The Krukenberg procedure in the bilateral amputee after electrical burn.

Nine Krukenberg procedures were performed in 5 bilateral amputee patients suffering severe electrical injuries of both hands and wrists. The surgical procedure yielded functioning pincers in 7 of 9 operations. The patients could perform their daily activities very well and showed pronounced improvement in their self-confidence, self-reliance, and quality of life. The postoperative rehabilitation of these patients was not difficult. The Krukenberg procedure should be considered a substitute for prosthesis, especially in developing countries.

Marked bone spur formation in a burn amputee patient.

This report presents an unusual case of a lower extremity burn amputee with a marked degree of bone spur formation. A 17-year-old man suffered 56% body surface area mixed-depth electrical and flame burns, necessitating left below knee amputation. He was admitted to a rehabilitation center 3 months postinjury for pylon fitting and gait training. Difficulty was encountered with poor skin tolerance to weight bearing because of the prominent distal bony margins in the stump. X-rays of the stump revealed a marked degree of linear bone spur formation, extending longitudinally from the distal tibia a

Energy cost of walking of amputees: the influence of level of amputation.

A comparison of selected gait parameters and the energy cost of prosthetic walking was made in seventy patients with unilateral traumatic and vascular amputations. Amputations above the knee, below the knee, and at the Syme's level were compared in both groups of amputees, and a control group of forty normal subjects also studied. In both groups of amputees performance was significantly better the lower the level of the amputation. When preservation of function is the chief concern, amputation should be performed at the lowest possible level.

Brain activation induced by pressure pain of healthy individuals versus amputees.

Pain, as a common symptom, seriously affects the physical and mental health of individuals. This work investigated the processing mechanisms of pressure pain by combining mathematical modeling, functional Magnetic Resonance Imaging (fMRI) and Electroencephalography (EEG) methods. Differences in brain activation between healthy individuals and amputees were revealed, and EEG features for effective pain recognition were identified. The results showed that pain sensation was generated in the brain only when the output of T-cells exceeded a critical potential and that the frequency of the membrane…

Kinetic asymmetry in unilateral transfemoral amputees: Gait biomechanics and reliability assessments.

BACKGROUND: Individuals with unilateral transfemoral amputation (TFA) commonly experience gait-related loading asymmetries, elevating their risks of lower limb injuries and degenerative changes. To evaluate the effects of interventions on these asymmetries, it is crucial to assess test-retest reliability and establish minimum detectable changes (MDCs) for loading-related measures. Determining them for these measures would help distinguish meaningful interventional effects from daily variability. This study evaluated test-retest reliability and established MDC values for ground reaction force (

Machine Learning Model for Predicting Walking Ability in Lower Limb Amputees.

Background/Objectives: The number of individuals with lower limb loss (LLL) is rising. Therefore, identifying the walking potential in individuals with LLL and prescribing adequate prosthetic systems are crucial. Various factors can influence participants' walking ability, to different extents. The aim of the present study was to apply machine learning methods to develop a predictive mode. This model can assist rehabilitation and limb loss care teams in making informed decisions regarding prosthesis prescription and predicting walking ability in individuals with LLL. Methods: The present study

Stimulation of peroneal nerves reveals maintained somatosensory representation in transtibial amputees.

INTRODUCTION: Several studies have found changes in the organization of the primary somatosensory cortex (SI) after amputation. This SI reorganization was mainly investigated by stimulating neighboring areas to amputation. Unexpectedly, the somatosensory representation of the deafferented limb has rarely been directly tested. METHODS: We stimulated the truncated peroneal nerve in 24 unilateral transtibial amputees and 15 healthy controls. The stimulation intensity was adjusted to make the elicited percept comparable between both stimulation sides. Neural sources of the somatosensory-evoked ma…

Extramedullary Osseointegration-A Novel Design of Percutaneous Osseointegration Prosthesis for Amputees.

