The pulmonary autograft: results and left ventricular function.

From June 1991 through January 1996, 60 patients (mean age, 30 years) underwent aortic valve replacement with pulmonary autografts. Indications for operation were aortic stenosis in 31 patients, aortic incompetence in 27 patients, and endocarditis in 2 patients. Twenty-four patients were congenital patients (median age, 10 years; range, 5 months to 27 years) with aortic incompetence in 17, isolated aortic stenosis in 5, and small stenotic prostheses in 2. Transthoracic echocardiography was performed preoperatively in all patients and serially after surgery with the aims of measuring aortic and…

[Pulmonary autograft as aortic valve replacement in children].

The aortic stenosis represents 5% of all congenital cardiac pathology. The conventional treatments often do not give very satisfactory results. Since 1990, as alternative to the traditional aortic valve replacement, we used, in our institution, the Ross procedure (pulmonary autograft). This consists in using the pulmonary valve as an autograft in aortic position to replace the diseased valve. Then, the pulmonary outflow is reconstructed with a cryopreserved homograft. This study includes 15 patients, aged between 16 months and 18 years, treated with this technique since 1990. There has been no

A nonuser's questions about cultured epidermal autograft.

Clinical experience assumes major importance as the source of data on cultured epidermal autograft (CEA), since a large controlled study is probably not forthcoming. Among the general questions to be answered are: Which patients are candidates for grafting with CEA? Do selection criteria specify size of total body surface area burn or age? Is CEA more suitable for certain body areas than others? Does CEA close the wound as quickly, as well, and as safely as other available methods of coverage? Fresh or frozen allograft, the gold standard, is presumed to be safe, but the current concern about t

[Dental autograft: a supplementary dimension in dental practice].

This article deals with the possibilities and limitations of autogenous transplantations. In properly selected cases (trauma, missing teeth,...), this procedure might be a valuable alternative treatment. The indications for transplantation, the surgical technique, the recipient site and the choice of the transplant are discussed. The most important parameters which influence the success of a transplantation are: the stage of root formation, the atraumatic removal and insertion of the germ, the final position of the transplant and the post-operative fixation. Clinical examples will illustrate s

A randomized trial of conjunctival autografting for pterygium in the tropics.

This is the first report of a randomized trial of the conjunctival autografting technique for pterygium removal. This trial was conducted in the Caribbean where risk of pterygium recurrence is considered to be high. Although not statistically significant, there was a lower recurrence rate in grafted cases (21% of 19 cases) than in controls done by the bare sclera technique (37% of 16 cases). The author found that age of the patient was strongly associated with recurrence regardless of which procedure was used. Findings of this study emphasize the importance of randomized controlled trials in a

Limbal autograft transplantation for ocular surface disorders.

Limbal autograft transplantation is presented in 26 consecutive cases comprising both acute and chronic chemical injury (20 cases), thermal burns (2 cases), contact lens-induced keratopathy (3 cases), and ocular surface failure after multiple surgical procedures (1 case), with follow-up ranging from 2 to 45 months (mean, 18 months). The operative technique usually involved transfer of two free grafts of limbal tissue from the uninjured or less injured donor eye to the severely injured recipient eye, the latter having been prepared by limited conjunctival research and superficial dissection of

Clinical studies of autografting in acute lymphocytic leukaemia.

The role of autologous marrow transplantation has been reviewed in this chapter. The transplantation results in first remission are difficult to interpret due to the prognostically heterogeneous patient population transplanted and the ever-changing natural history of first complete remission due to more aggressive conventional-dose chemotherapy regimen. The biological role of marrow purging, i.e. elimination of leukaemic cells from the graft, is yet unclear. It is the opinion of the authors that studies in second and subsequent remission can resolve burning issues such as the efficacy of vario

Reconstruction of the brachiocephalic circulation by means of a jugular autograft.

Three patients with symptomatic occlusions of subclavian artery were operated upon by means of carotid subclavian bypass using ipsilateral jugular vein. The indications for this technique include unavailability of saphenous veins or contraindications to use of prosthetics. Late clinical and angiographic followup have revealed satisfactory results and no changes in caliber of vein graft.

[Recurrent pterygia: frequency and treatment by conjunctival autograft].

