Challenges in implementing preventive treatment for latent tuberculosis infection: a narrative review from provider and demand-side perspectives.

BACKGROUND: Preventive treatment is an important measure to interrupt the development of tuberculosis (TB) in people with latent tuberculosis infection (LTBI), which is among the key elements of TB prevention and control in the future. However, the implementation of preventive treatments has been affected by various factors and has fallen far short of expectations. Therefore, this study aims to systematically identify barriers to initiating and implementing LTBI preventive treatment globally from both demand-side and supply-side perspectives, providing targeted evidence to advance the End TB S

Review of assistive devices for the prevention of pressure ulcers: an engineering perspective.

PURPOSE: Pressure ulcers (PUs) are prevalent among immobile bed or wheelchair-reliant individuals who experience prolonged sedentary positions. Pressure relief and frequent repositioning of body posture help to mitigate complications associated with PUs. Adherence with regular repositioning is difficult to maintain due to nursing labour shortages or constraints of in-home caregivers. Manual repositioning, transferring, and lifting of immobile patients are physically demanding tasks for caregivers. This review aimed to explore and categorize these devices, discuss the significant technical chal

Establishing targets for advanced HIV disease: A call to action.

The World Health Organization (WHO) has published a guideline for the management of individuals with advanced HIV disease (AHD) to reduce HIV-related deaths. The guideline consists of a package of recommendations including interventions to prevent, diagnose and treat common opportunistic infections, including tuberculosis (TB), cryptococcosis and severe bacterial infections, along with rapid initiation of antiretroviral treatment and enhanced adherence support. Currently no clear targets exist for these key interventions. Emerging programmatic data from Uganda, Tanzania and Nigeria suggest tha

Effects of Hydrogen Sulfide on the Microbiome: From Toxicity to Therapy.

Significance: Hydrogen sulfide (H2S), an important regulator of physiology and health, helps resolve inflammation and promotes tissue repair in the gastrointestinal tract. Recent Advances: Gut microbiota live as a multispecies biofilm in close interaction with the upper mucus layer lining the epithelium. The relative abundance, spatial organization, and function of these microorganisms affect a broad range of health outcomes. This article provides a state-of-the-art review of our understanding of the cross talk between H2S, the gut microbiota, and health. H2S can have toxic or therapeutic effe

Advanced ovarian cancer patients identify opportunities for prehabilitation: A qualitative study.

Prehabilitation may modify frailty and increase resilience in a subset of ovarian cancer patients; however there is low adherence to most programs. Our aim was to investigate potential barriers and facilitators of prehabilitation during neoadjuvant chemotherapy (NACT). We identified 15 patients who underwent NACT from 2016 to 2018. Patients underwent a semi-structured one-on-one interview. Transcripts were coded by 4 independent reviewers to identify emerging themes related to patients' experience, functioning and exercise during chemotherapy. Data saturation occurred after 15 interviews. Pati

Treatment of rheumatoid arthritis with anti-TNF-alpha agents: a reappraisal.

It has been found that tumour necrosis factor(TNF)-alpha plays a pivotal role in the pathogenesis of rheumatoid arthritis (RA), and the development of drugs targeting this molecule has extended the therapeutical approaches to RA patients. A number of observational studies of large patient series have also been published over the last few years, many of which have been based on national registries designed to monitor the efficacy and safety of anti-TNF agents, and allow healthcare institutions to control expenditure. Registry data can also help in identifying clinical and laboratory findings ca

Current Evidence, Challenges, Opportunities, and Future Directions of Rotator Cuff Failure: A Systematic Review.

Rotator cuff tears (RTs) are among the most prevalent musculoskeletal conditions worldwide, imposing substantial burdens of chronic pain, functional limitation, and socioeconomic cost. The underlying pathophysiology is multifactorial, encompassing progressive mechanical attrition, tendon degeneration, and irreversible biological deterioration, including muscle atrophy, fatty infiltration, and altered glenohumeral kinematics that may ultimately culminate in cuff tear arthropathy. The central clinical challenge lies in stratifying patients appropriately and selecting the optimal treatment strate

Using American Sign Language-Fluent Community Health Navigators to Advance Cancer Screening Adherence through Videoconferencing With Deaf, Deafblind, and Hard of Hearing Adults: Protocol for a Randomized Controlled Trial.

BACKGROUND: Cancer screening nonadherence persists among adults who are deaf, deafblind, and hard of hearing (DDBHH). These barriers span individual, clinician, and health care system levels, contributing to difficulties understanding cancer information, accessing screening services, and following treatment directives. Critical communication barriers include ineffective patient-physician communication, limited access to American Sign Language (ASL) cancer information, misconceptions about medical procedures, insurance navigation difficulties, and intersectional barriers for multiply marginaliz

From support to recovery: the evolving role of LVAD in reversing heart failure.

In recent years, the use of Left Ventricular Assist Devices (LVAD) in the treatment of heart failure has been increasingly widespread. Not only do they provide circulatory support for patients, but the reverse biological changes in myocardial tissue induced by LVAD have led to the recovery of heart function in some patients, allowing for the removal of the device-which termed bridge to recovery (BTR). Despite promising prospective studies reporting LVAD explantation rates exceeding 48-60% in BTR-focused cohorts, real-world registries (e.g., INTERMACS) demonstrate explantation rates below 5%, u

Incidence rate of tuberculosis among HIV infected children in Ethiopia: systematic review and meta-analysis.

BACKGROUND: Tuberculosis is one the leading causes of death from a single infectious disease, caused by the bacillus mycobacterium tuberculosis. In Ethiopia, even though several primary studies have been conducted on the incidence of tuberculosis among HIV-infected children, the pooled incidence rate of tuberculosis among HIV-infected children (aged 0-14 years) is unknown. Therefore, the main objectives of this systematic review and meta-analysis are to estimate the pooled incidence rate of tuberculosis among HIV-infected children and its predictors in Ethiopia. METHOD: International electron

Enhancing the utility of exploratory 'factors associated with' studies in global mental health: the importance of context in Sub-Saharan Africa.

"Factors associated with" studies, exploratory multivariable regression analyses that do not begin with a prespecified primary exposure, have recently come under increasing criticism, with some arguing that they are methodologically weak and contribute little to scientific knowledge. Much of this criticism is justified. Many published studies in global mental health (GMH) rely on small samples, inconsistent adjustment strategies, automated variable-selection methods, and interpretations that imply causality where none can be established. These limitations deserve attention and should not be ov

Measuring patient engagement with HIV care in sub-Saharan Africa: a scoping study.

INTRODUCTION: Engagement with HIV care is a multi-dimensional, dynamic process, critical to maintaining successful treatment outcomes. However, measures of engagement are not standardized nor comprehensive. This undermines our understanding of the scope of challenges with engagement and whether interventions have an impact, complicating patient and programme-level decision-making. This study identified and characterized measures of engagement to support more consistent and comprehensive evaluation. METHODS: We conducted a scoping study to systematically categorize measures the health system c

Bone marrow ECM-based vascular niche modeling for the evaluation of HSC/leukemic cell interaction with niche cells.

Bone marrow vascular niche regulates both hematopoiesis and hematopoietic disorders, including acute and chronic leukemia. However, it is still not fully understood at the structural and functional levels owing to its inaccessibility in situ. Here, a biomimetic bovine decellularized bone marrow extracellular matrix (dbECM) mixed hydrogel was developed. Proteomics validation confirmed its retention of bone marrow-specific ECM components. The mechanical properties and microstructure of the mixed hydrogel provided a biomimetic platform that integrates biochemical and physical cues for niche funct
