OBJECTIVE: Extensive research has demonstrated high prevalences of post-traumatic stress disorder (PTSD) and major depression (MD) in war-surviving populations. However, absolute estimates are lacking, which may additionally inform policy making, research and healthcare. We aimed at estimating the absolute global prevalence and disease burden of adult survivors of recent wars (1989-2019) affected by PTSD and/or MD.
METHODS: We conducted a systematic literature search and meta-analysis of interview-based epidemiological surveys assessing the prevalence of PTSD and/or MD in representative sampl

Artemisinin partial resistance (ART-R) has now emerged in the Horn, Eastern and Southern Africa. Mutations in the Plasmodium falciparum kelch13 gene, strongly associated with ART-R, are increasingly reported in several Central and West Africa countries. Here, we opine that the emergence and spread of ART-R in Central and West Africa should not be overlooked given public health, clinical and economic consequences. Again, in addition to the recent funding cuts from the United States (US) government, some regions in these countries are affected by armed conflicts that undoubtedly will disrupt mal…

Infections are well-known complications in patients following traumatic injuries, frequently leading to high morbidity and mortality. In particular, trauma occurring in disaster settings, both natural and man-made, such as armed conflicts and explosives detonation, results in challenging medical conditions that impede the best management practices. The incidence of invasive fungal infections (IFI) is increasing in trauma patients who lack the typical risk factors like an immune compromised state or others. This narrative review will focus on IFI as a direct complication after natural disasters…

A body of a 91-year-old donor underwent whole-body CT scanning before being transferred for use in an anatomical dissection course. Metallic … suffered for almost 70 years from this violation of the Geneva Conventions. In the light of current armed conflicts, the present case is an example of how one injustice can result in severe, lifelong medical consequences. It also shows that the history behind a wounded thigh can have an enormous impact on teaching ethics to today's medical students.

Avalanche accidents are frequently lethal events with an overall mortality of 23%. Mortality increases dramatically to 50% in instances of complete burial. With modern day dense networks of ambulance services and rescue helicopters, health workers often become involved during the early stages of avalanche rescue. Historically, some of the most devastating avalanche accidents have involved military personnel. Armed forces are frequently deployed to mountain regions in order to train for mountain warfare or as part of ongoing conflicts. Furthermore, military units are frequently called to assist

Vitamin A supplementation is a public health intervention that clinical trials have suggested can significantly improve child survival in the developing world. Yet, prominent scientists in India have questioned its scientific validity, opposed its implementation, and accused its advocates of corruption and greed. It is ironic that these opponents were among the pioneers of populationwide vitamin A supplementation for ocular health. Historically, complex interests have shaped vitamin A supplementation resistance in India. Local social and nutritional revolutions and shifting international parad

Many civilian deaths and injuries across the globe are a consequence of 'explosive remnants of war' (ERW). These ERW include mines, unexploded ordnance (UXO) and cluster bombs. The numbers of deaths, injuries and resulting disabilities from ERW are expected to increase as armed conflicts proliferate. This will have a substantial effect on all aspects of health. This article first describes the mechanisms of these ERW and the nature of the injuries they can cause. It then briefly outlines the immediate medical management such injuries require; the long-term outcomes and sequelae, and what can b…

OBJECTIVE: The aim of this study was to assess the levels, types and barriers of post-conflict restoration of health infrastructure in war-affected health facilities in the Amhara region, Ethiopia.
RESULTS: Out of 180 health facilities, only 5(2.8%) were fully restored in health system infrastructures and service delivery. Eighty (54.4%) generators, 24(20.2%) water supply, 41(23.6%) furniture, 45(46.9%) ambulance and 27(16.2%) buildings infrastructure were not restored. Health information system devices were also damaged during the war. Among 145 health centers with looted or damaged desktop

In conflict settings, public health interventions such as vaccination campaigns and improvements in water, sanitation, and hygiene (WASH) could benefit all parties involved. However, while polio vaccination campaigns frequently succeed in securing humanitarian pauses, WASH initiatives attempting to improve safe water supply and sewage disposal rarely achieve the same outcome. Using the Israel-Gaza conflict as an example, we analyze the factors contributing to the success of polio vaccination campaigns compared to WASH initiatives. We identify four key elements that facilitate the implementatio

