Amebiasis is caused by Entamoeba histolytica, commonly found in the liver and colon. Its presentation is rare in the gallbladder. A 25-year-old female presented with epigastric pain radiating towards the right hypochondrium. The patient was diagnosed with calculous cholecystitis. Later on, performing a laparoscopic cholecystectomy, histopathology resulted in acute-on-chronic necrotizing cholecystitis with E. histolytica trophozoites. A 220-kDa E. histolytica lectin is capable of downregulating some inflammatory cytokines (IL-5, IL-6, INF-gamma and TNF-alpha) and thus of inducing an overall anti-inflammatory Th-phenotype in leucocytes of selected, perhaps constitutionally predisposed, individuals irrespective of their HLA-DR3 profile (i.e., in this study patients long recovered from amebic abscess of the liver). This probably inhibited cytokine response pattern could increase the risk for developing amebic abscess of the liver in the course of invasive intestinal amebiasis.

Differential diagnosis of intestinal amebiasis (106 patients), acute Flexner's dysentery (105 patients) and nonspecific ulcerative colitis (31 patients) based on the clinical data and rectoromanoscopic findings is described. Rapidly and gradually progressing in the first 7 days intestinal amebiasis is characterized by a gradual onset, normal t0 with mounting general weakness, celiectasia, diarrhea with hyaloid mucus, focal catarrh-hemorrhagic changes and appearance of individual oval transversal ulcers on the colon mucosa.