The percutaneous osseointegrated (OI) prostheses have greatly improved the overall quality of life for amputees. However, the long-term maintenance of the OI prostheses is still challenging. A major problem is bone resorption around the bone-implant-skin interface, which might cause implant loosening or osteomyelitis. Another problem is the breakage of connecting components between the intramedullary implant and external prosthesis due to excessive stress. We designed a novel osseointegration implant by changing the bone-implant contact from the inner cortex to the outer surface of cortical bo…

Standing posture and balance modalities in unilateral transfemoral and transtibial amputees.

INTRODUCTION: Lower limb amputation impairs postural performance that could be characterized by biomechanical parameters. This study is to i … ces in body posture were only noted between the non-amputee and the transtibial groups. Transtibial amputees leaned more forwardly their trunk by 3.5° compared to able-bodied (p = 0.028). The mean CoP position in transfemoral amputees was closer to the non-amputated side than transtibial amputees (p = 0.034) and as compared to the dominant side in non-ampu …

Lower limb amputees undergo long-distance plasticity in sensorimotor functional connectivity.

Amputation in adults is associated with an extensive remapping of cortical topography in primary and secondary sensorimotor areas. Here, we used tactile residual limb stimulation and 3T functional magnetic resonance imaging in humans to investigate functional connectivity changes in the sensorimotor network of patients with long-term lower limb traumatic amputations with phantom sensation, but without pain. We found a pronounced reduction of inter-hemispheric functional connectivity between homologous sensorimotor cortical regions in amputees, including the primary (S1) and secondary (S2) soma…

Perioperative and early rehabilitation outcomes following osseointegration in UK military amputees.

INTRODUCTION: This paper examines the pain management, from surgery to specialist rehabilitation, of the first seven military transfemoral amputee patients treated in the UK with femoral osseointegration. All the patients had sustained complex ballistic injuries on the battlefield. The patients were characterised by long-standing problems with functional rehabilitation due to limitations with conventional prostheses, including stump soft tissue issues and impaired biomechanics. METHODS: A prospective service investigation was undertaken to evaluate the effectiveness of the pain management of

A systematic review of shock-attenuating componentry for lower limb amputees.

BACKGROUND: Shock-attenuating pylons are commonly fitted to prostheses in order to compensate for the anatomical and biomechanical shock-absorbing features of the lower limb removed upon amputation. However, studies concerning their shock-attenuating capacity are highly variable and, to date, have not yet been reviewed, making them difficult to interpret and apply in clinical practice. OBJECTIVES: To synthesise and appraise the available literature examining the effectiveness of shock-attenuating pylons in attenuating shock upon limb loading compared to rigid pylons among lower limb amputees.…

Incidence and morbidity of concomitant spine fractures in combat-related amputees.

BACKGROUND CONTEXT: High-energy blasts are the most frequent cause of combat-related amputations in Operations Iraqi and Enduring Freedom (OIF/OEF). The nondiscriminating effects of this mechanism often result in both appendicular and axial skeletal injuries. Despite this recognized coincident injury pattern, the incidence and consequence of spine fractures in trauma-related combat amputees are unknown. PURPOSE: This study sought to determine the incidence and morbidity of the associated spine fractures on patients with traumatic lower extremity amputation sustained during OIF/OEF. STUDY DES…

High-resolution ultrasound findings in the symptomatic residual limbs of amputees.

INTRODUCTION: Amputations are increasingly prevalent because of medical complications associated with diabetes, meningitis, peripheral vascular disease, and neoplasms as well as road traffic accidents and war. The use of ultrasound (U/S) to diagnose the cause of pain in the residual limb of amputees is presented together with a review of the literature. PATIENTS AND METHODS: 133 civilian patients with one or more amputations were included in the study. They were seen over a 2-year period, at the Douglas Bader Unit, Queen Mary's Hospital, Roehampton, Surrey, United Kingdom, in an outpatient se…