The prolonged war in Gaza has led to the near-total collapse of the healthcare system, leaving over two million people without access to essential medical services, clean water, electricity, or basic sanitation. Many hospitals have been bombed or are barely functioning due to extreme constraints. Healthcare personnel are overwhelmed, injured, or forced to work under dangerous conditions. Essential medicines and vaccines are critically lacking. In this context, the risk of large-scale outbreaks of infectious diseases-such as cholera, hepatitis A, measles, and typhoid fever-is rapidly increasing

Formal infectious disease surveillance in Ukraine has been disrupted by Russia's 2022 invasion, leading to challenges with tracking and containing epidemics. To analyze the effects of the war on infectious disease epidemiology, we used open-source data from EPIWATCH, an artificial intelligence early-warning system. We analyzed patterns of infectious diseases and syndromes before (November 1, 2021-February 23, 2022) and during (February 24-July 31, 2022) the conflict. We compared case numbers for the most frequently reported diseases with numbers from formal sources and found increases in overa

The occupational hazards of health workers (HWs) in standard work environments have been well defined in both the developed and developing world during routine working conditions. Less defined are the hazards to HWs during pandemics, epidemics, natural disasters, wars, conflicts, and other crises. How do crises affect the infrastructure of medical systems? What are the distinct needs of the patient population during crises? What are the peculiarities of the Crisis Health Worker (CHW)? What are the known CHWs' occupational risks? What are the protective factors? By means of a PubMed search, we

Ten weeks after the 1941 Japanese military attack on Pearl Harbor, Hawaii, the U.S. government authorized the removal of more than 110,000 Japanese American men, women, and children from their homes in Western portions of the country to incarceration camps in desolate areas of the United States. The mass incarceration was portrayed as necessary to protect the country from potential acts of espionage or sabotage that might be committed by someone of Japanese ancestry. However, an extensive government review initiated in 1980 found no evidence of military necessity to support the removal decisio

This paper explores the management of mass casualty incidents in Eastern Ukraine, focusing on the application of the Eight Domains of Mass Casualty Management by the Ukrainian Medical Service. Following the Russian invasion, Ukraine's military and civilian health services have had to adapt to unprecedented casualty rates to prevent overwhelming the healthcare system. The Eight Domains-distribution, decompress, delay, delegate, deliver faster and deliver better, dynamic levels of care, and de-escalation-serve as compensatory mechanisms to manage this chronic major medical incident. The paper hi

Prolapse of the gravid uterus, which cannot be spontaneously reduced through the anal orifice, is extremely rare. It is considered to be incarcerated and constitutes a clinical and therapeutic emergency, as it endangers fetal and maternal life. However, few cases have been reported in the literature. Because of the rarity of these prolapses of the gravid uterus and their clinical presentation, especially in young women in rural areas with armed conflicts, in the form of a mass protruding from the anal canal with a reduction in the volume of the abdomen, the clinical diagnosis provides guidance…

The Middle East region is plagued with repeated armed conflicts that affect both civilians and soldiers. Injuries sustained during war are common and frequently associated with multiple life-threatening complications. Wound infections are major consequences of these war injuries. The microbiology of war-related wound infections is variable with predominance of Gram-negative bacteria in later stages. The emergence of antimicrobial resistance among isolates affecting war-related wound injuries is a serious problem with major regional and global implications. Factors responsible for the increase …

Blast injury is a frequent cause of injury during armed conflicts, and the force of a blast can cause closed colorectal injury and perforation.1 After identification of a blast-related colorectal injury, the surgical options are primary repair or fecal diversion with the option for secondary repair. This structured review was conducted to determine which patients could be treated with primary repair (PR) or with fecal diversion. The review method followed the Prisma Statement method for medical systematic review. All data from the relevant articles were collected in a single database. Articles…
