Treatment of bipolar disorder: new perspectives.

Treatment of bipolar disorder (BD) has traditionally focused on alleviation of acute symptoms and prevention of future recurrences. Current treatment guide-lines advocate more or less similar treatment algorithms for all patients. Such approach largely ignores the clinical, genetic, and pathophysiological heterogeneity of BD, which makes certain patients more (or less) likely to respond to specific treatments. Variables such as family history, comorbidity, course of illness, quality and duration of previous remissions, physical and medical comorbidity, and side-effects may help in selecting th…

Schizencephaly associated with bipolar II disorder.

A 55-year-old man with congenital hemiparesis of the right side, three episodes of generalised tonic-clonic seizure at 16 years of age, and … low-up. Although uncommon, schizencephaly may be considered as one of the differentials in cases of bipolar disorder along with congenital hemiparesis, mental retardation and/or seizures; and neuroimaging should be done to confirm the diagnosis.

Rapid cycling bipolar disorder--diagnostic concepts.

OBJECTIVES: This paper reviews the literature to examine the DSM-IV diagnostic criteria for rapid cycling in bipolar disorder. METHODS: Studies on the clinical characteristics of rapid cycling bipolar disorder were reviewed. To identify relevant papers, literature searches using PubMed and MEDLINE were undertaken. RESULTS: First observed in the prepharmacologic era, rapid cycling subsequently has been associated with a relatively poor response to pharmacologic treatment. Rapid cycling can be conceptualized as either a high frequency of episodes of any polarity or as a temporal sequence of ep…

Bipolar disorder: causes, contexts, and treatments.

Bipolar disorder is a chronic and often devastating illness that may go undiagnosed because of its complex and diverse presentation. Clinicians can provide psychological treatments, in conjunction with pharmacotherapy, that can reduce the frequency, severity, and duration of manic and depressive episodes. Because bipolar disorder is characterized by high degrees of comorbidity and high rates of medical complications, the clinician will frequently need to implement other treatments targeted to comorbid conditions, such as panic, generalized anxiety, substance abuse, and personality disorders. T…

[Lithium and bipolar disorder--a renaissance?].

OBJECTIVE: Overview and discussion regarding the use and relevance of lithium as an therapeutic agent for bipolar disorder in general, with a special focus on pregnancy, suicidality and possible side-effects. METHOD: Literature was handsearched and by searches of medline with special interest in recent results and comparative studies. RESULTS: Lithium is still of high importance in the treatment of bipolar disorder. It is unmatched regarding decades of clinical experience and scientific evidence. It has a proven efficacy in every spectrum of bipolar illness, with less profound data regarding…

Treatments for late-life bipolar disorder.

BACKGROUND: Bipolar affective disorder is not uncommon in the elderly; prevalence rates in the United States range from 0.1% to 0.4%. However, it accounts for 10% to 25% of all geriatric patients with mood disorders and 5% of patients admitted to geropsychiatric inpatient units. These patients often present a tremendous treatment challenge to clinicians. They frequently have differing treatment needs compared with their younger counterparts because of substantial medical comorbidity and age-related variations in response to therapy. Unfortunately, the management of geriatric bipolar disorder h…

Bipolar disorder: Neurochemistry and drug mechanisms.

Extract: The alternating states of mania, depression and mood moderation (euthymia) are so distinct that biological research in psychiatry was attracted to the study of bipolar disorders very early on. Moreover, the discovery of lithium as a simple metal ion that had a strong mood-stabilizing effect suggested that a simple biological pathophysiology might be easily discovered in manic-depressive illness and that this might lead the way to major biological discoveries in other mental disorders and human behavior in general. Overall this story has been heroic and exciting but it has left us in 2…

Gender differences in bipolar-II disorder.

UNLABELLED: Gender differences in bipolar-II disorder (BP-II) are understudied. STUDY AIM: was to test if there were gender differences in the clinical and family history features of BP-II. METHODS: Consecutive 374 BP-II private practice outpatients were interviewed by a senior psychiatrist using the Structured Clinical Interview for DSM-IV, modified to improve the detection of BP-II (by Benazzi and Akiskal 2003, J Affect Disord 73:33-38), the Montgomery Asberg Depression Rating Scale (MADRS), the Hypomania Interview Guide, and the Family History Screen. Logistic regression was used to study

Metabolism, lifestyle and bipolar affective disorder.

