Traumatic Atlanto-Occipital Dissociation and Atlantoaxial Instability: Concomitant Ligamentous Injuries without Neurologic Deficit: A Case Report.

CASE: A 35-year-old man was involved in a motor-vehicle collision and sustained multiple thoracoabdominal and extremity injuries. A high injury burden and a normal neurologic examination contributed to a delay in diagnosing a ligamentous, combined atlanto-occipital dissociation (AOD) and vertical atlantoaxial injury (AAI). Additional imaging revealed the combined injury, and the patient underwent an occipitocervical fusion. CONCLUSION: To our knowledge, this is the first case of a simultaneous traumatic AOD and AAI without fracture in a neurologically intact patient. Knowledge of reliable and

[Spinal ataxia caused by an occipito-atlanto-axial malformation in a lamb].

A three month old lamb suffering from locomotory disturbances was submitted to the clinic. It had a tilted head to the left and an atactic gait especially in the hind limbs. When it was forced to stop or to change direction it fell down. General condition and sensorium were apart from a reduced bathyaesthesia normal. By clinical and laboratory examination some possible causes of locomotion disturbances in lambs like copper deficiency, nutritive muscle dystrophy and meningitis could be ruled out. By X-ray examination an asymmetry of the os occipitale and atlas was diagnosed. They were not well

Measurement of the vertebral canal dimensions of the neck of the rat with a comparison to the human.

The aim of this study was to determine the dimensions of the vertebral canal in the neck of the rat, because little is known about the morphology of the rat's cervical spine. A comparison then was made to the vertebral canal in the neck of the human. In part 1 of this study, we determined the precision of three different methods to measure the vertebral canal. The error (coefficient of variation) in these methods was found to range from 1 to 8%. In part 2, we used a computer-based system to measure digital images of the vertebra and determined the anterior to posterior and the transverse verte

A biological basis for instantaneous centres of rotation of the vertebral column.

The instantaneous centre of rotation has proven to be a useful parameter of vertebral motion. The normal location of instantaneous centres has been determined in cadavers and in normal volunteers for the cervical, thoracic and lumbar spines, and abnormal location of centres has been shown to correlate with spinal pain. However, to date, an instantaneous centre has constituted no more than a convenient mathematical summary of vertebral kinematics. It has defied resolution into biologically meaningful parameters. This study offers a novel model of vertebral motion in which the instantaneous cent

Anatomy of the craniocervical junction - A review.

An in-depth understanding of the anatomy of the craniocervical junction (CCJ) is indispensable in skull base neurosurgery. In this paper, we discuss the osteology of the occipital bone, the atlas (C1) and axis (C2), the ligaments and the muscle anatomy of the CCJ region and their relationships with the vertebral artery. We will also discuss the trajectory of the vertebral artery and review the anatomy of the jugular foramen and lower cranial nerves (IX to XII). The most important surgical approaches to the CCJ, including the far lateral approach, the anterolateral approach of Bernard George an

Functional-adaptive anatomy of the axial skeleton of some extant marsupials and the paleobiology of the paleocene marsupials Mayulestes ferox and Pucadelphys andinus.

In this study, the axial skeletons of two Early Paleocene marsupials, Mayulestes ferox and Pucadelphys andinus, were analyzed functionally and compared to that of six South American and three Australian species of extant marsupials. In the case of the South American opossums, myological data of the epaxial musculature were collected and analyzed and osteological-myological associations were related to locomotor behavior. Various features of the vertebral column that relate to diet or to locomotor or postural patterns were pointed out. These features include: the craniocaudal development of the

Posterior atlantoaxial fusion. A new internal fixation device.

STUDY DESIGN: Biomechanical and clinical testing of an atlantoaxial posterior fixation device. OBJECTIVES: The authors tested an internal fixation device to maintain the atlas and axis in an anatomic relationship while fusion occurs. The device should also facilitate intraoperative reduction of any residual anterior atlantoaxial subluxation. The device should allow the use of cancellous rather than cortical bone graft. SUMMARY OF BACKGROUND DATA: Previous techniques of atlantoaxial fusion were not universally successful, and the quality of reduction was assessed infrequently. METHODS: Biome

CT and MRI in hydatid disease of cervical vertebrae.

