Treatment of long bone fractures in children by elastic stable intramedullary nailing: Outcome and challenges in a unit with restricted technical platform.

INTRODUCTION: Elastic stable intramedullary nailing (ESIN), developed by the Nancy school in France, is the gold standard for surgical treatment of long bone fractures in children. In Africa, few works have been devoted specifically to this technique. OBJECTIVE: This study aimed to describe the outcome and to present the challenges with this technique in the treatment of long bone fractures in children. PATIENTS AND METHODS: This was a prospective and descriptive study over 4 years including patients aged 0-15 years old operated using ESIN. RESULTS: Sixty-two patients underwent ESIN, of who

Results of Antibiotic-Impregnated Cement/Polymer-Coated Intramedullary Nails in the Management of Infected Nonunion and Open Fractures of Long Bones.

Background Fractures of long bones can sometimes lead to complications such as infection or nonunion, resulting in significant patient morbidity. Surgical intervention and antibiotics are often necessary to treat these complications. Antibiotic-impregnated cement/polymer-coated intramedullary nails have emerged as an effective surgical treatment for infected nonunion and open fractures. These implants elude high concentrations of antibiotics at the infection site while stabilizing the fracture. Extensive research has shown promising results, with success rates ranging from 60% to 100%. Benefit

A Comparative Study of Treatment With External Fixator Versus Antibiotic Coated Intramedullary Nail in Infected Non-union Long Bones.

INTRODUCTION: Infected non-union of bones shows problems in providing stability and controlling the infection. The treatment comprises debridement with or without the use of antibiotic cement and achieving stability by either external or internal fixation with or without bone grafting. The use of antibiotic-impregnated cement-coated intramedullary nailing gives stability and treats the infection, thereby permitting early mobilization. External fixation is a definitive mode of fracture stabilization indicated in cases of infected non-unions of the humerus, tibia, and juxta-articular bone. The p

Femoral non-union above a fused knee: Successful treatment with exchange nailing and intramedullary delivery of bone morphogenetic protein.

Bone healing is a complex and well-orchestrated physiological process, in which bone repairs and regenerates regaining its original biomechanical and biochemical properties. It is estimated that 5 to 10% of all fractures are complicated by delayed union or non-union. Progression to non-union is thought to be multifactorial, even though the exact biological sequence remains obscure. Treatment should aim to addressing deficiencies in both the mechanical and biological components, along with eliminating co-factors that could negatively affect the locally induced fracture healing response. We repo

Intramedullary nailing with supplemental plate and screw fixation of long bones of patients with osteogenesis imperfecta: operative technique and preliminary results.

Although intramedullary fixation is the standard form of surgical fixation of the long bones of children with osteogenesis imperfecta (OI), it remains fraught with complications. Implant breakage, implant cut out, long bone fracture, nonunion, and rod bending have all been described as complications of intramedullary long bone fixation in children with OI. Supplemental fixation techniques represent an attempt to decrease these risks of surgical implant failure of intramedullary devices. A supplemental plate and screw construct at a fracture or an osteotomy site in addition to an intramedullary

An intramedullary nail coated with antibiotic and growth factor nanoparticles: An individualized state-of-the-art treatment for chronic osteomyelitis with bone defects.

Among various infections, chronic osteomyelitis is one of the most challenging in terms of treatment. This infection is more common among patients with open fractures and those who have undergone elective orthopedic procedures. The treatment of osteomyelitis requires high antibiotic doses and an aggressive and multifaceted surgical approach. The use of parenteral antibiotics alone, without debridement, is not sufficiently effective, due to the formation of sequestra and the low vascularity of the affected area. The surgical options available for patients with chronic osteomyelitis include sequ

Elastic Stable Intramedullary Nailing for High-Risk Proximal Femoral Unicameral Bone Cysts in Children: A Retrospective Case Series.

BACKGROUND: Proximal femoral unicameral bone cysts (UBCs) are challenging in children because the lesions occur in a weight-bearing region near the proximal femoral physis. This study describes outcomes after elastic stable intramedullary nailing-(ESIN)-based treatment and the indication-based use of selective transphyseal nail advancement. METHODS: We retrospectively reviewed consecutive skeletally immature patients with pathologically confirmed proximal femoral UBCs treated at 2 centers with curettage, high-speed burring, allogeneic bone grafting, and 2 titanium elastic nails. Minimum follo

Effect of postoperative fracture gap on bone union in femoral diaphyseal fractures treated with intramedullary nailing: a retrospective clinical study.

BACKGROUND: Intramedullary nailing (IMN) is considered the gold standard surgical technique for the management of femoral shaft fractures. Nonunion is characterized by the absence of complete healing within nine months post-injury or the lack of any radiographic evidence of callus formation within three months. In this study, we aimed to evaluate the effects of postoperative fracture interval and translation and other factors on bone union in femoral shaft fractures treated with IMN. MATERIALS AND METHODS: A total of 87 patients with femoral shaft fractures who underwent IMN surgery in our cl

Effectiveness of antibiotic-impregnated bone cement in treating postoperative infections after femoral shaft fracture treatment with intramedullary nailing.

Postoperative infections are a significant challenge in orthopedic surgeries, particularly after procedures involving femoral shaft fractures treated with intramedullary nailing. Antibiotic-impregnated bone cement (AIBC) offers a promising solution by providing targeted antimicrobial delivery to the surgical site, potentially enhancing infection control and bone healing. A retrospective analysis was conducted from January 2020 to January 2023, involving 40 patients who developed postoperative infections after femoral shaft fracture treatment with intramedullary nailing. All patients underwent

Recurrence rates after functional surgery versus amputation for nail squamous cell carcinoma not involving the bone: A systematic review.

Background Nail unit squamous cell carcinoma (nSCC) is a malignant subungual tumour. Although it has a low risk of metastasis and mortality, the tumour has a significant local recurrence rate. There is insufficient data to determine whether functional surgery is less effective than amputation for nSCC that does not involve the bone. Objectives We aimed to investigate existing data on the outcomes of functional surgery and amputation for nSCC without bone invasion. Materials and Methods We carried out an extensive search in PubMed, Embase, Cochrane Library, Web of Science, and Scopus for approp

Management of hypertrophic femoral diaphyseal nonunion due to sclerotic bone formation (corticalization) at the intramedullary nail tip after dynamization.

The aim of this study was to investigate the effect of the presence of the "cortex sign" (corticalization) in femoral diaphysis fractures determined by the dynamization of nails because of delayed union. The study included 12 patients with a closed transverse femoral fracture (AO 32a3) treated with dynamization (all the screws distal of the nail were removed) because of delayed healing and followed up for at least 2 years. These patients were evaluated for the presence of bone union, cortex-like sclerosis (corticalization) distal to the nail, and the distance of the corticalization from the jo

Precice Stryde® Magnetic Internal Lengthening Nail does not Impair Bone Healing Despite Radiographic and Clinical Symptoms.

AIMS: The Precice Stryde® internal magnetic lengthening nail allowed many patients a full weight-bearing experience during femur and tibia lengthening, but concerns over corrosion, pain and radiographic changes led to the implant's recall. Despite the recall, it is important to understand the rate of these occurrences and their influence on the overall success of the lengthening procedure. We aimed to investigate radiographic changes, patient-reported symptoms and bone healing indices for our cohort of Stryde lengthening. MATERIALS AND METHODS: Our surgical database and electronic medical rec
