Objective improvement in adults with cerebellopontine angle arachnoid cysts after surgical treatment.

BACKGROUND: Intracranial arachnoid cysts are extra-axial benign lesions mainly found in the middle cerebral fossa. Rare case series report various cranial nerve dysfunctions associated with cerebellopontine angle (CPA) cysts and there is no consensus with regard to their surgical management; some reports claiming that subjective improvement in adults with intracranial arachnoid cysts cannot justify surgical treatment. METHODS: This retrospective study included all 12 consecutive adult patients treated by microsurgical fenestration for symptomatic CPA arachnoid cysts between 2010 and 2019 and

Suprasellar arachnoid cysts in adults: clinical presentations, radiological features, and treatment outcomes.

A tendency for suprasellar arachnoid cysts (SACs) to occur in young children is known. Data of adult SACs were rare in previous reports. The aim of this study is to discuss their clinical presentations, radiological features, and treatment outcomes based on 23 adult patients who underwent endoscopic fenestration in our hospital between January 2003 and December 2018. Preoperative cyst volume ranged from 12.3 to 72.5 cm3 (mean 39.8 ± 19.8). Endocrine disorders occurred in 7 (30.4%) patients. Hydrocephalus was observed in 20 patients. In the patients with hydrocephalus, the mean preoperative Eva

Spinal arachnoid cysts in adults: diagnosis and management. A single-center experience.

OBJECTIVEAdult spinal arachnoid cysts (SACs) are rare entities of indistinct etiology that present with pain or myelopathy. Diagnosis is made on imaging studies with varying degrees of specificity. In symptomatic cases, the standard treatment involves surgical exploration and relief of neural tissue compression. The aim of this study was to illustrate features of SACs in adults, surgical management, and outcomes.METHODSThe authors searched medical records for all SACs in adults in the 10-year period ending in December 2016. Radiology and pathology reports were reviewed to exclude other spine c

Does subjective improvement in adults with intracranial arachnoid cysts justify surgical treatment?

OBJECTIVE Subjective improvement of patients who have undergone surgery for intracranial arachnoid cysts has justified surgical treatment. The current study aimed to evaluate the outcome of surgical treatment for arachnoid cysts using standardized interviews and assessments of neuropsychological function and balance. The relationship between arachnoid cyst location, postoperative improvement, and arachnoid cyst volume was also examined. METHODS The authors performed a prospective, population-based study. One hundred nine patients underwent neurological, neuropsychological, and physiotherapeuti

Surgery for intracranial arachnoid cysts in children-a prospective long-term study.

PURPOSE: Intracranial arachnoid cysts are cystic malformations found in both adults and children. While many are asymptomatic, some cause symptoms and warrant surgical treatment. In this prospective population-based study, we aimed to study the short- and long-term outcome after surgical intervention in children with arachnoid cysts referred to our centre. METHODS: Twenty-seven pediatric patients (13 f. 14 m, mean age 9.4 years) with de novo cysts were consecutively included during a 5-year period. The presenting symptoms were headache (n = 12), balance disturbance and dizziness (n = 6), seiz

Bilateral temporal lobe agenesis with bilateral arachnoid cysts - a rare congenital anomaly.

BACKGROUND: Bilateral temporal lobe agenesis/hypogenesis along with middle cranial fossa arachnoid cysts (ACs) is extremely rare, and very few cases have been reported in the literature. METHODS: We present the case of 2-year-old female presenting with chief complaints of headache, vomiting, and fever. There was a history of premature delivery in the seventh month and up-rolling of eyes since birth. RESULTS: Magnetic resonance imaging findings revealed bilateral temporal lobe agenesis with bilateral ACs, along with encephalomalacia in both parietal lobes. Ultrasound abdomen findings and bioc

Arachnoid cysts confined to the internal auditory canal or facial nerve canal.

OBJECTIVE: To illustrate the morphological characteristics and pathological significance of arachnoid cysts confined to the internal auditory canal or facial nerve canal. DESIGN: Retrospective case series. SUBJECTS: Three patients' cases were reviewed, and temporal bone sections examined histologically. RESULTS: In this series, three intracanalicular arachnoid cysts were found in the internal auditory canal and one in the facial nerve canal. All lesions consisted of distinct, space-occupying cysts with a sharp boundary with surrounding tissues. They were thin-walled and compressed the surro

Multiple extradural spinal arachnoid cysts causing diffuse myelomalacia of the spinal cord.

INTRODUCTION: Extradural spinal cyst is a rare cause of compression myelopathy. It is usually solitary and its typical location is posterior to the spinal cord. We present a case of multiple spinal arachnoid cysts causing diffuse myelomalacia secondary to a significant compression of the spinal cord with no symptom relief after surgical decompression. CASE REPORT: A 35-year-old female patient presented to our hospital complaining of progressive weakness and numbness of both lower extremities for the last 2 months, being more prominent on the right side. Her history was significant for back pa

Pre- and postnatal MR imaging of hypothalamic hamartomas associated with arachnoid cysts.

We describe two cases of hypothalamic hamartoma associated with arachnoid cysts. One case was initially documented on prenatal MR images. Because of the rarity of the association and resultant distortion in regional anatomy, the solid component of the mass may be overlooked. This would certainly be true when using lower-resolution diagnostic studies such as fetal MR imaging. The lesion could also be confused with a cystic tumor such as pilocytic astrocytoma. Thorough evaluation is required in patients with precocious puberty, gelastic seizures, and the presence of a suprasellar arachnoid cyst.

Diagnosis of perineurial arachnoid cysts using computed tomography: technical and clinical considerations.

