BACKGROUND: Acute rejection is a major risk factor for chronic allograft nephropathy, although the link(s) between these events is not understood. The hypothesis of this study is that alterations in tubular basement membranes (TBMs) that occur during acute rejection may be irreversible and thereby play a role in the development of chronic allograft nephropathy.

PURPOSE: To determine the outcome of patients who received phototherapeutic keratectomy (PTK) for recurrent erosion syndrome due to anterior basement membrane dystrophy (ABMD).
DESIGN: A retrospective, noncomparative case series.
PARTICIPANTS: Forty-eight eyes of 43 consecutive patients who underwent PTK for recurrent erosions occurring in ABMD at the Hunkeler Eye Center from 1991 to April 1995. We report here four additional cases of hyalinizing trabecular adenoma of the thyroid and describe the findings of the accumulated basement membrane (BM) material. Focal lumpy depositions of BM material were noted in the stroma of all four tumors and were particularly prominent in two. We studied the nonenzymatic glycosylation of lens epithelial basement membranes (LEBM) of senile cataractous lenses of both diabetic and nondiabetic patients. The human LEBMs were isolated from surgically removed senile cataracts and purified by osmotic lysis and detergent treatments. The reformation of basement membrane antigens was examined during healing following gingivectomy procedures. Three antigens, Type IV collagen, bullous pemphigoid antigen and epidermolysis bullosa acquisita (EBA) antigen, were identified by indirect immunofluorescent tests. Linear deposition of immunoglobulin G was seen by direct immunofluorescence along the tubular basement membranes in the kidney of a dog with chronic tubulointerstitial nephritis. Autoantibody eluted from the affected kidney bound to the tubular basement membrane of normal canine kidney, but not to several other normal canine basement membranes. During nephrotoxic nephritis in the rat, an increased urinary excretion of glucosyl-galactosyl hydroxylysine and of galactosyl-hydroxylysine has been observed in the autologous phase of the disease. Antiserum to the human cutaneous basement membrane-anchoring fibril complex specifically binds to the basement membrane zone of normal human skin as demonstrated by indirect immunofluorescence and, in addition, cross-reacts with normal human glomerulus. This antiserum does not prevent binding of pemphigoid antibody to its specific antigen; thus the antigen of bullous pemphigoid is unlikely to be a component of the cutaneous basement membrane (basal lamina).

Skeletal muscle capillary basement membrane thickness was determined in age- and sex-matched normal and spontaneously diabetic Mystromys albicaudatus. In nondiabetic animals the average basement membrane thickness was 482.6+/-48.7 A as against 779.0+/-319.9 A in the diabetic ones. This difference was statistically significant at P less than 0.005.

Sera from 208 individuals were examined for antibasement membrane (anti-BM) antibodies by means of an indirect immunofluorescence technique. An understanding of basement membrane (BM) assembly at a molecular level provides a foundation with which to develop repair strategies for diseases with defects of BM structure.
