BACKGROUND: Nasal bone fracture is a common injury, but its repair can be unexpectedly difficult. To date, intranasal gauze packing has been used to support the nasal bone in position and to maintain contact between the mucosa of the septum and the cartilage. This gauze packing, however, has a tendency to shift, resulting in an inflammatory response characterized by a foul odor until removal, which detracts from the comfort of the patient. We developed a novel device, the nasal bone clip (NBC), which provides a satisfactory nasal airway and improved comfort.

Critical sized defects (CSDs) are a serious challenge in orthopedics that require the development of more robust and effective treatments to improve quality of life for patients. Current CSD research is limited by the applicable and affordable animal models available. Mice would be the preferred species as they are cheaply housed and have many transgenic variations readily available; however, their small size makes CSD surgeries difficult and expensive. We propose the use of cerclage wires to achieve internal plate fixation. PEEK plates were secured to the right femur of 26 C57BL/6 mice using

Background: Kirschner wire (K-wire) fixation is widely used to repair metacarpal and phalangeal fractures. In this study, we simulated K-wire osteosynthesis of a 3-dimensional (3D) phalangeal fracture model and investigated the fixation strength at various K-wire diameters and insertion angles to clarify the optimal K-wire fixation method for phalangeal fractures. Methods: The 3D phalangeal fracture models were created by using computed tomographic (CT) images of the proximal phalanx of the middle finger in five young healthy volunteers and five elderly osteoporotic patients. Two elongated cyl

BACKGROUND: With a type C3 distal radius fracture it is extremely difficult to maintain the reduction and to restore congruity of the articular surface because the support for the volar and dorsal bone cortex has been lost. An external fixator crossing wrist in combination with Kirschner wire (K-wire) fixation was popularly used by most studies in recent years. But loss of reduction often occurred especially in the volar sides.
METHODS: A total of 30 cases of type C3 distal radius fracture were treated by a volar buttress plate combined with a transarticular external fixator and other techniq

Significant research exists regarding heat production during single-hole bone drilling. No published data exist regarding repetitive sequential drilling. This study elucidates the phenomenon of heat accumulation for sequential drilling with both Kirschner wires (K wires) and standard two-flute twist drills. It was hypothesized that cumulative heat would result in a higher temperature with each subsequent drill pass. Nine holes in a 3 × 3 array were drilled sequentially on moistened cadaveric tibia bone kept at body temperature (about 37 °C). Four thermocouples were placed at the center of four

PURPOSE: The purpose of this study was to define a safe trajectory with regard to iatrogenic posterior interosseous nerve (PIN) injury when drilling the bicipital tuberosity for EndoButton repair (Smith & Nephew Endoscopy, Andover, MA) of distal biceps tendon ruptures.
METHODS: Ten cadaveric forearms were dissected. The bicipital tuberosity was exposed and the biceps tendon detached. The supinator and PIN were exposed dorsally. A K-wire was drilled perpendicular to the surface of the tuberosity. By use of digital calipers, the distance from the exit point of this wire to the PIN was measured.

We treated a 6-year-old child for hyperteleorbitism. We performed a facial bipartition steel wire osteosynthesis of the frontal bone. After 7 years we observed two episodes of pneumococcal meningitis, which were treated with intravenous antibiotic, resulting in a prompt recovery. The computed tomographic scan and nuclear magnetic resonance image showed the steel wire included in the frontal sinus and in contact with the dura mater. Removal of the wire and suture of the dura allowed prompt recovery.

Paediatric bones exhibit greater elasticity due to their distinct biomechanical properties, enabling them to absorb and dissipate mechanical forces more effectively than adult bones. This inherent flexibility provides increased resistance to fractures under comparable loading conditions. However, despite this resilience, high-energy trauma can still result in complex injury patterns. Fractures involving the physis are of particular concern, as they carry a significant risk of growth disturbances and long-term deformities. We present a rare case of a girl in early childhood (preschool) who susta

BACKGROUND: Smart glasses (SG) are a wearable device consisting of a small computer built into a head-mounted monitor (HMM) that can display various kinds of information. Lateral side visualization is displayed on the HMM of SG so the operator may also see it and the anteroposterior (AP) direction simultaneously. The present pilot study aimed to investigate the usefulness of SG in wire insertion for bone fracture.
METHODS: We marked both the front and lateral sides of the shaft of an artificial femur bone. From the tip of the greater trochanter, we inserted a 2.4-mm Kirschner wire (K-wire) so

BACKGROUND: The "palm tree-like wiring" introduced by Kapandji and its modifications for proximal humeral fractures should be given the generic name "retrograde intramedullary multiple pinning through the deltoid 'V'" (IMPV). IMPV is still preferable for treating valgus-impacted four-part fractures in which K-wires have the advantage of working as an internal fixation material as well as a tool in fracture reduction.
METHODS: Three 2.4-mm K-wires formed into the desired shape are used as the intramedullary pins and a 3.0- to 3.2-mm upwardly angled hole for each wire is opened in the deltoid "

One hundred cases of osteosynthesis done by the use of intraosseous wiring are presented. Two techniques of wiring are described, one used in transverse fractures and arthrodeses in which an additional Kirschner wire is employed, the other in which No. 0 monofilament wire can be placed perpendicular to the line of osteosynthesis. Forty-seven fractures were treated and there was union in all cases; 53 arthrodeses were performed, with failure in five, due, it is believed, to a failure to observe the essential elements of the technique, namely the need to obtain congruous bone ends and to maintai

Background: Percutaneous Kirschner wire (K-wire) fixation of hand and wrist fractures is a common trauma procedure, yet there remains little consensus on the best management of wires postoperatively. If wire's ends are left external to the skin, it remains unknown which dressing regimen best reduces infection risk. We felt that a systematic review was required to assess the current consensus on this question within the published literature. Methods: An electronic search was carried out across multiple databases. Abstracts were screened by two independent reviewers against inclusion criteria an

Hammertoe deformities are among the most common conditions treated by foot and ankle surgeons. Many different types of proximal interphalangeal joint arthrodesis fixation techniques have been used. These include implant fixation, absorbable fixation, screw fixation, two-pin fixation, and single-Kirschner wire fixation. Each of these has their own set of associated advantages and disadvantages. One of the most common techniques to address hammertoe deformity is the use of Kirschner wires. Although Kirschner wires have been reliable and produced good outcomes, there are some drawbacks associated

Bone fixation in digital replantation must provide adequate rigidity, and must be applied in a fast and easy manner. Various fixation methods have been used so far with certain advantages and disadvantages. The authors report two new intramedullary proximal phalangeal implant designs to secure two amputated bone segments tightly. They compare their intramedullary implants with commonly applied K-wire fixation methods and plate fixation via biomechanical analysis in terms of bending rigidity and distraction strength. Their two-sided asymmetrical screw provided the highest rigidity and distracti

An uncommon case of a closed dorsal fracture dislocation of the base of the fifth metacarpal bone associated with a flake fracture of the hamate is described. Under general anaesthesia, the dislocation was reduced by traction and direct pressure over the metacarpal base. To maintain the reduction, Kirschner wires were inserted percutaneously.
