Outcomes after Atherectomy Treatment of Severely Calcified Coronary Bifurcation Lesions: A Single Center Experience.

BACKGROUND: Coronary bifurcation and calcified lesions account for 15-20% and 6%-20% of percutaneous coronary interventions (PCI), respectively. Treatment of these lesions is associated with high periprocedural complication rates and unfavorable long-term clinical outcomes, including high rates of revascularization. This retrospective, single-center study evaluated the outcomes of atherectomy treatment for heavily calcified coronary bifurcation lesions. METHODS: Patients who underwent a coronary atherectomy procedure to treat a heavily calcified lesion between January 2010 and March 2016 at M…

Real-World Multicenter Registry of Patients with Severe Coronary Artery Calcification Undergoing Orbital Atherectomy.

OBJECTIVES: We evaluated the safety and efficacy of orbital atherectomy in real-world patients with severe coronary artery calcification (CAC). BACKGROUND: The presence of severe CAC increases the complexity of percutaneous coronary intervention as it may impede stent delivery and optimal stent expansion. Atherectomy may be an indispensable tool for uncrossable or undilatable lesions by modifying severe CAC. Although the ORBIT I and II trials report that orbital atherectomy was safe and effective for the treatment of severe CAC, patients with kidney disease, recent myocardial infarction, long…

Angiographic outcome after intracoronary X-Sizer helical atherectomy and thrombectomy: first use in humans.

The objective of this study was to evaluate the early angiographic outcome in the first human subjects who underwent intracoronary atherectomy and thrombectomy using the X-Sizer helical cutting and aspiration system. Percutaneous coronary interventions in patients with thrombo-occlusive disease or friable degenerative saphenous vein grafts are associated with considerable periprocedural morbidity and mortality, predominantly related to microscopic distal embolization. X-Sizer catheter system is a novel atherectomy and thrombectomy device that consists of a helix cutter connected to a handheld …

Transluminal extraction catheter atherectomy followed by immediate stenting in treatment of saphenous vein grafts.

OBJECTIVES: The purpose of this study was to evaluate the effectiveness of transluminal extraction catheter (TEC) atherectomy followed by immediate Palmaz-Schatz coronary stenting of coronary bypass vein grafts. BACKGROUND: Degeneration of saphenous vein coronary bypass grafts has become a common problem. Repeat bypass surgery is associated with greater risk and a poorer outcome than the initial operation. Moreover, percutaneous interventional procedures in vein grafts have been associated with high procedural complication rates, including distal embolization, and high restenosis rates. TEC a…

Plaque rupture as a cause of apparent coronary aneurysm formation following directional coronary atherectomy.

Late coronary aneurysm formation was observed following treatment by directional coronary atherectomy. Intravascular ultrasound disclosed that the mechanism involved was plaque rupture. The cause of coronary aneurysm may be multifactorial after coronary interventions. Intravascular ultrasound imaging is useful for understanding the pathologic mechanism of coronary aneurysm production.

Brachial approach directional coronary atherectomy of a left coronary artery saphenous vein bypass graft.

Directional coronary atherectomy (DCA) of a saphenous vein bypass graft to the left coronary artery was performed percutaneously from the brachial artery approach using a 7F endomyocardial biopsy sheath. Initial positioning was accomplished with a left bypass graft catheter inserted in the sheath. This technique permits use of smaller catheters than usual for DCA in patients in whom larger guides cannot be used.

Safety and outcomes of upfront large-size rotational atherectomy burrs in left main coronary interventions.

OBJECTIVES: Data on the safety and efficacy of percutaneous coronary intervention (PCI) with adjunctive rotational atherectomy (RA) in left main (LM) disease are limited. Current guidelines recommend using a small-size RA burr initially, with upsizing if required. The safety of upfront use of a large-size RA burr (≥1.75 mm) is unknown. The authors assessed the safety of an upfront large rotablation burr in the treatment of LM calcification and the procedural and long-term outcome of this approach in comparison to a small-size burr. METHODS: Retrospective analysis of all patients undergoing PC…

Intravascular lithotripsy versus rotational atherectomy for coronary atherosclerosis calcification: a systematic review and meta-analysis.

BACKGROUND: Intravascular lithotripsy (IVL) is a novel calcium modification strategy with little evidence of efficacy compared to widely used rotational atherectomy (RA). AIMS: We sought to compare the efficacy and safety level between IVL and RA in moderate-to-severe coronary artery calcification (CAC). METHODS: We searched three databases until March 2025 for eligible observational studies and randomized controlled trials. The primary endpoint was major adverse cardiac event (MACE) while secondary endpoints were clinical outcomes, periprocedural complication, and procedural outcomes. Mante…

Optical Coherence Tomography Characterization of Coronary Lithotripsy Following Atherectomy for Treatment of Severely Calcified Lesions.

