[A vocation for life on land, sea and air - health assessment of ambulance workers].

Introduction: Each aspect of prehospital emergency care places a different demand on the physical and mental health of workers. Objective: We aimed to compare the stress and other risk factors of ground, air, water ambulance workers. Method: A cross-sectional study was carried out in August–October 2022. Nonrandom, convenience data sampling was conducted on a voluntary basis, using an anonymous, online questionnaire at the ambulance stations of the Hungarian National Ambulance Service (n = 70), the bases of the Hungarian Air Rescue Nonprofit Ltd. (n = 24) and the Hungarian Water Rescue Service

Early feasibility of an embedded bi-directional brain-computer interface for ambulation.

Current treatments for paraplegia induced by spinal cord injury (SCI) are often limited by the severity of the injury. The accompanying loss of sensory and motor functions often results in reliance on wheelchairs, which in turn causes reduced quality of life and increased risk of co-morbidities. While brain-computer interfaces (BCIs) for ambulation have shown promise in restoring or replacing lower extremity motor functions, none so far have simultaneously implemented sensory feedback functions. Additionally, many existing BCIs for ambulation rely on bulky external hardware that make them ill-

Reverse shoulder arthroplasty in upper extremity ambulators: a matched-cohort analysis.

BACKGROUND: Upper extremity ambulators (UEAs) who require prolonged use of assistive devices for mobility have a high incidence of shoulder pathology secondary to increased stress across the shoulder joint with upper extremity weight-bearing. Reverse shoulder arthroplasty (RSA) for rotator cuff arthropathy has historically been associated with increased complications in UEA, but more recent studies have shown more promising outcomes. The objective of this study is to evaluate clinical outcomes and complication rates between these 2 groups to define the relative risk of RSA in the UEA populatio

Evaluation of Cardiac, Autonomic Functions in Ambulant Patients with Duchenne Muscular Dystrophy.

BACKGROUND: The most effective treatment for chronic lateral ankle instability is still open lateral ankle ligament repair. However, because arthroscopic repair is less intrusive and more surgeons are choosing it, it has become more and more common. For persistent lateral ankle instability, the purpose of this systematic review and meta-analysis (SRMA) was to compare the clinical results of arthroscopic and open repair. METHOD: This SRMA was conducted per PRISMA guidelines. A comprehensive literature search was performed across PubMed, Embase, Scopus, and the Cochrane Library using predefined

Ambulance use and subsequent fragmented hospital readmission among older adults.

BACKGROUND: Interhospital care fragmentation, when a patient is readmitted to a different hospital than they were originally discharged from, occurs in 20%-25% of readmissions. Mode of transport to the hospital, specifically ambulance use, may be a risk factor for fragmented readmissions. Our study seeks to further understand the relationship between ambulance transport and fragmented readmissions in older adults, a population that is at increased risk for poor outcomes following fragmented readmissions. METHODS: We analyzed inpatient claims from Medicare beneficiaries in 2018 who had a hospi

Impact of Early Ambulation on the Prognosis of Coronary Artery Bypass Grafting Patients.

BACKGROUND: The effect of delayed ambulation on the outcome of coronary artery bypass grafting (CABG) remains to be clarified. METHODS AND RESULTS: The long-term and in-hospital outcomes of 887 patients who underwent isolated CABG (455 off-pump cases, 135 urgent cases) were evaluated, with a focus on the timing of first ambulation. In-hospital mortality cases were excluded. Early ambulation (first ambulation within 3 days after operation) was achieved in 339 (38%) patients. In the multivariable logistic regression analysis, longer operation time and urgent case, EuroSCORE II, re-thoracotomy,

Community ambulation in people with lower limb amputation: An observational cohort study.

Lower limb amputation (LLA) is still a health issue requiring rehabilitation and long-term care even in industrial societies. Several studies on subjects with LLA have been focused on the efficacy of rehabilitation and factors influencing the use of prosthesis. However, literature data on the recovery of ability to walk outdoors, and thus to participate in social life in this population is limited. To investigate potential correlations between socio-demographic and clinical factors, and the use of the prosthesis for indoor and/or outdoor walking referred to as community ambulation (CA) in subje

Preserved thenar muscles in non-ambulant Duchenne muscular dystrophy patients.

