Inducing and manipulating sensations of incompleteness.

Incompleteness (INC) and Harm Avoidance (HA) are known core dimensions of obsessive-compulsive disorder (OCD). Emotional processing and inhibitory learning (IL) have both been proposed as viable mechanisms of exposure for the alleviation of HA, yet the relationship with INC remains unclear. This study evaluated differential manipulation responses to induced INC. The authors specifically tested whether targeting the key component of IL, expectancy violation generated by the discrepancy between an articulated prediction and the actual exposure experience, would lead to improved outcomes. A sampl

Magnetic resonance imaging of joint disease.

MRI is capable of demonstrating superb anatomic detail of the appendicular skeleton and articulations. Disorders of subcortical bone, cartilage, ligaments, and synovium are being evaluated by MRI. MRI should become an increasingly important diagnostic tool in a variety of articular disorders as technical improvements are made and clinical experience accumulates.

How much is a word? Predicting ease of articulation planning from apraxic speech error patterns.

BACKGROUND: According to intuitive concepts, 'ease of articulation' is influenced by factors like word length or the presence of consonant clusters in an utterance. Imaging studies of speech motor control use these factors to systematically tax the speech motor system. Evidence from apraxia of speech, a disorder supposed to result from speech motor planning impairment after lesions to speech motor centers in the left hemisphere, supports the relevance of these and other factors in disordered speech planning and the genesis of apraxic speech errors. Yet, there is no unified account of the struc

Overall intelligibility, articulation, resonance, voice and language in a child with Nager syndrome.

OBJECTIVE: The purpose of this study was to provide a description of the language and speech (intelligibility, voice, resonance, articulation) in a 7-year-old Dutch speaking boy with Nager syndrome. To reveal these features comparison was made with an age and gender related child with a similar palatal or hearing problem. METHODS: Language was tested with an age appropriate language test namely the Dutch version of the Clinical Evaluation of Language Fundamentals. Regarding articulation a phonetic inventory, phonetic analysis and phonological process analysis was performed. A nominal scale wi

Feature analysis of segmental errors in children with phonological disorders.

There has been a longstanding controversy about the existence, nature, and differentiation of developmental apraxia of speech (DAS), leading to numerous investigations of characteristics that define this articulatory disorder. An analysis of substitutions relative to target sounds led Thoonen, Maassen, Gabreëls, and Schreuder (1994) to conclude that children with DAS show a pattern of feature retention in their error productions that contrasted with that of children with normal articulation. This pattern, in which place of articulation was retained in the substituted sound less frequently than

The Effect of Mixed Articulation Therapy on Perceptual and Acoustic Features of Compensatory Errors in Children with Cleft Palate.

BACKGROUND: Compensatory errors are a conventional part of an articulation disorder identified by speech pathologists in patients with Cleft palate (CP). This study aimed to evaluate the effect of new mixed articulation therapy on the perceptual and acoustic features of these errors. METHODS: The single-case experimental design, ABA design, was used in this study. Five CP young children ages 3.9 to 5.4 months, received online multimodal articulation therapy for 5 weeks, 6 sessions per week, utilizing both standard and maximum opposition approaches. Patients underwent baseline and follow-up se

Bilateral talipes equinovarus from Tikal, Guatemala.

An incomplete skeleton recovered from a multiple, secondary burial at Tikal, Guatemala, shows malformed foot bones consistent with a diagnosis of bilateral idiopathic talipes equinovarus. Bones attributable to the skeleton include paired bowed fibulae, fragmentary calcanei, complete tali, naviculars, cuneiforms, metatarsals, and some phalanges. The tali are reduced in size, flattened, and hyperextended, with the tibia partially articulated on the calcaneus, posterior to the talus. The cuboid and cuneiforms show marked contraction of the inferior surfaces, and angulation. The metatarsals and ph

Facial-muscle weakness, speech disorders and dysphagia are common in patients with classic infantile Pompe disease treated with enzyme therapy.

Classic infantile Pompe disease is an inherited generalized glycogen storage disorder caused by deficiency of lysosomal acid α-glucosidase. If left untreated, patients die before one year of age. Although enzyme-replacement therapy (ERT) has significantly prolonged lifespan, it has also revealed new aspects of the disease. For up to 11 years, we investigated the frequency and consequences of facial-muscle weakness, speech disorders and dysphagia in long-term survivors. Sequential photographs were used to determine the timing and severity of facial-muscle weakness. Using standardized articulati

Follow-up of children with early expressive phonology disorders.

Fifty-two children identified at age 4 to 6 years as demonstrating a moderate to severe expressive phonology disorder were followed to the third and fourth grades. Children were classified into two groups based on the presence of an early phonology disorder in isolation (P) or the presence of a phonology disorder with other language problems (PL). At follow-up, articulation measures failed to differentiate the groups; however, the PL group performed more poorly than the P group on measures of phoneme awareness, language, reading decoding, reading comprehension, and spelling. The P group demons

Auditory-perceptual acuity in rhotic misarticulation: baseline characteristics and treatment response.