Hydatid disease (HD) of the cervical spine is rare. A case investigated by computed tomography (CT) and magnetic resonance imaging (MRI) is presented. While CT shows the bone lesion better, MRI is superior in demonstrating compression of neural structures. The complementary use of CT and MRI in such cases is suggested.

[Suboccipital Pott's disease].

A new case of sub-occipital Pott's disease is reported: a 26 year old, coloured male patient from Senegal was admitted for: cervical pain of 6 month duration, neck stiffness, dysphagia, left XII paralysis and left hemiparesis. The radiological study found an anterior atlanto-axial subluxation, basilar impression, and increased thickness of the retropharyngeal soft tissue, lysis of the left occipital condyle and the left lateral mass. CT scan study revealed a retropharyngeal mass and an epidural contrast-enhanced lesion at the C2 C3 C4 levels. Chest-X ray showed one tuberculous lesion at the ri

Segmentation in staged human embryos: the occipitocervical region revisited.

The first seven somites, the rhombomeres, and the pharyngeal arches were reassessed in 145 serially sectioned human embryos of stages 9-23, 22 of which were controlled by precise graphic reconstructions. Segmentation begins in the neuromeres, somites and aortic arches at stage 9. The following new observations are presented. (1) The first somite in the human, unlike that of the chick, is neither reduced in size nor different in structure, and it possesses sclerotome, somitocoel and dermatomyotome. (2) Somites 1-4, unlike those of the chick, are related to rhombomere 8 (rather than 7 and 8) and

Biomechanics of the cervical spine. I: Normal kinematics.

UNLABELLED: This review constitutes the first of four reviews that systematically address contemporary knowledge about the mechanical behavior of the cervical vertebrae and the soft-tissues of the cervical spine, under normal conditions and under conditions that result in minor or major injuries. This first review considers the normal kinematics of the cervical spine, which predicates the appreciation of the biomechanics of cervical spine injury. It summarizes the cardinal anatomical features of the cervical spine that determine how the cervical vertebrae and their joints behave. The results a

Radiologic review: the cervical spine in juvenile rheumatoid arthritis.

In our patients, zygapophyseal joint fusion was the most frequent and characteristic roentgenographic finding of cervical spine involvement in JRA. It was not associated with any subset of patients. Atlanto-dens interval greater than 4.5 mm was present in 20% of patients, but was not related to the development of neurologic manifestations. Mild enthesopathic-like changes around the upper cervical area were observed in some children. In patients with severe long standing disease and extensive zygapophyseal joint fusion, longitudinal ligament calcification was observed. Growth disturbances of th

[Cervical involvement in rheumatoid arthritis (author's transl)].

The authors studied 577 records of rheumatoid arthritis examined radiologically in the Department of Radiology B of Cochin Hospital: 165 patients showed evidence of lesions of the cervical spine. The risk of cervical involvement in rheumatoid arthritis is thus not negligeable: 28%. This involvement may occur at any time during the course of the disease and presents as pain or by rare and more or less slight neurological manifestations. Radiologically emphasis must be placed upon the orientation points of straight and lateral films, and the value of lateral films with the neck in flexion. Atlan

Dysplasia of the odontoid process in Morquio's syndrome causing quadriparesis.

Eleven patients with documented Morquio's syndrome who had dysplasia of the odontoid process and resulting atlanto-axial instability were reviewed. They were found to be at risk for acute traumatic quadriparesis, chronic myelopathy of a variable and often rapid rate of progression, and sudden death by respiratory arrest. The evidence suggests that early prophylactic posterior cervical fusion is indicated for patients with this malformation and that once quadriparesis is established, recovery of function is limited.
