Long-term changes in hepatic fibrosis following hepatitis C viral clearance in patients with and without HIV.

BACKGROUND: While acute changes in hepatic fibrosis are recognized shortly after achieving sustained virological response (SVR) using direct-acting antiviral therapies, long-term outcomes for the growing population of successfully treated patients with HCV remain uncertain. The aim of this study is to characterize long-term changes in fibrosis following SVR in patients with and without HIV and to identify potential factors associated with progression or regression of fibrosis. METHODS: We completed a prospective longitudinal study of 162 subjects with HCV (34% HIV-coinfected) with pre-treatme

Cost-effective therapeutic hypothermia treatment device for hypoxic ischemic encephalopathy.

Despite recent advances in neonatal care and monitoring, asphyxia globally accounts for 23% of the 4 million annual deaths of newborns, and leads to hypoxic-ischemic encephalopathy (HIE). Occurring in five of 1000 live-born infants globally and even more in developing countries, HIE is a serious problem that causes death in 25%-50% of affected neonates and neurological disability to at least 25% of survivors. In order to prevent the damage caused by HIE, our invention provides an effective whole-body cooling of the neonates by utilizing evaporation and an endothermic reaction. Our device is co

Aprotinin and transfusion requirements in pediatric craniofacial surgery.

OBJECTIVE: To evaluate transfusion requirements in children receiving aprotinin during craniofacial surgery. BACKGROUND: Pediatric craniofacial procedures may involve massive blood loss. Aprotinin may decrease perioperative blood loss and transfusion requirements. METHODS: Patients (aged 1 month to 3 years) who had major reconstructive craniofacial surgery received intraoperative aprotinin (13 patients) or placebo (13 patients). Administered colloids and blood products were recorded. RESULTS: Patients in the aprotinin and placebo groups had similar mean age, body weight, body surface area,

A case of Farber lipogranulomatosis.

A 35 month old girl had suffered from painful joint contractures of the whole body since a few months after birth, and she gradually developed numerous periarticular and subcutaneous nodules, hoarseness, swallowing difficulty with recurrent respiratory infections, nystagmus, and mental and developmental retardation. She was misdiagnosed as having juvenile rheumatoid arthritis at several university hospitals. Serologic studies for rheumatoid arthritis were all negative. Radiologic findings of the whole body showed osteoporosis and bony erosions; on brain CT the brain was diffusely atrophied. On

Cervical disc replacement with polyetheretherketone cages: clinical experience with 151 cases.

BACKGROUND: There is active debate amongst surgeons regarding best available method for cervical inter-body fusion. This study evaluates success of disc replacement with polyetheretherketone (PEEK) cage in patients operated for cervical radiculopathy or myelopathy. METHODS: This case series was conducted between 2008 and 2012 during which 151 patients were treated with cervical inter-body fusion with PEEK cages. PEEK cages were packed with cancellous bone taken from iliac crest. The duration of follow up was 1 year. Subsidence, fusion, cage migration, and/or breakage were assessed using seria

Update on the roles of angiography and embolisation in pelvic fracture.

Trauma accounts for approximately 1 in 10 deaths worldwide. The presence of a pelvic fracture increases this mortality risk. Successful management depends on accurate diagnostic staging and control of fracture-related haemorrhage. From the standpoint of the trauma surgeon, this necessitates thorough compression and stabilisation of the fracture using external compression, combined with retroperitoneal or preperitoneal packing. However, vascular injury in many parts of the body can preferentially and effectively be treated on an emergency basis with angiographic procedures, using superselective

Osteolytic sinusitis and pneumomediastinum: deceptive otolaryngologic complications of cocaine abuse.

Recreational cocaine abuse via intranasal "snorting," "free-base" smoking, "body-packing," or intravenous injection can be lethal. Increasing illicit use of cocaine hydrochloride and the misuse of legal over-the-counter (OTC) nasal drugs are known causative agents of nasal septal perforation with loss of taste and smell. Although 2 to 3 mg/kg is the recommended maximum dose for topical anesthesia, cocaine snorters may use 1,000 mg or more daily on a "run." Furthermore, the newer route of smoking the extracted volatile "free-base" form of the adulterated street drug provides a plasma concentrat

How soft is too soft? Tuning order and disorder in dimeric core-soft colloids with bond flexibility.