The rhotic sound /r/ is one of the latest-emerging sounds in English, and many children receive treatment for residual errors affecting /r/ that persist past the age of 9. Auditory-perceptual abilities of children with residual speech errors are thought to be different from their typically developing peers. This study examined auditory-perceptual acuity in children with residual speech errors affecting /r/ and the relation of these skills to production accuracy, both before and after a period of treatment incorporating visual biofeedback. Identification of items along an /r/-/w/ continuum was

Exploring hypotheses about phonological awareness, memory, and reading achievement.

Reading-related skills were tested in adults with and without reading disabilities (RD) and adults with profound hearing impairment (PHI) who did not differ on average Performance IQ. The RD group and the PHI group both demonstrated levels of phonological awareness lower than the control group's, but only the RD group also exhibited deficits in other skills, including verbal short-term memory, morphological awareness, speeded written naming, and reading comprehension. The average reading level of the PHI group was significantly higher than that of the RD group. Three control group members also

The concomitant problems of young stutterers.

This investigation was designed to survey a sample of 650 speech-language pathologists providing services in elementary schools and to obtain information on the concomitant problems of stutterers in their caseloads. The results of the survey revealed that 32% of all the young stutterers were free of any other problems. The remaining 68% were reported as having some other type of problem (articulation, language, voice, emotional disturbance, neurological handicap, etc.) in addition to their stuttering. Results also present the type of therapy received by each subgroup. Limitations of the survey

Acoustic markers associated with impairment in language processing in Alzheimer's Disease.

This study broaches in a novel way the analysis of cognitive impairment characteristic of the early stages of Alzheimer's Disease (AD). Specifically, we attempt to determine the acoustic speech parameters that are sensitive to the onset of the disease, and their association with the language deficit characteristic of AD. Speech analysis was carried out on 21 elderly patients with AD using Praat software, which analyzes the acoustic components of speech. The data obtained were subjected to stepwise regression, using the overall scores obtained in the test as the criterion variable, and the scor

Clinical application of palatal lift appliance in velopharyngeal incompetence.

The presence of nasal air leak in cleft palate patients with velopharyngeal incompetence leads to characteristic nasal snort. The efficacy of the palatal lift appliance in patients with adequate velopharyngeal tissue with incompetence was tested. Speech quality improved after the wearing of palatal lift appliance. Palatal lift appliances are simple and efficient in reducing the nasal air leak. Ongoing speech therapy is necessary and advised for patients receiving palatal lift.

From phonological therapy to phonological awareness.

Children with speech difficulties often have delayed phonological awareness development and associated literacy problems. Speech-language pathologists (S-LPs) typically use phonological and articulatory approaches in their treatment of such children. However, it is unclear to what extent phonological awareness training, originally designed to promote literacy skills, might also improve children's speech output. This article adopts a psycholinguistic approach to examine the nature and development of phonological awareness and to explore the relationship between phonological awareness training a

No place for motor speech acts in the assessment of dysphagia? Intelligibility and swallowing difficulties in stroke and Parkinson's disease patients.

Speech and language therapists are increasingly being asked to treat dysphagic patients. Concern has been expressed and surveys have confirmed that radiological assessment procedures are rarely available. Consequently, patients must often be assessed and their treatment planned on the basis of bedside examinations. Despite evidence that swallowing disorders need not be related to problems of articulation, recommendations on the procedure of such examinations frequently include an evaluation of speech and non-speech articulatory movements. A study is reported of patients who exhibit both dyspha

Perception and production of approximant consonants by normal and articulation-delayed preschool children.

Disagreement exists concerning the relationship between the perception of phonetic contrasts and their production by both normal and articulation-delayed children. The perception of three approximant consonant contrasts (/w/-/r/, /w/-/l/, /r/-/l/) was examined in two groups of 3-year-old children: normal children who did and did not articulate /r/ and /l/ correctly and articulation-delayed children who misarticulated /r/ and /l/. Perception was assessed in a two-choice forced-choice identification task in which the subjects heard a word and pushed a button lighting a picture corresponding to t

The effect of auditory rhythmic stimulation on articulatory accuracy in apraxia of speech.

In order to study the effects of different rates of rhythmic auditory stimulation on the phonemic accuracy of speakers who have apraxia of speech, eight apraxic subjects read four equated monosyllabic passages under one control and three experimental conditions. In the experimental conditions, rhythmic auditory stimulation provided by a metronome was imposed at each subject's oral reading rate as well as 75% and 125% of this established rate. In the control condition, the subjects read without any accompanying rhythmic stimulation. It was found that the stimulus supplied by an auditory metrono