BACKGROUND: A combined strategy of atherectomy followed by intravascular lithotripsy (dual-prep strategy) for severely calcified lesions may optimize stent deployment, but mechanistic evidence from prospective optical coherence tomography (OCT) imaging is limited. This study was designed to comprehensively characterize patterns of calcium modification during a dual-preparation strategy and to explore imaging determinants of calcium fracture and stent expansion using serial and 3-dimensional OCT assessment. METHODS: This was a prespecified, multicenter, single-arm OCT substudy of the DUAL-PREP…

Rotational atherectomy for calcified coronary lesions in severe aortic stenosis before transcatheter aortic valve implantation.

OBJECTIVES: Calcified coronary artery disease (CAD) is prevalent in elderly patients with degenerative severe aortic stenosis (AS). Preparation of such calcified CAD using rotational atherectomy (RA) in those patients with severe AS is controversial and may carry a high risk of complications. The authors aimed to compare in-hospital outcomes following RA in patients with severe AS before transcatheter aortic valve implantation (TAVI) vs patients without AS. METHODS: The authors retrieved data from the Prospective Segeberg TAVI Registry from January 2016 to October 2021. All AS patients who un…

Tenting Sign on Intravascular Ultrasound: A Red Flag for Coronary Perforation Risk During Rotational Atherectomy.

Coronary perforation (CP) is a rare but one of the most serious complications of rotational atherectomy (RA). The presence of a "tenting sign" on intravascular ultrasound, which reflects outward deformation of the normal vessel wall due to extreme wire bias, may indicate an elevated risk of CP following RA. We report a case of CP in the left circumflex coronary artery that occurred at a site where the tenting sign was observed on IVUS before RA.

High Risk Optical Coherence Tomography Findings in a Case of Coronary Perforation Following Orbital Atherectomy.

Coronary perforation (CP) is an uncommon but serious complication of orbital atherectomy (OA). Optical coherence tomography (OCT) evidence of catheter "tenting" against the healthy vessel wall may indicate unsatisfactory guidewire bias and elevated perforation risk during OA. We describe OCT findings of a case with OA-related CP.

Procedural safety of rotational atherectomy and modified balloon angioplasty: insights from a German national registry.

BACKGROUND: Modified balloons (MB) and rotational atherectomy (RA) are recommended tools for treatment of coronary plaques with superficial calcium. Knowledge about in-hospital safety is limited. METHODS: Patients with coronary artery disease who underwent coronary angiography with RA or MB angioplasty in Germany were identified via ICD and OPS codes from 2017 to 2020. Acute coronary syndromes were excluded. Since patients were not randomized toward MB or RA, potential confounding factors were taken into account using the propensity score methods. Thereby, inverse probability weighting was ap…

Impact of Periprocedural Myocardial Injury on Long-Term Outcomes After Percutaneous Coronary Intervention Requiring Atherectomy.

The prognostic implications of cardiac troponin elevation after percutaneous coronary intervention (PCI) with atherectomy have not been established. The aim of this study was to investigate the incidence of periprocedural myocardial injury (PMI) and its association with cardiovascular events in patients with severely calcified lesions who underwent PCI with atherectomy. The study analyzed 346 patients (377 lesions) who underwent PCI with atherectomy between January 2018 and December 2021. Peak post-PCI high-sensitivity cardiac troponin (hs-cTn) was measured. The primary outcome was target lesi…

Prediction of optimal debulking segments before rotational atherectomy based on pre-procedural intravascular ultrasound findings.

This study evaluated whether intravascular ultrasound (IVUS) examination before rotational atherectomy (RA) can predict the optimal route of passage of the RA burr along the vessel. 30 patients with calcified lesions who underwent IVUS before and immediately after RA were enrolled. IVUS analyses were performed at the minimum lumen area (MLA) site and at 0.5 mm intervals. Each IVUS cross-section was divided into 4 quadrants around the center of the lumen, and pre- and post-RA IVUS cross-section images were merged. Of 1140 cross-sections, 498 (44%) contained debulked regions. When the guidewire …

Successful management of lower extremity distal embolization following percutaneous atherectomy with the JetStream G3 device.

There has been a resurgence of atherectomy as a strategy to treat peripheral atherosclerosis. Distal embolization occurs frequently during peripheral endovascular interventions but is of particular concern during atherectomy. We describe a case of distal embolization following atherectomy with the Pathway JetStream system successfully managed with aspiration thrombectomy and infrapopliteal angioplasty. A detailed review of the incidence, treatment, and prevention of DE in this context are presented.

ROBITAL Technique: Combination of Sequential ROtational and orBITAL Atherectomy in Management of Complex Coronary Calcium.

BACKGROUND & AIMS: Heavily calcified coronary artery disease remains one of the most formidable challenges in contemporary percutaneous coronary interventions, often leading to suboptimal stent expansion, procedural complications, and poor long-term outcomes. Over the years, a wide range of calcium modification techniques have been developed, including cutting and scoring balloons, rotational atherectomy, orbital atherectomy, high-pressure balloons, intravascular lithotripsy, and laser atherectomy, each with unique mechanisms, strengths, and limitations. This article aims to provide a comprehe…
