Shock after blast wave injury is caused by a vagally mediated reflex.

OBJECTIVE: Bomb blast survivors occasionally suffer from profound shock and hypoxemia without signs of external injury. We hypothesize that a vagally mediated reflex such as the pulmonary defensive reflex is the cause of shock from blast wave injury. This study was a prospectively randomized, controlled animal study. METHODS: By using a previously described model of blast wave injury, we randomized rats to one of four groups: control, blast-only, bilateral cervical vagotomy plus atropine 200 microg/kg i.p. only, and bilateral cervical vagotomy plus atropine 200 microg/kg i.p. before blast inj

Blast-Related Mild Traumatic Brain Injury: Case Report Highlighting the Long-Term Neurocognitive Sequelae Following Blast Exposure in Veterans.

Traumatic brain injury (TBI) remains a significant public health concern, particularly among military service members exposed to blast-related trauma. While mild TBIs often resolve within weeks, a subset of patients may experience lifelong, persistent, debilitating symptoms. We report a case of a previously healthy male combat medic who sustained a blast-related traumatic brain injury during deployment in Afghanistan, with no loss of consciousness or direct head trauma. Despite the initial mild presentation, he has experienced chronic post-concussive symptoms for over 15 years. This patient pr

Surviving the Storm: Insights Into the Pericardial Injuries and Roller Coaster of Multisystem Trauma in Bomb-Blast Victims.

Pericardial injuries due to penetrating trauma such as bomb-blast shrapnel are rare but are associated with significant mortality. Timely diagnosis and a multidisciplinary approach are essential for effective management. This case series, comprising two cases, emphasizes the need for rapid, appropriate imaging and multidisciplinary care in the management of all associated injuries in these complex polytrauma patients. It also underscores the need for a high index of suspicion despite negative clinical features of pericardial injuries for timely performing CT scans and adaptive treatment strate

Acute Facility Management of Blast Injuries In Low- and Middle-Income Countries: A Systematic Review and Meta-Analysis.

INTRODUCTION: Blast injuries can occur by a multitude of mechanisms, including improvised explosive devices (IEDs), military munitions, and accidental detonation of chemical or petroleum stores. These injuries disproportionately affect people in low- and middle-income countries (LMICs), where there are often fewer resources to manage complex injuries and mass-casualty events. STUDY OBJECTIVE: The aim of this systematic review is to describe the literature on the acute facility-based management of blast injuries in LMICs to aid hospitals and organizations preparing to respond to conflict- and …

Development of a finite element model for blast injuries to the pig mandible and a preliminary biomechanical analysis.

BACKGROUND: Explosive damages to the maxillofacial bones exhibit complex dynamic response processes; the current traditional model and methods for blast wounds are unable to meet research needs. The finite element (FE) method has obvious advantages in complex biomechanical analysis. The objective of this study was to develop an FE model for blast injuries to the pig mandible and investigate the feasibility of using FE method as an ideal research tool for mandible blast wounds. METHODS: A hexahedral FE model of a pig mandible was established to simulate explosive damage in air by using MIMICS …

Mass-casualty victim "surge" management. Preparing for bombings and blast-related injuries with possibility of hazardous materials exposure.

Bombings and other blast-related events place severe demands on pre-hospital and in-hospital systems. The resulting surge of victims can overwhelm the resources of any facility not prepared for such an event. The September 11 terrorist attacks underscore the urgency of our need for preparedness. The challenges become even more daunting when there is possible hazmat exposure as well; this means that adequate and rapid disposition of victims is even more critical in order to avoid contamination of hospitals systems or whole communities. Federal agencies have been designated and federal mandates

Blast exposure causes early and persistent aberrant phospho- and cleaved-tau expression in a murine model of mild blast-induced traumatic brain injury.

Mild traumatic brain injury (mTBI) is considered the 'signature injury' of combat veterans that have served during the wars in Iraq and Afghanistan. This prevalence of mTBI is due in part to the common exposure to high explosive blasts in combat zones. In addition to the threats of blunt impact trauma caused by flying objects and the head itself being propelled against objects, the primary blast overpressure (BOP) generated by high explosives is capable of injuring the brain. Compared to other means of causing TBI, the pathophysiology of mild-to-moderate BOP is less well understood. To study t

Blast injury leads to chronic shifts in protein expression of distinct astrocyte populations.

Blast traumatic brain injury results in chronic pathology, especially for those receiving repetitive injuries. To evaluate cellular changes induced by these pressure waves, we studied post-mortem prefrontal cortex of military personnel with a history of multiple blast exposures and military controls with no battlefield experience. Chronic increases in IBA1 (microglia) and GFAP (glial fibrillary acidic protein; astrocyte) immunoreactivity occurred in injured brains and also confirmed that GFAP-expressing astrocytes altered predominantly at interface regions of the brain: around blood vessels, t

The effects of blast-induced traumatic brain injury on brain cellular mechanics and differentiation.

Blast-induced traumatic brain injury (bTBI) causes significant disruptions in cellular and subcellular structures within the central nervous system (CNS) when an extremely large force is applied. The corresponding changes in biomechanical properties and cellular functionalities of neuronal and glial cells due to bTBI remain largely unexplored. In this work, high blast overpressures (BOPs) of 14.5 psi (single shockwave) and 29.0 psi (double shockwave) were applied to adult hippocampal progenitor cells (AHPCs) in two different directions (overpressure applied from 'top-to-bottom' and 'bottom-to-

Evaluating the effectiveness of handheld ultrasound in primary blast lung injury: a comprehensive study.

The incidence of blast injuries has been rising globally, particularly affecting the lungs due to their vulnerability. Primary blast lung injury (PBLI) is associated with high morbidity and mortality rates, while early diagnostic methods are limited. With advancements in medical technology, and portable handheld ultrasound devices, the efficacy of ultrasound in detecting occult lung injuries early remains unclear. This study evaluates the effectiveness of immediate lung ultrasound in diagnosing PBLI. The study involved 25 healthy male Bama mini-pigs subjected to BST-I-type biological shock wav…

Quantifying acute changes in neurometabolism following blast-induced traumatic brain injury.

Brain health is largely dependent on the metabolic regulation of amino acids. Brain injuries, diseases, and disorders can be detected through alterations in free amino acid (FAA) concentrations; and thus, mapping the changes has high diagnostic potential. Common methods focus on optimizing neurotransmitter quantification; however, recent focus has expanded to investigate the roles of molecular precursors in brain metabolism. An isocratic method using high performance liquid chromatography with electrochemical cell detection was developed to quantify a wide range of molecular precursors and neu