Lifestyle, illness and treatment factors in people with bipolar disorder (BD) may confer additional risk of morbidity and mortality to the increasing rates of obesity, metabolic syndrome, diabetes mellitus and cardiovascular mortality in the general population. The aim of this review is to examine whether the risk of obesity and related morbidity and mortality are raised in BD, and possible contributory effects of lifestyle, illness and treatment factors to this risk.Systematic search of Medline and Cochrane Collaboration for relevant studies followed by a critical review of literature was carr…

Valproate in bipolar disorder: 2000 onwards.

OBJECTIVE: To describe the evidence for the efficacy and tolerability of valproate in treatment of bipolar disorders, as well as factors associated with favorable or unfavorable outcomes. METHOD: Studies published from 2000 onwards were reviewed, as well as published abstracts. For clinical trials, randomized, prospective studies were emphasized. RESULTS: Several mechanisms of action of valproate on central nervous system neurons that may be relevant to its actions in bipolar disorders have been recently reported. These include inhibition of glycogen synthase kinase and activation of extrace…

The phenotypic spectra of bipolar disorder.

Many findings that seem to be inconsistent in bipolar disorder research could be explained by heterogeneity of the illness and by imprecise diagnostic boundaries. This review of published data finds support for the existence of three main subtypes of bipolar disorder: (1) classical, (2) psychosis spectrum and (3) 'characterological'. These differ with respect to clinical presentation and course of illness, family history and possibly long-term treatment response. For instance, in a series of genetic studies, lithium responders showed an episodic course of illness with a family history of mostl…

Ziprasidone monotherapy in pediatric bipolar disorder.

Four children, aged 7-16 years, with bipolar disorder were switched to ziprasidone from mood stabilizers, anticonvulsants, or other atypical antipsychotics because of poor response, troubling side effects, breakthrough symptoms, or concern over potential toxicity. Within 3 days, patients experienced a resolution of hypomania, hallucinations, aggression, irritability, depression, and insomnia. One 16-year-old, who was switched from carbamazapine, also required adjunctive lorazepam for situational anxiety; the others either responded to, or were ultimately managed with, ziprasidone monotherapy. …

Unmet clinical needs in bipolar disorder.

Bipolar disorder is a complex, chronic condition associated with considerable morbidity and mortality, including a high rate of suicide. Currently available treatment options for bipolar disorder fail to adequately address many of the important needs of bipolar patients. Long-term maintenance therapy with lithium has been shown to prevent further episodes of mania and depression and to decrease the likelihood of suicide. However, many patients stop lithium treatment after only a few weeks, because of either untoward side effects or other factors, such as the belief that they no longer require …

Neuropsychology of bipolar disorder: a review.

BACKGROUND: Bipolar disorder (BD) may be associated with significant and persistent cognitive impairment. The aim of this study was to describe the profile of cognitive deficits in BD at different phases of the illness and determine whether it is different from that of schizophrenia and unipolar (UP) depression. METHODS: A systematic review of the computerised literature of neuropsychological studies of BD published between 1980 and 2000. RESULTS: General intellectual function: this was largely preserved in BD. Impairments when present were limited to acute episodes and to performance scores…

The expanding pharmacopoeia for bipolar disorder.

Over the past decade, the number of treatments available for bipolar disorder has undergone an extraordinary expansion. In that period, valproate and olanzapine have received regulatory approval in the United States for the acute treatment of mania, and carbamazepine has been indicated for this condition in many other countries. In addition to those agents, a number of other anticonvulsants (in particular lamotrigine, gabapentin, and topiramate) are in trials, as are the atypical antipsychotics clozapine and risperidone, and other novel compounds. This article critically reviews the evidence f…

[Therapeutic strategies in refractory bipolar disorder].

Lithium is still regarded as the first choice substance in the prophylactic treatment of bipolar disorder. However, approximately one third of patients with a "classic" course of bipolar affective disorder do not adequately respond to lithium prophylaxis. The introduction of carbamazepine and valproic acid allowed a more differential syndrome- and course-orientated approach to the prophylactic treatment of bipolar disorder for the first time. However, about 10 to 20 percent of patients still remain refractory to standard regimes. Therefore, criteria for resistance to prophylactic treatment hav…

Lamotrigine in rapid-cycling bipolar disorder.

BACKGROUND: We evaluated the antidepressant and mood-stabilizing effects of lamotrigine, a novel anticonvulsant, in a group of rapid-cycling bipolar patients. Most were already nonresponders or poor partial responders to other conventional mood-stabilizing agents. METHODS: This open, naturalistic, and prospective study was conducted with five rapid-cycling bipolar patients (DSM-IV). Each received lamotrigine titrated to a minimum dose of 150 mg/day as monotherapy or in combination with other psychotropic agents. Patients were assessed with the Global Assessment Scale (GAS), Beck Depression In
