NOS3 Gene rs1799983 and rs2070744 Polymorphisms in Patients with Unstable Angina.

Acute coronary syndrome occurs when the heart muscle does not receive adequate oxygen and nutrients in a timely manner. Acute coronary syndromes are primarily due to atherosclerosis of the coronary arteries, i.e., coronary heart disease. Nitric oxide (NO) is synthesised from L-arginine in endothelial cells by the constitutive calcium-calmodulin-dependent enzyme, nitric oxide synthase (NOS), which mediates endothelium-dependent vasodilatation. Endothelial nitric oxide synthase (eNOS) is predominantly expressed in endothelial cells. Three NOS isoforms have been detected in different tissue: (1)

Transient Giant R Wave as a Marker for Ischemia in Unstable Angina.

Unstable angina is a clinical diagnosis that may present with or without electrocardiographic changes. The "giant R wave" on electrocardiogram has been reported as a manifestation of acute ischemia; however, it is a rare finding in current clinical practice. We describe a case of a patient with unstable angina and a transient "giant R wave" pattern with a culprit lesion in the right coronary artery.

Monocyte subsets and monocyte-platelet aggregates in patients with unstable angina.

Monocyte subsets and monocyte-platelet aggregates (MPAs) play important role in atherosclerosis and thrombosis. We aimed to determine their changes in patients with unstable angina (UA). In this cross-sectional case-control study, Global Registry of Acute Coronary Events (GRACE) score was determined in 95 UA patients without elevated troponin level. Thirty age-and-sex matched stable coronary heart disease (CHD) subjects served as control group. The classical (CD14++CD16-, Mon1), the intermediate (CD14++CD16+, Mon2) and the non-classical (CD14+CD16++, Mon3) monocytes, as well as subset-specific

Lipoprotein(a) Is the Best Single Marker in Assessing Unstable Angina Pectoris.

This study evaluated whether statin therapy changed a diagnostic validity of lipid and inflammatory markers in ischemic heart disease (IHD) patients. Levels of lipids, lipoproteins, apolipoproteins, inflammatory markers, and atherogenic indexes were determined in 49 apparently healthy men and women, 82 patients having stable angina pectoris (SAP), 80 patients with unstable angina (USAP), and 106 patients with acute ST-elevation myocardial infarction (STEMI) treated or not treated with statins. Diagnostic accuracy of markers was determined by ROC curve analysis. Significantly lower apoA-I in al

Effect of simvastatin on plasma interleukin-6 in patients with unstable angina.

PURPOSE: The primary aim of the study was to investigate the effect of sinvastatin on plasma interleukin-6 (IL-6) in patients with unstable angina pectoris (UAP). METHODS: Eighty-six patients with UAP were randomized into simvastatin (40 mg/d for 4 weeks) and the placebo group. Plasma IL-6 was measured by ELISA. RESULTS: There was a reduction in the plasma total cholesterol and LDL in the simvastatin group (P < 0.01). The simvastatin group also had better angina control than the placebo group (post-treatment angina score, 0.72+/-0.59 vs 1.07+/-0.76, P < 0.05). Following treatment, the averag

[Value of the laboratory markers of endothelial damage in unstable angina pectoris].

Patients with the worse clinical picture of ischemic heart disease were found to have a significantly high concentration of endothelin-1 (p < 0.05) both within 24 hours of observation and within 4 months after. The baseline level of C-reactive protein was significantly high (p < 0.05) throughout the study. The plasma level of the protein associated with pregnancy (p < 0.05) and homocysteine (p < 0.01) were significantly reduced.

Clinical significance of aortic knob width and calcification in unstable angina.

BACKGROUND: Chest radiography is a routine examination evaluating those patients with chest pain. There are few data about the correlation between aortic knob width, calcification and coronary atherosclerosis. METHODS AND RESULTS: The aortic knob width was measured and the presence of aortic knob calcification was assessed via a chest posteroanterior view in 178 consecutive patients. The aortic knob width and calcification were compared to the risk factor and the extent of coronary artery disease. Patient's age (69.5+/-7.95 vs 61.1+/-10.29 years, p=0.010), the prevalence of hypertension (65.9

Thrombopoietin contributes to enhanced platelet activation in patients with unstable angina.

