Rate of spontaneous voiding recovery after acute urinary retention due to bed rest in the hospital setting in a nonurological population clinical study of the relationship between lower limbs and bladder function.

OBJECTIVES: To understand the clinical relationship between lower limbs functions and the recovery of spontaneous voiding after an acute urinary retention (AUR) in older patients admitted to hospitals for non-urological causes using clinical parameters. MATERIALS AND METHODS: 56 adult patients (32 men; mean age: 77.9 ± 8.3 and 24 women; mean age 82.1 ± 4.6) with AUR were prospectively followed with validated Physical Performance Mobility Exam (PPME) instrument to evaluate the relationship between the recovery of mobility capacity and spontaneous voiding. After a short period of permanent blad

Effects of antioxidant supplementation on bone mineral density, bone mineral content and bone structure in healthy men during 60 days of 6° head-down tilt bed rest: Results from a randomised controlled trial.

Dietary countermeasures to mitigate detrimental spaceflight-induced effects on bone health would alleviate the requirements and the consequences imposed by other types of countermeasures for this risk. We hypothesised that antioxidant supplementation during 60 days of 6° head-down tilt bed rest (HDBR), an analogue of spaceflight, would have a protective effect on bone mineral density (BMD), content (BMC) and bone structure parameters. An exploratory, randomised, controlled, single-blind intervention trial was conducted in a parallel design with 20 healthy male volunteers (age 34 ± 8 y, weight

Human blood adenosine biomarkers and non-rapid eye movement sleep stage 3 (NREM3) cortical functional connectivity associations during a 30-day head-down-tilt bed rest analogue: Potential effectiveness of a reactive sledge jump as a countermeasure.

We investigated the alterations of sleep regulation and promotion biomarkers as adenosine through its enzymes total adenosine deaminase (tADA)/adenosine deaminase (ADA2) in a microgravity analogue environment of head-down-tilt bed rest and their association with brain connectivity networks during non-rapid eye movement sleep stage 3 (NREM3), as well as the effectiveness of the reactive sledge (RSL) jump countermeasure to promote sleep. A total of 23 healthy male volunteers were maintained in 6° head-down-tilt position for 30 days and assigned either to a control or to a RSL group. Blood collec

Automated assessment of mobility in bedridden patients.

Immobility in older patients is a costly problem for both patients and healthcare workers. The Hierarchical Assessment of Balance and Mobility (HABAM) is a clinical tool able to assess immobile patients and predict morbidity, yet could become more reliable and informative through automation. This paper proposes an algorithm to automatically determine which of three enacted HABAM scores (associated with bedridden patients) had been performed by volunteers. A laptop was used to gather pressure data from three mats placed on a standard hospital bed frame while five volunteers performed three enac

Does postoperative ileus exist?

In the past, it was assumed that normal feeding immediately after major abdominal surgery was impossible because normal intestinal motility was disrupted. Today, such a view is widely rejected. However, there has never been clear agreement on just what is the supposed abnormality in intestinal function. There are major difficulties in studying intestinal function after surgery. Furthermore, a number of other factors such as anesthetic agents and analgesia can influence the situation. There has been very little research on the subject in recent years, and uncertainty remains about this subject,

Impending aortic aneurysm rupture - a case report and review of the warning signs.

BACKGROUND: Bed rest duration following deployment of a vascular closure device after transfemoral left-sided cardiac catheterization is not standardized. Despite research supporting reduced bed rest, many hospitals require prolonged bed rest. Delayed ambulation is associated with back pain, urine retention, difficulty eating, and longer stay. OBJECTIVE: To study length of stay, safety, and opportunity cost savings of reduced bed rest at a large urban hospital. METHODS: A single-site 12-week study of 1-hour bed rest after transfemoral cardiac catheterizations using vascular closure devices.

Physiological adaptations affecting drug pharmacokinetics in space: what do we really know? A critical review of the literature.

As human spaceflight progresses with extended mission durations, the demand for effective and safe drugs will necessarily increase. To date, the accepted medications used during missions (for space motion sickness, sleep disturbances, allergies, pain, and sinus congestion) are administered under the assumption that they act as safely and efficaciously as on Earth. However, physiological changes have been documented in human subjects in spaceflight involving fluid shifts, muscle and bone loss, immune system dysregulation, and adjustments in the gastrointestinal tract and metabolism. These alter

Mobilisation of epistaxis patients--a prospective, randomised study documenting a safe patient care regime.

OBJECTIVE: The project aimed at determining whether patients admitted to hospital for the treatment of epistaxis can be mobilised instead of the conventional regime of bed rest without increasing the risks of new bleeding episodes. BACKGROUND: A survey of nursing regimes for epistaxis patients has shown that patients were prescribed bed rest in 11 of 16 hospital departments. This prescription has been based on the assumption that bed rest reduces the risk of renewed bleeding. There is no scientific documentation of the need for bed rest demonstrating any reduced risks of new bleeding episodes

[Does bedrest after embryo transfer cause a worse outcome in in vitro fertilization?].

OBJECTIVE: To evaluate the effect of bed rest following embryotransfer (ET) on the results of an in vitro fertilization (IVF) program. DESIGN: A prospective randomized study. SETTING: Gyn.-Obst. Department--Assisted Reproduction Centre, 1st Medical Faculty of Charles University Prague and General Hospital, Prague. METHODS: After randomisation of patients on two groups (N--no bed rest, L--bed rest), embryo transfer (ET) was performed and all patients stand-up 20 minutes later. Group N went home, group L continued bed rest in the hospital overnight. We compared results of IVF in both groups.

Maternal rest improves growth in small-for-gestational-age fetuses (<10th percentile).

BACKGROUND: Optimal management of fetuses diagnosed as small for gestational age based on an estimated fetal weight of <10th percentile represents a major clinical problem. The standard approach is to increase fetal surveillance with serial biometry and antepartum testing to assess fetal well-being and timing of delivery. Observational studies have indicated that maternal rest in the left lateral position improves maternal cardiac output and uterine blood flow. However, maternal bed rest has not been recommended based on the results of a randomized clinical trial that showed that maternal rest

A Comparison of the Incidence of Post-Dural Puncture Headache and Backache After Spinal Anesthesia: A Pragmatic Randomized Controlled Trial.

BACKGROUND: Although bed rest is recommended after spinal anesthesia to prevent the occurrence of post-dural puncture headache, current literature suggests that periods of bed rest did not prevent headache as well as increase the risk of other complications such as backache. However, information is scarce regarding an appropriate period of bed rest following a dural puncture. AIM: The aim of this study was to compare the incidence of post-dural puncture headache and backache after different periods of bed rest following spinal anesthesia. METHODS: This study was a pragmatic, parallel-group,

Cardiac function, structural, and electrical remodeling by microgravity exposure.

Space medicine is key to the human exploration of outer space and pushes the boundaries of science, technology, and medicine. Because of harsh environmental conditions related to microgravity and other factors and hazards in outer space, astronauts and spaceflight participants face unique health and medical challenges, including those related to the heart. In this review, we summarize the literature regarding the effects of spaceflight on cardiac structure and function. We also provide an in-depth review of the literature regarding the effects of microgravity on cardiac calcium handling. Our r

[Intestinal disorders].

Bed ridden patients commonly suffer a series of intestinal disorders which include: loss of appetite, swallowing disorders, a tendency to undergo gastric tract reflux, slowness in intestinal movements, accumulation of gas or fecal impactation. These contribute to an increased risk of potentially dangerous complications such as intestinal obstruction which can lead to necrosis or a perforation of different parts of the digestive tract, duodenal ulcer, hemorrhoids, or anal fissures.