We employ molecular dynamics simulations to explore how internal flexibility affects phase transitions in soft-matter systems composed of dimers interacting via a core-softened potential with two characteristic length scales. Monomers are connected by harmonic springs with varying stiffness, allowing us to tune the dimer rigidity from highly flexible to nearly rigid. Flexible dimers reproduce the behavior of monomeric systems, displaying well-defined body-centered cubic (BCC) and hexagonal close-packed (HCP) crystalline phases separated by a narrow amorphous region. As the bond stiffness incre

The prevalence of venous thromboembolism of the lower extremity among thermally injured patients determined by duplex sonography.

BACKGROUND: Morbidity and mortality from venous thromboembolism (VTE) remains a significant problem for hospitalized patients. Despite the ample prospective literature defining the prevalence of VTE in hospitalized patient populations, the prevalence of VTE in the thermally injured population remains largely unknown. METHODS: We prospectively studied 148 thermally injured patients with hospital stays of greater than 3 days with lower extremity duplex ultrasonograms obtained at admission and discharge. RESULTS: Nine patients experienced VTE (6.08%). Eight of the nine deep venous thromboses we

Successful thyroidectomy in a patient with Hermansky-Pudlak syndrome treated with recombinant activated factor VII and platelet concentrates.

Hermansky-Pudlak syndrome is a rare autosomal recessive disorder characterized by the absence of platelet dense bodies in association with albinism. We present the use of recombinant activated factor VII (rFVIIa) in a patient with Hermansky-Pudlak syndrome who underwent total thyroidectomy because of a large richly vascularized nodule (10 cm) compressing the trachea. The patient had a prolonged bleeding time (> 20 min) that remained unchanged after platelet transfusions. However, after infusion of platelets plus rFVIIa, it diminished to 5 min. The platelet aggregation response to adenosine dip

Rabies among African wild dogs (Lycaon pictus) in the Masai Mara, Kenya.

A pack of African wild dogs (Lycaon pictus) ranging to the north of the Masai Mara National Reserve in southwestern Kenya was monitored from 1988 to 1989. During a 6-week period (August 1-September 13, 1989), 21 of 23 members of this pack died. Seven carcasses were retrieved, of which 4 were suitable for necropsy and histopathologic examination. Gross findings varied among individuals and included multiple bite wounds, synovitis, lymphadenopathy, submandibular, cervical, and vocal cord edema, blood in bronchi, bronchioles, stomach, and intestine, and interioventral lung lobe consolidation. Removal of press-through-packs impacted in the upper esophagus using an overtube.

Foreign bodies in the upper esophagus should be removed as soon as possible to avoid serious complications. However, removals of foreign bodies in the upper esophagus are very difficult, especially if they have sharp edges, such as press-through-packs (PTPs). We experienced four cases of the impacted PTPs in the upper esophagus which was successfully extracted endoscopically with the overtube. Because two edges of PTPs were so firmly impacted in the esophageal wall in all cases, the PTPs were not movable in the upper esophagus. However, after insertion of the overtube, PTPs became movable and

Multilevel vertebral body replacement with a titanium mesh spacer for aneurysmal bone cyst: technical note.

A 64-year-old male presented with abrupt tetraparesis caused by a minor impact. Diagnostic images obtained on admission showed an aneurysmal bone cyst visible in the cervical spine at the fourth to upper sixth level, although the patient had been wearing a halo brace to diminish the symptoms. The vertebral body from the fourth to the sixth level was dissected, and this space was packed with a titanium cage filled with ceramic bone fragments mixed with fibrin glue. The combination of a titanium cage and an anterior locking plate can be made easily for anterior spinal fusion with enough rigidity

[Aneurysmal bone cyst of the sixth cervical spine: case report].