OBJECTIVES: We sought to investigate the potential role of elevated levels of thrombopoietin (TPO) in platelet activation during unstable angina (UA). BACKGROUND: Thrombopoietin is a humoral growth factor that does not induce platelet aggregation per se, but primes platelet activation in response to several agonists. No data concerning its contribution to platelet function abnormalities described in patients with UA are available. METHODS: We studied 15 patients with UA and, as controls, 15 patients with stable angina (SA) and 15 healthy subjects. We measured TPO and C-reactive protein (CRP)

Unstable angina with normal coronary angiography in hyperthyroidism: a case report.

Hyperthyroidism is associated with an increase in myocardial oxygen consumption that, due to an imbalance of oxygen demand and supply, can cause angina. However, subclinical hyperthyroidism rarely presents as chest pain in the resting state. Herein, we present a case of subclinical hyperthyroidism involving a 58-year-old male who complained of frequent chest tightness and typical electrocardiographic changes while in a resting state. Coronary angiography showed no significant lesion. Laboratory data showed that the patient suffered from hyperthyroidism, for which he was successfully treated wi

Platelet and leukocyte adhesion and activation in unstable angina and post-PTCA.

BACKGROUND: Unstable atherosclerotic plaques activate blood cells which may adhere to the coronary endothelium causing vessel occlusion. However, it is unknown if different clinical syndromes associated with plaque rupture induce similar blood cell activation and adhesion to the endothelium. METHODS: We studied changes in adhesion molecule expression of platelets (GPIIb/IIIa), neutrophils--CD18, CD11b and L-selectin--and monocytes (CD14) after interaction with active lesions of patients with stable angina subjected to PTCA and patients with unstable angina (UA). Generation of superoxide (SO)

Myocardial infarction and unstable angina: gender differences in therapy and outcomes.

In the United States, coronary heart disease (CHD) is the leading cause of death for women. Combined with hypertension, stroke, and other vascular conditions, one in every two women dies annually owing to a cardiovascular condition. During the past 20 years, overall death rate from CHD has declined, but the rate of decline has been lower for women compared with men. Trials and studies have demonstrated that CHD might be preventable; however, optimal treatment strategies for women are still in question. It is, therefore, essential that the medical profession continues its current efforts to foc

[Lectin-dependent platelet aggregation and agglutination in patients with unstable angina].

It is necessary to study the membrane receptors of platelets to understand the mechanism of their aggregation for effective treatment of patients with ischemic heart disease (IHD). The aim of this paper is to estimate the significance of carbohydrate residues of the glycoprotein receptors of platelet membranes and thrombogenesis in patients with unstable angina (UA). A total of 142 patients with UA and 20 healthy individuals were examined. The carbohydrate structure of platelet membranes was studied using lectins. Lectin-dependent platelet aggregation in patients with US was found to be associ

Inflammatory response to coronary stent implantation in patients with unstable angina.

Previous evidence has shown that coronary angioplasty leads to the release of inflammatory mediators. In this study, we sought to characterize the systemic inflammatory response after coronary stent implantation in patients with unstable angina by measuring different protein markers. Peripheral blood samples were taken before and 24 h, 48 h, and 7 days after successful coronary stenting in 58 patients. Several markers of acute-phase response were determined: C-reactive protein (CRP), alpha2-macroglobulin, haptoglobin, acid alpha1-glycoprotein, prealbumin and albumin. Besides, proinflammatory c

[Use of heparin in unstable angina and non-Q-wave infarction].

The limitations of conventional treatment by non-fractionated heparin (NFH) in unstable angina and non-Q wave infarction with a serious adverse event rate (infarction and/or death) of 7 to 9% at 30 days have led to research in the use of low molecular weight heparin (LMWH). In 1995, Gurfinkel et al reported the superiority of the association aspirin-LMWH (Nadroparine) over a more classical treatment with aspirin alone or the association of aspirin-NFH in unstable angina and non-Q wave infarction. In 1996, the FRISC trial confirmed the value of LMWH (Dalteparine). However, this trial compared t

The definitions of acute coronary syndrome, myocardial infarction, and unstable angina.

The acute coronary syndrome encompasses a spectrum of conditions that include acute myocardial infarction, unstable angina pectoris, and, to some extent, sudden cardiac death. Recently, the diagnosis of myocardial infarction has been redefined by The Joint European Society of Cardiology/American College of Cardiology Committee. However, the conceptual meaning of the term myocardial infarction has not been changed. Thus, the current diagnoses of myocardial infarction as well as of unstable angina are clinical syndromes based on symptoms, electrocardiogram, and sensitive biochemical markers.
