Reduced Retinal Vascular Density and Skeleton Length in Amblyopia.

PURPOSE: This study aimed to investigate the possible relationship between retinal vascular abnormalities and amblyopia by analyzing vascular structures of fundus images. METHODS: In this observational study, retinal fundus images were collected from 36 patients with unilateral amblyopia, 33 patients with bilateral amblyopia, and 36 healthy control volunteers. We developed a customized training algorithm based on U-Net to digitalize the vasculature in the fundus images to quantify vascular density (area and fractal dimension), skeleton length, and number of bifurcation points. For statistical…

Managing Pediatric Cataract Patients: Amblyopia Treatment and Binocularity Outcomes.

Unilateral congenital cataracts present multiple barriers in the development of vision and stereoacuity despite the improved visual optics that early surgery, contact lenses and intraocular lenses (IOL) have provided. With better understanding of the latent period (the timeframe in which the abnormal event has no long-term effect on visual development in the deprived eye) and the critical periods (the age range during which developing brains can be altered in a profound and permanent way by abnormal experience) for stereoacuity and amblyopia we can focus our treatment methods to not only impro…

Understanding the development of amblyopia using macaque monkey models.

Amblyopia is a sensory developmental disorder affecting as many as 4% of children around the world. While clinically identified as a reduction in visual acuity and disrupted binocular function, amblyopia affects many low- and high-level perceptual abilities. Research with nonhuman primate models has provided much needed insight into the natural history of amblyopia, its origins and sensitive periods, and the brain mechanisms that underly this disorder. Amblyopia results from abnormal binocular visual experience and impacts the structure and function of the visual pathways beginning at the leve…

Straatsma syndrome: Satisfactory amblyopia treatment. Report of two cases.

CLINICAL CASES: The cases are presented on 2 female patients with Straatsma syndrome, with satisfactory treatment of amblyopia. DISCUSSION: The level of anisometropia and myelination of retinal nerve fibres were different in these two patients. However, both achieved 0.20 (logMAR) visual acuity with correction in both eyes following amblyopia treatment with ocular patching. Visual prognosis of amblyopia associated with myelination of retinal nerve fibres and anisometropia is poorer than anisometropic amblyopia without myelination. It is well known that the former is refractory to occlusive th…

A pilot study using electronic reminders for amblyopia treatment.

Treatment compliance is the most important factor for predicting a successful outcome in amblyopia treatment. Electronic applications have been successfully employed in other medical conditions in an effort to improve compliance.Aim: To determine whether a smartphone medical adherence application (app) (RxmindMe® Prescription/Medicine Reminder and Pill Tracker) may be successfully incorporated into the treatment plan of patients 3-7 years of age who have not previously been treated for amblyopia. Methods: Children 3-7 years of age were randomized to receive electronic reminders (reminders group…

Unilateral congenital nasolacrimal duct obstruction and amblyopia risk factors.

INTRODUCTION: This study aimed to investigate the prevalence of amblyopia risk factors in patients with unilateral congenital nasolacrimal duct obstruction (CNLDO). PATIENTS AND METHODS: A retrospective interventional case series was performed on all consecutive patients of unilateral CNLDO who underwent probing over a 6-month period in 2017. All patients underwent a complete ocular examination, retinoscopy, axial length measurements and keratometry. Risk factors for amblyopia were noted based on the American Association for Pediatric Ophthalmology and Strabismus guidelines. The fellow eye of…

Contrast-balanced binocular treatment in children with deprivation amblyopia.

BACKGROUND: Children with deprivation amblyopia due to childhood cataract have been excluded from much of the emerging research into amblyopia treatment. An investigation was conducted to determine whether contrast-balanced binocular treatment - a strategy currently being explored for children with anisometropic and strabismic amblyopia - may be effective in children with deprivation amblyopia. METHODS: An unmasked, case-series design intended to assess proof of principle was employed. Eighteen children with deprivation amblyopia due to childhood cataracts (early bilateral n = 7, early unilat…

Deficient Binocular Combination of Second-Order Stimuli in Amblyopia.

PURPOSE: Sensory imbalances in humans with amblyopia have been well documented using luminance-modulated (first-order) stimuli. However, little is known regarding whether there is a deficient binocular combination in amblyopes for stimuli defined by modulations in contrast (second-order stimuli). To address this, we asked two questions: Does a sensory imbalance also exist in the binocular combination of second-order stimuli, and if so, is it more severe than that expected on the basis of the imbalance for first-order stimuli? METHODS: The sensory imbalances of 14 adult amblyopes (mean age: 30…

Conversion visual loss: a differential diagnosis in infant amblyopia.

PURPOSE: To report the first case of amblyopia due to a conversion disorder in a child. METHODS: A 9-year-old girl without allergies or family or personal antecedents of interest presented to our clinic. She manifested a progressive visual loss after an episode of lumbar pain. This progressive loss led the patient in a 6-month period to legal bilateral blindness: visual acuity inferior to 2/20 in both eyes and severe constriction of both visual fields. RESULTS: Several pathologic processes were discarded by means of multiple explorations and a general multidisciplinary analysis: physical pat…

[Neuronal and psychomotor malfunction secondary to delayed amblyopia treatment].

BACKGROUND: A review of neuronal and psychomotor alterations related to delay of amblyopia treatment was carried out. METHODS: We reviewed various studies to explain the anomalies of visual cortex because of the prevalence of anomalous stimulus in patients with amblyopia. RESULTS: Visual pathways are developed embryologically. The newborn has ocular dominance columns ready to be stimulated, but visual alterations present at this time will generate neuronal changes in visual cortex. CONCLUSIONS: Delay of amblyopia treatment with anomalous visual stimulus will provoke organic changes in visua…

[Facial asymmetry, asymmetry of muscle tonus and unilateral amblyopia].

The authors noted the high prevalence of facial asymmetry and of unilateral amblyopia in some hundred children suffering from chronic neurological conditions. Unilateral changes in muscle tone are responsible for such asymmetry. Amblyopia in itself may cause a facial asymmetry. The dental arcades are little affected even by severe deformities of their underlying bony supports. Dento-maxillary abnormalities are no more severe in these children than in a normal population.

Whole blood cyanide levels in patients with tobacco amblyopia.

Three patients presented with painless bilateral visual failure due to tobacco amblyopia. The whole blood cyanide levels were raised above those predicted from their high tobacco consumption, approaching lethal levels reported from acute inhalation of cyanide. Each patient had an excessive alcohol intake with biochemical evidence of hepatic dysfunction, the elevated whole blood cyanide levels being attributed to the associated impairment of cyanide detoxification. In each case the improvement in visual acuities following abstinence and hydroxycobalamin therapy was accompanied by a reduction in…

Suppressive processes in binocular vision: ocular dominance and amblyopia.

Recent suggestsions linking suppression observed in amblyopia ex anopsia with that manifested in normal ocular dominance coordinations were experimentally tested. Training procedures which normally alleviate functional amblyopic suppression were shown to have similar results in alleviating the suppression of the nondominant eye in visually normal observers. Methodological and theoretical implications of these results are discussed.
