians manage the disease. Secondary diabetes mellitus is known to occur in acromegaly due to insulin resistance caused by growth hormone excess. However, diabetes in acromegaly usually does not lead to ketosis. We describe an unusual case of a patient that presented with diabetic ketoacidosis in the emergency room with thirst, polyuria and dyspnoea. The patient was subsequently diagnosed with a growth hormone-secreting pitutary macroadenoma as the underlying pathology; after initial stabilisation with insulin and fluids, the patient was successfully treated with trans-sphenoidal surgery.

Pachydermoperiostosis maybe mistaken for acromegaly as it can present with progressive enlargement of hands and feet. We describe a 32 year old male with enlargement of hands and feet and extensive keloid formation. Family history was positive for similar complaints. X ray imaging showed normal heel pad thickness with acroosteolysis and subperiosteal new bone formation in hands and feet. IGF-1 was normal and glucose suppressed GH values were normal.
