Physiologic effects of transfusing red blood cells with high or low affinity for oxygen to passively hyperventilated, anemic baboons: systemic and cerebral oxygen extraction.

Anemic, passively hyperventilated baboons were given preserved red blood cells either with increased or with slightly reduced affinity for oxygen to restore the red cell volume. In the high affinity group there was a 50% increase in cerebral blood flow immediately after the transfusion, but there was no significant change in the low affinity group. The cardiac output decreased slightly in the low affinity group, and increased slightly but insignificantly in the high affinity group. Two hours after transfusion the cerebral blood flow had returned to normal in the high affinity group. In both gr

Body mass index is associated with pulmonary gas and blood distribution mismatch in COVID-19 acute respiratory failure. A physiological study.

BACKGROUND: The effects of obesity on pulmonary gas and blood distribution in patients with acute respiratory failure remain unknown. Dual-energy computed tomography (DECT) is a X-ray-based method used to study regional distribution of gas and blood within the lung. We hypothesized that 1) regional gas/blood mismatch can be quantified by DECT; 2) obesity influences the global and regional distribution of pulmonary gas and blood; 3) regardless of ventilation modality (invasive vs. non-invasive ventilation), patients' body mass index (BMI) has an impact on pulmonary gas/blood mismatch. METHODS:

Role of Arterial Blood Gas in Risk Stratification of Patients With Acute Exacerbation of Chronic Obstructive Pulmonary Disease in the Emergency Department: A Systematic Review.

Acute exacerbations of chronic obstructive pulmonary disease are a leading cause of emergency department visits and hospital admissions, associated with high morbidity, mortality, and healthcare costs. Accurate risk stratification in the emergency department is vital for identifying patients at risk of deterioration and guiding decisions regarding intensive care unit admission or ventilatory support. Arterial blood gas analysis remains the gold standard for assessing respiratory failure, providing direct measurements of potential of hydrogen (pH), partial pressure of carbon dioxide in arterial…

Penile Doppler ultrasound study in priapism: A systematic review.

BACKGROUND: Penile Doppler ultrasound (PDU) is suggested to be an alternative to blood gas analysis (BGA) from the corpora cavernosa in differentiating between high- and low-flow priapisms, with limited supportive evidence. AIM: To compare penile Doppler ultrasound study and blood gas analysis in the diagnosis of priapism, through a systematic review of the literature. METHODS: Studies were identified by literature search of Medline, Scopus, Cochrane and ClinicalTrials.Gov. Studies were included if their participants had priapism evaluated by Penile Doppler ultrasound, and reported data on t…

[Effect of pulmonary reventilation on gas exchange after cryolytic removal of obstruction in endobronchial tumors].

Blood gas analysis was used to assess the value of cryolytic endobronchial disobstruction in functional recovery of the reventilated lung parenchyma, with particular reference to the real improvement brought about in blood gas equilibrium by reventilation. Blood gas analysis was carried out in 5 cases of malignant tracheobronchial neoplasia (3 with atelectasis or subatelectasis) before and after disobstruction. Improved gas exchange, primarily in the form of better blood oxygen values, was noted in all cases, with a mean improvement of 28% in 3 cases and 7% in 2. It should be noted, however, t…

Diaphragm microvascular plasma PO2 measured in vivo.

Phosphorescence quenching techniques measure microvascular PO2 without direct surgical manipulation of the tissue. At a given arterial PO2, microvascular PO2 reflects the local O2 uptake-to-O2 delivery ratio, i.e., VO2/QO2. We evaluated the potential of phosphorescence quenching to determine microvascular PO2 in the rat costal diaphragm (PdiaO2). PdiaO2 and arterial blood gases were monitored across transient changes of inspired O2 among 21, 10, and 100% and also during hypotensive states evoked by progressive phlebotomy. After a transit delay, PdiaO2 responded rapidly to alterations of inspir

[Role of carbon dioxide in the correction of coagulation hemostasis during hypoxia].