A 19-year-old girl was admitted with a history of difficulty in moving her neck for several years and a sudden onset of neck pain three months before. Plain radiographs of the cervical spine revealed destruction of the left half of the 6th cervical body with an expansive soap-bubble appearance. Neurological examination on admission was within normal limits. The angiography and bone scintigraphy revealed no abnormality. MRI of T1-weighted image showed a cystic lesion with various signal intensities. T2-weighted image demonstrated a hyperintense balloon-like lesion in the vertebral body and left

Beyond geometric constraint: dynamic fluid-lattice-activated topochemical polymerization.

Topochemical polymerization affords lattice-directed polymers with high structural fidelity, but its scope is limited by the need for precise, static preorganization of reactive groups-an intrinsically fragile outcome of subtle intermolecular packing. Here we introduce dynamical fluid lattices into topochemical design to circumvent these geometric constraints, achieving near-quantitative monomer conversion even without ideal preorganization. Employing core-shell columnar liquid crystals, rapid molecular fluctuations within the fluid lattices enable temporary proximity of reactive sites that ac

Innervation of lung and heart in the ray-finned fish, bichirs.

Anatomical and functional studies of the autonomic innervation in the lung and the heart of the bichirs are lacking. The present review paper describes the presence of nerve fibers located in the muscle layers of the lung and its submucosa, the collection of unipolar neurons found in the submucosal and muscle layers of the glottis in a bichir species (Polypterus bichir bichir). Putative oxygen chemoreceptive, neuroepithelial cells (NECs) in the lung mucosa are also included. The latter share many immunohistochemical characteristics similar to those observed in the carotid body and neuroepithel

Loss of body weight and fat during exercise in a cold chamber.

Ten men spent one week in a cold climatic facility performing a simulated arctic military exercise demanding an energy expenditure of 13-16 MJ-day-1. Although the ration pack was adequate, extensive plate wastage led to a negative energy balance of 2.2 MJ-day-1. Fluid intake was also insufficient, with a 3.25% decrease of body weight, and a 9.7% decrease in skin thickness over the cold exposure. Extensive fat mobilization was indicated by a decrease of skinfold thicknesses, an increase of body density, and associated ketonuria and glycosuria. The fat breakdown far exceeded the calculated energ

Endoscopic removal of a press-through pack from the small bowel using double-balloon enteroscopy.

BACKGROUND AND AIMS: Press-through packs (PTPs) are a common cause of foreign body ingestion; however, their retention in the small bowel is rare. We present a case in which a PTP lodged at a stenotic site was safely removed using a double-balloon endoscope (DBE). METHODS: A 73-year-old woman with a history of ileal tuberculosis and resultant small-bowel stenosis presented with abdominal pain. CT revealed a high-density object in the lower abdomen, suspicious for a foreign body. A retrograde DBE approach was subsequently performed and revealed a PTP lodged in the ileum near the site of stenos

Behind the Unilateral Rhinorrhea: Delayed Pediatric Intranasal Foreign Bodies Presenting as Chronic Sinonasal Disease and Severe Complications.

Background and Clinical Significance: Intranasal foreign bodies are common pediatric otolaryngologic emergencies and are usually diagnosed and removed without difficulty. However, delayed or occult retention may mimic chronic unilateral sinonasal disease and occasionally result in severe inflammatory or infectious complications. This study presents three illustrative pediatric cases of delayed intranasal foreign bodies supported by a seven-year institutional experience. Case Presentation: A retrospective review was conducted of all pediatric patients (≤16 years) presenting with intranasal fore

Sonographic and CT imaging features of intestinal perforation from a pill and packing: A case report.

RATIONALE: Sharp foreign bodies such as toothpicks or chicken bones can lead to intestinal perforation. Small intestinal perforation secondary to foreign body ingestion is usually manifested as an acute abdomen without a history of trauma. Here we describe the diagnosis and treatment of a case of small bowel perforation caused by an ingested pill and its outer packing. PATIENT CONCERNS: An 84 years old male patient complained of right lower abdominal pain for 4 days and the pain was becoming progressively worse. DIAGNOSES: The patient, who has Alzheimer's disease, mistakenly took the pill (o