BACKGROUND: Clinical trials in Duchenne muscular dystrophy (DMD) focus primarily on ambulant patients. Results cannot be extrapolated to later disease stages due to a decline in targeted muscle tissue. In non-ambulant DMD patients, hand function is relatively preserved and crucial for daily-life activities. We used quantitative MRI (qMRI) to establish whether the thenar muscles could be valuable to monitor treatment effects in non-ambulant DMD patients. METHODS: Seventeen non-ambulant DMD patients (range 10.2-24.1 years) and 13 healthy controls (range 9.5-25.4 years) underwent qMRI of the rig

The positive predictive value of an ambulance prealert for stroke and transient ischaemic attack.

OBJECTIVE: Therapeutic options for ischaemic stroke, such as thrombolysis or thrombectomy, are time sensitive. Multiple innovations have been established to reduce the symptom-to-needle time. One such innovation is the prealerting of emergency department (ED) or stroke unit staff by prehospital personnel of suspected stroke patients. The diagnosis of stroke can sometimes be difficult, with stroke mimics being a recognized issue. The prealert mobilizes ED, stroke and imaging personnel, which, for a true-positive, improves door-to-needle times. However, there are a proportion of false-positive p

A retrospective comparison between non-conveyed and conveyed patients in ambulance care.

BACKGROUND: Not all patients where an ambulance is dispatched are conveyed to an emergency department. Although non-conveyance is a substantial part of ambulance care, there is limited insight in the non-conveyance patient population. Therefore, the study aim was to compare demographics, initial on-scene reasons for care, and vital signs between conveyed and non-conveyed patients attended by an ambulance. METHODS: A retrospective study of ambulance runs from 2 EMS regions in the Netherlands in 2016 was performed. For each ambulance run demographics (age, gender and geographical location), ini

Safety of Nurse-Led Ambulation for Patients on Venovenous Extracorporeal Membrane Oxygenation.

PURPOSE: Venovenous extracorporeal membrane oxygenation (VV ECMO) is an effective therapy in patients with acute lung injury and end-stage lung disease. Although immobility increases the risk of complications, ambulation of patients on VV ECMO is not the standard of care in many institutions. Staff concerns for patient safety remain a barrier to ambulation. In this case series, we present our experience utilizing a nurse-driven ambulatory VV ECMO process to safely rehabilitate patients. METHODS: We retrospectively reviewed all VV ECMO cases at our institution between January 1, 2011, and Nove

Ambulation and independence among Veterans with nontraumatic bilateral lower-limb loss.

In describing functional outcomes and independent living in a cohort of bilateral major amputees, we sought to provide current estimates of function and independence after a second major amputation in an elderly Veteran population with peripheral arterial disease and/or diabetes. After retrospectively reviewing and excluding the electronic health records of those failing to meet the inclusion criteria, we identified 40 patients with a history of unilateral major amputation who underwent a second major amputation during the defined study period. Of these, 43% (17) were bilateral transfemoral am

Use of Functional Ambulation Performance Score as measurement of gait ability: review.

Gait analysis systems are widely used for the assessment of gait disabilities and provide more accurate and detailed information than clinical tests. Scores and indexes have been proposed to summarize the large volume of data produced, each emphasizing different aspects of gait. Based on specific spatiotemporal parameters, the Functional Ambulation Performance Score (FAPS) quantifies gait at a self-selected speed. Integrated within electronic walkways, the FAPS is commonly used for clinical evaluations and has been used in an increasing number of publications over the past few years. However,

Factors associated with ambulance use among patients with low-acuity conditions.

BACKGROUND: The use of ambulances for low-acuity medical complaints depletes emergency medical services (EMS) resources that could be used for higher-acuity conditions and contributes to emergency department (ED) overcrowding and ambulance diversion. Objective. We sought to understand the characteristics of patients who use ambulances for low-acuity conditions. We hypothesized that patients who arrive to the ED by ambulance for low-acuity conditions are more likely to be members of vulnerable populations. METHODS: A secondary analysis was performed on the National Hospital Ambulatory Medical …

Gait component changes observed during independent ambulation in young children.

The components of gait of the new independent ambulator can differ from that of the more experienced ambulator. The purposes of this review were to describe observable gait components exhibited at the onset of independent ambulation, the progression of changes in these components, and the time when a more mature pattern should be present consistently. Cross-sectional, longitudinal and single-recording studies, which analyzed children's gait at the onset of independent ambulation and followed changes in the components, were reviewed. Only components that can be observed by clinicians were inclu
