Self-Adjusted Amplification Parameters Produce Large Between-Subject Variability and Preserve Speech Intelligibility.

The current study used the self-fitting algorithm to allow listeners to self-adjust hearing-aid gain or compression parameters to select gain for speech understanding in a variety of quiet and noise conditions. Thirty listeners with mild to moderate sensorineural hearing loss adjusted gain parameters in quiet and in several types of noise. Outcomes from self-adjusted gain and audiologist-fit gain indicated consistent within-subject performance but a great deal of between-subject variability. Gain selection did not strongly affect intelligibility within the range of signal-to-noise ratios teste

[A survey and childhood deafness in a neonatology unit of Caritas Baby Hospital in Bethlehem].

AIM: The prevalence of deafness in a pediatric hospital Caritas Baby Hospital in Bethlehem was described. METHOD: All the clinical records of neonates born September 25, 2006 to March 25, 2007 screened for deafness were selected. RESULTS: Among the 274 screened newborns, 112 were females and 162 males, 97% from the districts of Bethlehem and Hebron, and 3% from other locations in the West Bank. The most recurrent diagnoses were jaundice, potential sepsis and prematurity. Sixty-three were positive at the first screening, 26 at the second screening and only 4 were visited by the audiologist, w

The expanding role of the otolaryngologist in managing infants and children with hearing loss.

Universal newborn hearing screening will increase the number of very young children requiring care from an otolaryngologist. Although the otolaryngologist is the perceived expert in managing hearing loss, he or she must collaborate with a team of specialists to provide comprehensive treatment for the newly diagnosed child and its family. Working within a team that includes a pediatrician, audiologist, otolaryngologist, speech pathologist, teachers, and care coordinators allows each professional to contribute significantly to the well-being of the child and parents. Group cooperation and parent

Audiological rehabilitation of older people: visual and vibrotactile considerations.

This paper discusses the peripheral and central visual and tactile pathways with respect to the diminishing integrity of the ageing nervous system. This discussion is based upon extant data derived from investigators in vision and vibrotactile sensation. The purpose in examining these investigations is to ascertain the problems associated with the use of vision and vibrotactile stimulation in the auditory rehabilitation of older people. Speed of processing and simultaneous use of pathways are factors to be considered carefully in order not to contribute to perceptual confusions. The writers hy

Hemifacial microsomia: priorities and sequence of comprehensive otologic management.

Hemifacial microsomia is evidenced by the clinical spectrum of microtia, mandibular deformities, and middle ear malformations. This constellation of defects suggests that the most efficacious treatment will be multidisciplinary management, which includes the otolaryngologist, audiologist, plastic surgeon, and temporal bone radiologist. In the past, microtia--the most clinically apparent malformation--usually has been studied and treated with respect to the area of special interest of the investigator, whether from an otologic, reconstructive, or radiologic perspective. Newer management, howeve

Clinical acumen and vestibular schwannoma.

OBJECTIVE: This study aimed to quantify the atypical presentations of patients with vestibular schwannoma by applying the audiologic criteria for the treatment of patients to the Cambridge series of such tumors to show that clinical acumen is necessary for the effective screening of these tumors. STUDY DESIGN: This study was a retrospective case review of 473 patients with vestibular schwannoma. SETTING: The surgery was conducted at the Department of Otoneurological and Skull Base Surgery, University Hospital, Cambridge, a tertiary referral center. PATIENTS: A total of 473 patients with a u

Sudden hearing loss and autoimmune inner ear disease.

This case report describes the audiologic and medical diagnostic evaluations, results, and treatment options in a patient with a classic pre … nd retrocochlear involvement and may initially appear to be inconsistent with pure-tone thresholds. Audiologists must be familiar with AIED because early identification is critical. Additionally, an immunologic basis may be a factor in other disorders, including many cases of Meniere's disease.

Diagnosis and treatment of ear disease among children in the Ellisras district. An outreach programme.

Audiologists and ENT registrars examined 2,036 children aged 10 years or younger by means of pure-tone audiometry, tympanometry and otoscopic examinations. Twenty per cent of these children had ear pathology and 7.5% had impaired hearing. Forty-three pus swabs taken from patients with suppurative otitis media most often cultured Proteus mirabilis and Pseudomonas species. The prevalence of otitis media with effusion was twice as high in white children as in black children, resulting in a much higher prevalence of hearing loss in the first group. The black children had 7 times more dry perforati…

Clinical application of otoacoustic emissions: what do we know about factors influencing measurement and analysis?

Three electrophysiologic audiologic procedures-aural immittance measurement, auditory brainstem response (ABR), and otoacoustic emissions (OAE)- were first described in the 1970's. Immittance measurement and ABR have contributed importantly for years to the assessment of auditory function in children and adults, whereas OAEs have not yet been incorporated into the everyday audiology test battery. In this article, we argue that the transition from OAE measurement by hearing scientists in laboratory settings to routine application by audiologists in the clinic will be greatly facilitated by (1) …

Psychological Comorbidities of Tinnitus.

In this chapter, I address the topic of tinnitus in the context of the patient's trajectory of care, with special attention to psychological … ometric tools to quickly draw a portrait of the psychological state of patients that can be used by audiologists, psychologists, or doctors. Therapeutic avenues can then be discussed with the patients to ensure them the best support possible.

Providing an internet-based audiological counselling programme to new hearing aid users: a qualitative study.

People with an acquired hearing loss often have difficulty adjusting to a first hearing aid. Studies have shown that audiological counselling can facilitate adjustment to a first hearing aid. Because of its interactive nature, the internet could be a valuable tool to gain information about the experiences of the new hearing aid user and to address the needs for audiological counselling. An internet-based audiological counselling programme in the form of daily e-mails during the first month after the hearing aid fitting was offered to three new hearing aid users. The data, qualitative in nature

Audiologic screening of newborn infants who are at risk for hearing impairment.

The importance of early identification of hearing impairment is well documented. The Joint Committee on Infant Hearing 1982 Position Statement established the goal of identification and habilitation of hearing-impaired infants by age 6 months but did not specify the procedure for initial audiologic screening. In these guidelines, ASHA specifies the recommended procedure for audiologic screening of infants at risk for hearing impairment that includes a) parent/caregiver education; b) audiologic screening by ABR; and c) referral to a comprehensive evaluation, follow-up, and management system for

Comparing the International Classification of Functioning, Disability, and Health Core Sets for Hearing Loss and Otorhinolaryngology/Audiology Intake Documentation at Mayo Clinic.

OBJECTIVES: The International Classification of Functioning, Disability, and Health (ICF) Core Sets for Hearing Loss (CSHL) consists of short lists of categories from the entire ICF classification that are thought to be the most relevant for describing the functioning of persons with hearing loss. A comprehensive intake that covers all factors included in the ICF CSHL holds the promise of developing a tailored treatment plan that fully complements the patient's needs. The Comprehensive CSHL contains 117 categories and serves as a guide for multiprofessional, comprehensive assessment. The Brief

PURPOSE: The purpose of this letter, in response to Hall, Mehta, and Fackrell (2017), is to provide important knowledge about methodology and statistical issues in assessing the reliability and validity of an audiologist-administered tinnitus loudness matching test and a patient-reported tinnitus loudness rating. METHOD: The author uses reference textbooks and published articles regarding scientific assessment of the validity and reliability of a clinical test to discuss the statistical test and the methodological approach in assessing validity and reliability in clinical research.