In acute animal experiments coagulation hemostasis reactions and blood acid-base state were investigated when dogs were breathing hypoxic (10% O2) or hypoxic-hypercapnic (10% O2, 5% CO2) gas mixtures. When given the hypoxic mixture, activation of blood coagulation was accompanied by depression of anti-coagulatory and fibrinolytic properties. These changes developed together with distinct hypoxemia, respiratory alkalosis and secondary metabolic acidosis. When given the hypoxic-hypercapnic mixture, no hypercoagulation occurred which can be explained by higher (than on the hypoxic mixture) paO2,

Selected blood chemistry values in mobility-impaired broiler breeder hens with suspected calcium tetany using the i-STAT handheld clinical analyzer.

Calcium tetany is a poorly defined disease of broiler breeder hens that results from acute hypocalcemia. It is characterized by impaired mobility, increased mortality, and absence of gross lesions that would explain the impaired mobility. To evaluate if hens with impaired mobility had calcium tetany or other abnormalities, blood values from normal and affected hens were determined using the i-STAT handheld clinical analyzer. Three flocks were evaluated weekly prior to peak production (range 25-30 wk of age) comparing normal hens to hens with clinically apparent calcium tetany. Calcium tetany s

Flow-controlled expiration improves gas exchange in anaesthetised horses undergoing orthopaedic surgery.

BACKGROUND: Flow-controlled expiration (FLEX) has been shown to significantly enhance oxygenation in horses under laboratory conditions. OBJECTIVE: This study aims to corroborate these findings by evaluating the effects of FLEX on gas exchange in a randomised clinical trial involving a large population of clinical horses undergoing orthopaedic surgery. STUDY DESIGN: Prospective randomised clinical trial. METHODS: A total of 406 healthy adult horses scheduled for elective orthopaedic procedures were recruited for this prospective clinical trial. Horses were randomly assigned to FLEX or VCV (

Development of a Low-Cost Spectrometer for Hemolysis Detection in Emergency Blood Gas Samples.

BACKGROUND: Hemolysis is a major cause of sample rejection in blood gas analysis. We aimed to develop a low-cost spectrometer for hemolysis detection in emergency laboratories. METHODS: The spectrometer was developed using a complementary metal-oxide-semiconductor (CMOS) sensor and DVD-ROM grating. We evaluated the linearity, precision, and interference resistance of the instrument in accordance with CLSI and China National Accreditation Service for Conformity Assessment (CNAS) guidelines. In 51 clinical blood gas samples subjected to mechanically simulated hemolysis, the correlation between …

Making sense of blood gas results.

Recent technologic advances have allowed the production and marketing of cage-side blood gas analyzers to private practitioners. The widespread use of cage-side portable blood gas analyzers in veterinary practices has increased the need to develop the basic skills of blood gas analysis as part of a tool kit for practicing veterinarians. Rapid expansion of emergency and critical care medicine as a specialty and increased numbers of veterinary emergency and veterinary specialty practices have occurred concurrently with the availability of blood gas analyzers that are affordable for private pract…

An overlooked aspect on metabolic acidosis at birth: blood gas analyzers calculate base deficit differently.

OBJECTIVE: Metabolic acidosis (MA) at birth is commonly defined as umbilical cord arterial pH < 7.0 plus base deficit (BD) ≥ 12.0 mmol/L. Base deficit is not a measured entity but is calculated from pH and Pco(2) values, with the hemoglobin (Hb) concentration [Hb] included in the calculation algorithm as a fixed or measured value. Various blood gas analyzers use different algorithms, indicating variations in the MA diagnosis. The objective was therefore to calculate the prevalence of MA in blood and extracellular fluid with algorithms from three blood gas analyzer brands relative to the Clinic

Evaluation of fetal and neonatal acid-base status.

Determination of fetal acid-base status, either at birth, intrapartum, or antepartum, is the gold standard for the diagnosis of fetal asphyxia. Indirect methods such as Apgar scores and fetal heart rate monitoring show at best only minimal correlation. It is evident that intrauterine hypoxia is a rare cause of central nervous system injury, and the finding of a normal acid-base status at birth should lead the clinician to search for other causes of central nervous system injury or birth depression. A combination of techniques including fetal acid-base assessment, electronic fetal heart rate mo
