Lateral ligament injuries of the ankle. Surgical treatment of late cases.

34 ankles with prolonged instability of the lateral ligaments were operated on by the Evans tenodesis procedure. The follow-up period was 24 to 35 months. 33 patients showed subjective improvement, only one had marked postoperative complaints. It was observed that the Evans operation gives good stability of the ankle. As a simple and safe procedure this operation is to be recommended for standard correction of old posttraumatic lateral instability of the ankle joint.

[Fresh ligament injuries at upper ankle joint and foot (author's transl)].

Crush injuries and abnormal pronation and supination are the most common reasons for typical ligament tears in this region. Most of them should be treated conservatively by plaster of Paris and early weight-bearing after exact clinical and X-ray diagnosis. At upper ankle joint, suture of ruptured fibular collateral ligaments should be favoured. For dislocations and fracture dislocations of the Lisfranc type operative reposition and K-wiring is recommended. Our treatment results with fibular collateral ruptures and fracture dislocations of the Lisfranc type are presented.

[Ligamentous ankle injury: an underestimated trauma?].

Ligamentous injuries of the ankle joint are among the most common injuries in sports, with landing on the opponent's foot or direct contact counting among the most common injury mechanisms. Initial measures most notably include a clinical examination, supplemented by X-rays, sonography or MRI as required. In most cases, conservative treatment is the treatment of choice. At the beginning, pain and swelling have to be reduced. Then function must be regained before patients may return to sports after a return-to-play test. Some injury patterns are more complex with concomitant injuries, and despi

Diagnostics in suspicion of ankle syndesmotic injury.

Ankle sprains are among of the most common injuries seen in daily orthopaedic practice. Beside injuries of the lateral ligament, which is the most frequently injured single structure in the body of athletes, sprains can also affect the tibiofibular syndesmosis. These injuries are known as high ankle sprains. They can occur with or without a bony injury. In this report, we will discuss the high ankle sprain without any bony injury. These kinds of injuries are rare, but often associated with complications and are frequently misdiagnosed or undiagnosed. The purpose of this article is to provide a

Hormonal therapy: ACL and ankle injury.

BACKGROUND: No definitive explanation for the difference in rate of male and female noncontact ACL injury has been found. The hormonal environment, known to be different in men and women has been hypothesized as a possible source for this difference in injury rate. PURPOSE: To confirm earlier work looking at periodicity of noncontact ACL injury. To increase sample size by adding ankle sprains. To determine the rate of noncontact ACL injury and ankle sprains in collegiate basketball and soccer. To determine if the use of oral contraceptives affects the rate of noncontact ACL injury and ankle s

An unusual ankle injury in top skiers.

Although skiers' feet are immobilized in extremely rigid boots, ligamentous lesions of the ankle may occur, especially in world class slalom skiers. The lesion arises when a sudden external rotation of the ankle causes the talus to press against the fibula and thus open the distal tibiofibular articulation. This study reports on a group of 10 skiers recruited between 1975 and 1986. These are professional skiers participating in World Cup races. Three patients underwent an operation for lesion of the tibiofibular syndesmosis, while the other seven were treated conservatively. All were able to r

Inverse Relationship Between Radiographic Lateral Ankle Instability and Osteochondral Lesions of the Talus in Patients With Ankle Inversion Injuries.

BACKGROUND: Insufficient or excessive bony constraint surrounding the talus might contribute to the occurrence of ligamentous injury or bone contusion, respectively, at the time of ankle inversion injuries. This study aimed to investigate the relationship between radiographic lateral ankle instability and osteochondral lesions of the talus (OLT) following ankle inversion injuries. METHODS: A total of 195 patients (113 men and 83 women; mean age, 38.7 years) with a history of ankle inversion injuries were included in this study. All patients underwent ankle magnetic resonance imaging (MRI) and

Clinical assessment of acute lateral ankle sprain injuries (ROAST): 2019 consensus statement and recommendations of the International Ankle Consortium.

Lateral ankle sprain injury is the most common musculoskeletal injury incurred by individuals who participate in sports and recreational physical activities. Following initial injury, a high proportion of individuals develop long-term injury-associated symptoms and chronic ankle instability. The development of chronic ankle instability is consequent on the interaction of mechanical and sensorimotor insufficiencies/impairments that manifest following acute lateral ankle sprain injury. To reduce the propensity for developing chronic ankle instability, clinical assessments should evaluate whether

Nurse practitioners treating ankle and foot injuries using the Ottawa Ankle Rules: a comparative study in the emergency department.

BACKGROUND: Nurse practitioners (NPs) in the Emergency Department (ED) have been trained to assess a range of clinical problems and minor complaints such as acute ankle injury. This study compared assessment of suspected ankle and foot injuries using the Ottawa Ankle Rules (OAR) by NPs and ED medical doctors (ED-Drs). METHODS: A prospective, comparative, observational study was undertaken in an Australian acute adult and paediatric urban district ED. NPs and ED-Drs recorded information for patients with acute ankle and/or mid-foot injuries on demographic characteristics, OAR features, use of

Concomitant lisfranc injuries in the setting of ankle fractures: A case series.

Lisfranc injuries are rare and commonly misdiagnosed, which can lead to severe complications. This paper presents patients with concomitant Lisfranc injuries and ankle fractures. We report 3 cases. Case 1 is a 14-year-old girl with trimalleolar fractures and Lisfranc dislocation who achieved a full recovery. Case 2 is a 55-year-old woman with complex Lisfranc and ankle fractures, where delayed intervention resulted in persistent instability and pain. Case 3 is a 62-year-old woman with a previously missed Lisfranc injury with an ankle fracture that was fixed with full recovery after correct dia

Nurse-initiated radiographic-test protocol for ankle injuries: A randomized controlled trial.

INTRODUCTION: Nurse-initiated radiographic-test protocol was compared with usual practice in reducing unnecessary ankle and foot radiographic-test requests and shortening patients' length of stay (LOS) in an emergency department (ED) by reducing their waiting time for physician reassessment. METHODS: Patients with ankle injuries were enrolled in an unblinded randomized controlled trial. Participants were randomized to receive either the protocol (n = 56) or usual practice (n = 56). Primary outcome was the proportion of radiographic-test requests. Secondary outcomes were the proportion of frac

A Novel Indirect Reduction Technique in Ankle Syndesmotic Injuries: A Cadaveric Study.

OBJECTIVE: To describe a novel technique using preoperative computed tomography (CT) to plan clamp tine placement along the trans-syndesmotic axis (TSA). We hypothesized that preoperative CT imaging provides a reliable template on which to plan optimal clamp tine positioning along the TSA, reducing malreduction rates compared with other described techniques. METHODS: CT images of 48 cadaveric through-knee specimens were obtained, and the TSA was measured as well as the optimal position of the medial clamp tine. The syndesmosis was then fully destabilized. Indirect clamp reductions were perfor

On equal footing: Trends in ankle/foot injuries for men vs. women.

OBJECTIVES: The objective of the current study was to examine trends in ankle/foot (A/F) injuries during the period 2001-2014, in order to determine whether the incidence of these injuries has changed and whether a previously identified difference in risk by gender still existed. In addition, other driver and crash-related risk factors were examined separately for men and women. METHODS: Passenger vehicle drivers aged 16+ were identified from NASS-CDS; weighted data were analyzed for model years 2001-2014. Model years (MY) were grouped as 2001-2004 (older) vs. 2005-2014 (newer), and drivers i