Of 91 pterygia treated by simple excision (by the same surgeon, using the same technique) followed by beta-irradiation 7 recurred. Of 15 recurrent pterygia treated by complete excision by the bare-sclera technique associated with a conjunctival autograft only 1 recurred again. Conjunctival autografting is simple and efficacious, and gives very good esthetic results in cases of recurrent pterygium.

Rectus Femoris Autograft Harvesting: Tips and Tricks.

The rectus femoris tendon has recently emerged as a potential graft option for anterior cruciate ligament reconstruction because of its favorable biomechanical properties, substantial graft length, and minimally invasive harvest characteristics. As the most superficial layer of the quadriceps tendon complex, the rectus femoris tendon can be selectively harvested while preserving the deeper quadriceps layers and extensor mechanism continuity. This technical note describes a reproducible minimally invasive technique for rectus femoris tendon harvest, emphasizing surgical anatomy, dissection plan

Morbidity Associated With Distal Tibial Autograft Harvest.

Distal tibial autograft harvesting has been studied in the past, but morbidity at the level of the donor site is unclear. The purpose of this retrospective review is to assess morbidity in distal tibial autograft harvesting associated with foot and ankle arthrodesis procedures. A retrospective analysis was performed utilizing patients treated in the last 13 years at a large, multicenter, academic, tertiary referral, research institution. Included patients were between the ages of 18 and 80 years old. One-hundred and seven patients (39 male; 68 female) underwent ipsilateral distal tibial bone g

Osteochondral Autograft Transplantation for Capitellar Osteochondritis Dissecans.

BACKGROUND: The goal of the osteochondral autograft transplantation (OAT) procedure is to replace both the bone and cartilage that have been compromised by osteonecrosis of the capitellum, a condition known as osteochondritis dissecans (OCD). In children, the vascularity of the capitellum is limited compared with that in adults because the physis acts as a physical barrier to vascular ingrowth from the metaphysis to the epiphysis. The necrotic subchondral bone cannot keep up with the weight-bearing demands of certain high-level athletes such as gymnasts, accumulating microfractures and eventua

Counter Autografting of Dorsal Septum in Crooked Nose.

INTRODUCTION: The nasal septum plays an important role in nasal form and function. In this study, we describe a novel, alternative septoplasty approach that results in an aesthetically acceptable dorsal profile and improves airway function in patients with a crooked nose. PATIENTS AND METHODS: This study enrolled 26 patients who presented with crooked noses between 2012 and 2017. All patients underwent open rhinoplasty under general anesthesia. During correction of the cartilaginous part, a dorsal strip from the deviated septum was prepared and inverted in a 180° fashion to exert a counter-fo

A Technique for Arthroscopic Osteochondral Autograft Transplantation of the Hip.

Osteochondritis dissecans of the hip is a rare condition with limited treatment options. For patients who have undergone arthroscopic microfracture and chondroplasty, long-term results showing clinical improvement are scarce. Although open osteochondral grafting is a demanding and invasive procedure, promising results are reported after short-term follow-up. We present a technique for an arthroscopic autologous osteochondral transplantation with a graft from the ipsilateral knee to the femoral head.

Pulmonary autograft in aortic position: is everything known?

The Ross operation provides several advantages compared to other valve substitutes to manage aortic valve disease, such as growth potential, excellent hemodynamics, freedom from oral anticoagulation and hemolysis, and better durability. However, progressive dilatation of the pulmonary autografts after Ross operation reflects the inadequate remodeling of the native pulmonary root in the systemic circulation, which results in impaired adaptability to systemic pressure and risk of reoperation after the first decade. A recently published article showed that remodeling increased wall thickness and …

Outcomes of Reintervention on the Autograft After Ross Procedure.

BACKGROUND: After a Ross procedure, a small subset of patients requires reintervention for autograft dilatation or valve insufficiency. We sought to determine the indications, nature, and outcomes of autograft reinterventions in the left ventricular outflow tract after a Ross procedure. METHODS: We retrospectively reviewed the charts of 316 consecutive patients, age 4 days to 70 years, who underwent a Ross procedure at our institution. Of these, 47 patients (15%) required autograft reintervention during a median follow-up of 8.2 years. RESULTS: Forty-seven patients, median age 26 years (inte
