[Autologous transfusion program].

The term autologous transfusion describes transfusion of any blood component that was donated by intended recipient. A recipient who serves as his or her own donor receives the safest possible transfusion in that the risks of transfusion-transmitted infection and alloimmunization are eliminated. A preoperative autologous transfusion program provides many benefits to the donor-patient the blood donor center and the hospital transfusion service; requires a good communication between the transfusion physician and the collecting facility and needs a rigorous technical organization of the blood ban

Transfusion Medicine and Immunohematology

Blood transfusion is essential and vital in the successful treatment of many malignant and nonmalignant hematological disorders. Children with thalassemia, adults with myelodysplastic syndromes, and patients with autoimmune hemolytic anemias, leukemias, or aplastic anemias become chronically dependent on blood transfusions. Modern treatment procedures such as high-dose chemotherapy and progenitor cell transplantation require intensive support with blood components and products. The serological basis of blood transfusion, the available blood components and products, and adverse effects of blood

Trends and outcomes in multicomponent blood transfusion: an 11-year cohort study of a large multisite academic center.

BACKGROUND: Most studies reporting on blood component utilization overlook patients transfused with more than one type of blood product (multicomponent transfusion). These patients are of importance, as they are large consumers of blood products and likely have different characteristics and outcomes than nontransfused patients and patients transfused with only one blood component type. Our study aimed to determine the prevalence of multicomponent transfusion at a large multisite academic center, as well as the patient characteristics and outcomes associated with multicomponent transfusion. Warm fresh whole blood transfusion for severe hemorrhage: U.S. military and potential civilian applications.

OBJECTIVE: The objective of this study was to review the history and current literature regarding the benefits and risks of warm fresh whole blood transfusion to include recent U.S. Army research from Afghanistan and Iraq. We also discuss current indications for its use as well as potential civilian applications for large-scale disasters. BACKGROUND: The use of warm fresh whole blood currently only persists in emergency life-threatening scenarios when tested stored blood components are not available. Recent combat operations in Afghanistan and Iraq have redirected attention on the benefits an

Next-generation blood products: Results of a survey on interest, indications, drivers, and barriers to adoption.

BACKGROUND: Next generation blood products (NGBPs), specifically dried plasma/platelets/cryoprecipitate/fibrinogen concentrate, and frozen platelets, can alleviate the strain on the blood supply due to their advantages of ease of transport and storage, reduced overall cost, and decreased wastage. STUDY DESIGN AND METHODS: A working group within the Association for the Advancement of Blood and Biotherapies developed a survey to determine the level of interest, potential indications, drivers, and barriers to the implementation of these NGBPs. RESULTS: Among the 125 respondents, familiarity wit

Risks and side effects of therapy with plasma and plasma fractions.

Transfusion of plasma can lead to adverse reactions or events. Immune-mediated reactions are most common--these include allergic and anaphylactic reactions, transfusion-related acute lung injury (TRALI) and haemolysis. They can range in severity from mild to fatal. Fluid overload and citrate toxicity can occur after rapid or massive transfusion. In developed countries, microbial transmission rates are low because of donor selection and testing. Pathogen reduction processes can be applied to either single-unit components (methylene blue) or plasma pools (solvent-detergent). They have the unwant

The alloimmunized patient: effective transfusion support and newer experimental approaches.

Refractoriness occurs in a proportion of patients who receive multiple platelet transfusions. Alloantibodies, in particular those directed against the class I human leucocyte antigens (HLA) present on the platelet surface, are frequently associated with accelerated platelet destruction and transfusion failure. Compatible platelets for transfusion may be found by selecting donations from HLA-typed individuals, crossmatching patients' sera against donor lymphocytes and platelets or both. Maintaining a large panel of HLA-typed apheresis donors is expensive and some mismatching is inevitable due t

Current issues in transfusion medicine in Norway.

Important current issues in transfusion medicine in Norway are discussed. Current patient legislation specifically defines blood donors as patients, and blood and blood products are defined as drugs. Donor selection is controversial, especially deferral of all persons born in, or having lived for more than one year in areas with high prevalence of infections that are transmitted by blood. The threshold for becoming a blood donor is high, but registered donors donate frequently, e.g. 2,4 whole blood donations per year on average. Some blood banks have specialized in multicomponent aphereis tech

Current risks in blood transfusion in Japan.

Over the past decades, the incidence of transfusion-transmitted diseases has been dramatically reduced. These reductions have been due to a multifocal approach to the collection, processing, and release of blood components. The estimated risks of transfusion-transmitted hepatitis viruses are now extremely small, but the possibility of infections with emerging pathogens always exists because preventive measures may not be available for all cases. Thus, some patients may be harmed before preventive measures are introduced. Beside transfusion-transmitted infections (TTI), unsolved residual risks

Stability of coagulation factors in feline fresh frozen plasma intended for transfusion after 1 year of storage.

OBJECTIVES: The most common use of plasma transfusion is for haemostatic purposes, but coagulation factor activities in stored feline plasma are unknown. The concentration and stability of coagulation factors I (fibrinogen), II, V, VII, VIII, IX, X, XI and XII in feline fresh frozen plasma (fFFP) stored for 1 year were studied. METHODS: Fifty-five units of fFFP were produced from 55 fresh whole-blood donations obtained from indoor healthy blood donor cats. Twenty-one units were stored for <2 weeks (T0) and 34 were stored for 1 year (T1). After the completion of storage, specific coagulation f

Male-predominant plasma transfusion strategy for preventing transfusion-related acute lung injury: a systematic review.

OBJECTIVES: To assess 1) the effectiveness of male-predominant plasma transfusion strategy for preventing transfusion-related acute lung injury and related mortality; and 2) whether this effect varies across different patient subgroups. DESIGN: Systematic Review and meta-analysis: Data were identified by querying MEDLINE and EMBASE (including proceedings of major conferences on blood transfusions), searching the Internet for hemovigilance reports, reviewing reference lists of eligible articles and contacting experts in the field. Eligible were all studies reporting transfusion-related acute l

[Successful treatment of severe hemorrhage by using thrombocyte suspension with cryoprecipitate in type III Willebrand disease].

A 24-year-old woman with severe haemorrhagic complication due to type III von Willebrand's disease is reported, as the first such case in Hungary. Analysis of seven family members affected by the type III mutant gene was compatible with an autosomal recessive mode of inheritance. Severe, iatrogenic epistaxis of the patient was not improved by cryoprecipitate and DDAVP. Platelet suspension together with cryoprecipitate immediately stopped bleeding. Simultaneous application of cellulare (platelet suspension) and plasmatic (cryoprecipitate) Willebrand protein is suggested for patients with severe

Whole blood first resuscitation and association of blood product utilization based on mechanism of injury.

INTRODUCTION: Studies evaluating the effect of whole blood (WB) vs. component-based resuscitation accounting for mechanism of injury are lacking. We sought to evaluate the effects of a WB first resuscitation strategy regarding mechanism of injury. METHODS: This is a retrospective cohort study of trauma patients who received any blood product during resuscitation from January 2016 to November 2021. The primary outcomes were total blood volume utilized within 24 h and 30-day mortality. Multivariable gamma regression with a log link was used for blood volume and logistic regression for mortality