Perineurial arachnoid cysts are often multiple lesions that are usually found in the lumbosacral area. These cysts may cause compression of surrounding structures. Examination of the first, second and third sacral nerves should be performed on patients with prolonged symptoms of a herniated disc or cauda equina syndrome. Three cases are presented for which computed tomography provided accurate, noninvasive diagnosis. Perineurial cysts are often associated with asymptomatic herniated disc; thus, surgery on the disc with an underlying symptomatic cyst may result in a failed back syndrome.

[Sub, retro- and supracerebellar arachnoid cysts and pseudocyts in adults. Six observations].

Four arachnoïd cysts and two arachnoïd communicating pouchs in sub, retro and supra cerebellar situation are presented. These formations are rarely encountered in adults and distinction is often difficult from other cystic lesions whose structure is different (neuroglial and ependymal cysts). Intellectual slowing, mild ataxia or urinary disturbances, related to chronic hydrocephalus may be the only clinical symptoms. After computed tomography, isotopic cysternography, rather than metrizamide cysternography, must precise the possible communication between the cystic pouch and the subarachnoïd s

Absent enhancement interface sign: a novel radiologic indicator of intrasellar arachnoid cysts.

PURPOSE: Intrasellar arachnoid cysts are extremely rare, and their differentiation from other cystic lesions can be challenging because of their similar imaging characteristics. METHODS: This study aimed to describe the imaging characteristics observed in five patients and correlate them with developmental and growth mechanisms. RESULTS: These cysts are characterized by the lack of contrast enhancement in their walls and posterior displacement of the pituitary gland. As a result, no enhancing tissue is present at the anterior portion of the sella where it interfaces with the sphenoid sinus.

Surgical management of quadrigeminal cistern arachnoid cysts: case series and literature review.

OBJECTIVE: Quadrigeminal cistern arachnoid cysts (QACs) are congenital lesions that can cause pineal region compression and obstructive hydrocephalus when sufficiently large. Management of these cysts is controversial and rates of reintervention are high. Given the limited data on the management of QACs, the authors retrospectively reviewed 20 years of cases managed at their institution and performed a literature review on this topic. METHODS: The authors performed a retrospective analysis of patients treated for QAC at their institution between 2001 and 2021. They also performed a literature

Endoscopic Management of Lateral Ventricle Arachnoid Cysts-Case Series with Proposed Classification.

BACKGROUND: We present a case series of 7 patients with intraventricular arachnoid cysts of lateral ventricle managed with endoscopic treatment with proposed classification of the cyst based on anatomic extent. METHODS: In all patients, the lateral ventricle arachnoid cyst was primarily located in the trigone and body of the lateral ventricle. Proposed classification is based on extension of the arachnoid cyst. Type 1 is an arachnoid cyst located in the lateral ventricle only, type 2 is a lateral ventricle arachnoid cyst extending to the quadrigeminal cistern, and type 3 is the lateral ventri

Review/Perspective On the Diagnosis and Surgical Management of Spinal Arachnoid Cysts.

BACKGROUND: Spinal arachnoid cysts (SAC) are typically congenital, spontaneous, traumatic (i.e., including iatrogenic/surgical), or inflammatory in origin. In descending order, they occur in the thoracic, lumbar, and cervical spine, and originate from focal entrapment of the arachnoid membrane. Arachnoid cysts represent 1-2% of all cystic spinal masses/tumors. The majority are extradural arachnoid cysts (EDAC) while 10% of all arachnoid cysts are intradural (IDAC) including subarachnoid, or extra-arachnoidal/subdural. Only rarely are they intramedullary in location. The clinical symptoms/signs

Endoscopic Third Ventriculostomy for Obstructive Hydrocephalus and Ventriculocystostomy for Intraventricular Arachnoid Cysts.

OBJECTIVE: To evaluate and discuss the outcomes of a combination of ventriculocystostomy (VC) and endoscopic third ventriculostomy (ETV) for obstructive hydrocephalus (HCP) due to ventricular/cisternal arachnoid cysts, and only ETV for obstructive HCP due to different etiologies. METHODS: We retrospectively reviewed all 40 symptomatic patients (aged 4 months - 61 years) of obstructive HCP treated by ETV or VC+ETV during October 2014 - April 2019. VC+ETV was performed in 7 patients with intraventricular/cisternal arachnoid cyst and obstructive HCP. Only ETV was performed in 33 patients with ob

Anxiety and Depression in Patients with Intracranial Arachnoid Cysts-A Prospective Study.

BACKGROUND: Arachnoid cysts yield cognitive deficits that are normalized after surgical cyst decompression. OBJECTIVE: The present study aimed to investigate whether arachnoid cysts also affect symptoms of anxiety and depression, and if surgical cyst decompression leads to reduction of these symptoms. METHODS: Twenty-two adult patients (13 men and 9 women) with symptomatic temporal or frontal cysts were included in this questionnaire (Hospital Anxiety and Depression Scale [HADS])-based prospective study. The mean time between answering the preoperative questionnaire and surgery was 37 days.

The Transcondylar Approach to Access Symptomatic Arachnoid Cysts of the Hypoglossal Canal.

BACKGROUND AND IMPORTANCE: A direct transcondylar approach to treat symptomatic arachnoid cysts of the hypoglossal canal has not yet been described in the literature. CLINICAL PRESENTATION: Here, we present a skull base approach in 2 female patients (68 and 38 yr) with progressive dysphagia, dysarthria, half-sided weakness, and atrophy of the tongue due to an arachnoid cyst of the hypoglossal canal. After patient informed consent, both patients were successfully operated on without complications using a transcondylar approach, and their symptoms improved 3 mo after surgery; in both patients,
