Axillary TAVR under locoregional anesthesia in a patient with peripheral artery disease.

Transcatheter aortic valve replacement (TAVR) is now a relatively commonly performed procedure and the number of eligible patients is growing exponentially.  In candidates with peripheral arterial disease, the axillary artery approach is an option worth considering. Usually TAVR performed using this approach is done under general anesthesia with tracheal intubation. At our center, however, we have developed a surgical approach to TAVR using the axillary artery under locoregional anesthesia.  This video tutorial demonstrates the technical details of our strategy. Because this procedure is extra…

Axillary artery aneurysm combined with brachial plexus palsy due to Takayasu arteritis.

We report the case of a young female patient who presented with a right axillary artery aneurysm with neurological symptoms due to Takayasuarteritis (TA). Decompression of the aneurysm and arterial repair with an interposition vein bypass resulted in a full recovery. The case emphasises that peripheral aneurysms can occur due to TA and that there may be neurological complications.

Axillary artery injury from a closed humeral neck fracture: A case report.

Axillary artery injury from humeral neck fracture is an uncommon event. Vascular damage due to these injuries may threaten limb loss. In some cases, the signs of ischemia may not be evident just after the injury and may only appear later on. Therefore, a high index of suspicion is essential. It is important to bear this association in mind, so as to make an early diagnosis and avoid serious complications. We are describing the case of a young patient involved in a motor vehicle crash with an axillary artery trauma due to the surgical neck of humerus fracture. The patient underwent a basilic ve…

Carotid axillary artery bypass: an option following failed open and percutaneous procedures.

Percutaneous transluminal angioplasty and stenting is being employed with increasing frequency for stenosis involving the brachiocephalic circulation. However, the efficacy of these procedures is limited by intimal hyperplasia and subsequent re-stenosis. Long-term results for treating lesions of the subclavian or innominate artery have shown significant re-stenotic rate. Accordingly, carotid-subclavian bypass remains as an important procedure. Alternative methods may still be required, particularly after failure of open re-vascularization and percutaneous techniques. This report describes an u

Supraclavicular approach for repair of an axillary artery pseudoaneurysm after axillobifemoral bypass.

A 61-year-old woman underwent right axillobifemoral bypass using a reinforced expanded polytetrafluoroethylene T-shaped graft for high aortic occlusion. One year later, anastomotic pseudoaneurysm of the axillary artery was noted. We performed pseudoaneurysmectomy and graft interposition at the same anastomotic site through an infraclavicular approach. Unfortunately, the pseudoaneurysm recurred four months later. Therefore, we performed a second reoperation through a supraclavicular approach, in addition to the infraclavicular one. We were able to achieve better exposure from the axillary arter…

Axillary artery cannulation in surgery for acute or subacute ascending aortic dissections.

BACKGROUND: Perfusion through the right axillary artery is an alternative to aortic or femoral artery cannulation during surgery for ascending aortic dissections. The results of this strategy, particularly beyond the immediate postoperative period, are not well described. METHODS: Eighty-three patients (median age, 58 years) with acute or subacute ascending aortic dissection underwent surgical repair with right axillary artery perfusion through an interposition Dacron graft. Sixty-five patients (78%) had DeBakey type I dissections. Procedures performed concomitantly with ascending aortic repl…

Pre-expanded pedicled thoracodorsal artery perforator flap for postburn axillary contracture reconstruction.

INTRODUCTION: The axillary region is one of the sites most frequently affected by postburn contractures. In this clinical study, we used pre-expanded pedicled thoracodorsal artery (TDA) perforator flaps for release of postburn contracture of the axillary region. PATIENT AND METHODS: Five patients with severe axillary burn contractures were reconstructed with six pre-expanded pedicled TDA perforator flaps between 2008 and 2010. All were men ranging in age from 20 to 26 years (mean, 22 years). Mean time of follow-up was 12 months. Flap and donor site complications, preoperative, and postoperati

CT angiogram and endovascular stent graft for an axillary artery gunshot wound.

A patient with an axillary artery gunshot wound pseudoaneurysm was evaluated and managed with computed tomographic angiogram and endovascular stent graft. Vascular injuries resulting from penetrating trauma or interventional vascular procedures are relatively uncommon. Subclavian and axillary arterial injuries may be associated with substantial morbidity and mortality if not managed expeditiously. The inaccessibility of these arteries makes stent graft treatment particularly attractive. The stable patient with a pseudoaneurysm or an arteriovenous fistula seems to be the ideal candidate to trea…

Axillary artery cannulation: routine use in ascending aorta and aortic arch replacement.

BACKGROUND: Ideal perfusion during ascending aorta-arch surgery should allow easy implementation of antegrade cerebral perfusion while avoiding atheroembolization or false lumen perfusion in dissections. We report favorable experience with direct axillary artery cannulation. METHODS: Between 1999 and 2003, 284 patients with a mean age of 62.2 years (25 to 85), underwent axillary artery cannulation using a right angle wire-reinforced catheter. During this interval, attempted axillary cannulation was abandoned in only 14 patients because of inadequate backflow or other complications. Eighty-fiv…

Axillary artery transection following anterior shoulder dislocation: classical presentation and current concepts.

Injury to the axillary artery following anterior shoulder dislocation is a very rare occurrence. This review serves to illustrate the now classical case of an elderly gentleman with a recurrent dislocation, transection of the axillary artery and its invariable association with a severe brachial plexus lesion, which is the most important determinant of long-term disability. It also highlights the pathognomic triad of anterior shoulder dislocation, expanding axillary haematoma and diminished peripheral pulse, to highlight awareness of this important injury. The literature on this injury has been…

Clinical results with left axillary to left anterior descending coronary artery bypass.

BACKGROUND: The minimally invasive direct coronary artery bypass procedure is not feasible if the left internal mammary artery has been used or has inadequate flow. We have applied a modified minimally invasive direct coronary artery bypass procedure, which uses a graft from the left axillary artery to the left anterior descending coronary artery in such situations. METHODS: The graft is anastomosed to the left axillary artery adjacent to the clavicle and tunneled underneath the vein, where it enters the thorax through the first interspace and courses to the left anterior descending coronary …

Extra-anatomic bypass graft for management of axillary artery occlusion in pitchers.

OBJECTIVE: Our goal was to evaluate the long-term results of vein bypass grafts for axillary artery occlusion, specifically those placed extra-anatomically to prevent arterial injury in pitchers. METHODS: With the greater saphenous veins used as the selected conduit, arterial bypass grafts were routed anterior to the pectoralis minor muscle in four baseball pitchers who had occlusion of the axillary artery. We performed a follow-up in excess of 10 years with evaluations of the bypass grafts by ultrasonic duplex scan and magnetic resonance angiography. RESULTS: All four pitchers treated in th…

Avoiding Iatrogenic Axillary Artery Injury in Reverse Total Shoulder for Fracture: The Use of a Separate Axillary Incision: A Case Report.

CASE: A 71-year-old woman sustained a left proximal humerus fracture dislocation. Imaging demonstrated displacement of the humeral head medial to the axillary artery without initial injury. To avoid iatrogenic injury, a separate medial axillary incision was successfully used to remove the humeral head and then reverse total shoulder arthroplasty was performed through a deltopectoral approach. At 1 year, the patient had excellent range of motion and PROMIS Physical Function CAT (v2.0) score was 46.2. CONCLUSIONS: A separate medial axillary approach can be used to safely remove anterior/inferio…

Carotid-axillary bypass with vertebral artery transposition for superior sulcus tumor resection.

Superior sulcus tumors with subclavian artery involvement pose significant challenges to achieving complete resection while preserving cerebral and upper limb perfusion. We report the case of a 47-year-old woman with a left Pancoast tumor undergoing en bloc resection following induction therapy. A carotid-axillary bypass using a ringed polytetrafluoroethylene graft combined with transposition of a dominant vertebral artery to the common carotid artery was performed. The postoperative course was uneventful, with no neurological deficits and preserved limb perfusion. This case highlights that ta

Subclavian or axillary artery cannulation for extracorporeal membrane oxygenation: A systematic review.

BACKGROUND: Subclavian or axillary artery cannulation for venoarterial extracorporeal membrane oxygenation (VA-ECMO) provides a valuable alternative to femoral access; however, a comprehensive overview of such an approach in this setting is lacking. This review examined types of access, clinical complications, and outcomes of subclavian/axillary cannulation, emphasizing the pros and cons of this VA-ECMO approach as well as areas for further investigation. METHODS: A systematic search was conducted in PubMed, Scopus, and Web of Science for articles published between January 2000 and December 2…

Delayed-Onset Postsurgical Myofascial Pain Following Axillary Artery Cannulation: A Case Report.

Chronic postsurgical pain (CPSP) is often attributed to neuropathic, genetic, or psychosocial factors, whereas musculoskeletal causes, such as myofascial pain syndrome (MPS), are frequently overlooked. We report a case of delayed-onset MPS as a musculoskeletal manifestation of CPSP following axillary artery cannulation, aiming to raise awareness of this potential etiology. A 48-year-old man developed persistent right anterior chest pain one year after total arch replacement surgery. Physical examination revealed postural asymmetry, atrophy of the right pectoralis major muscle, mechanical allod…
