Cerebral perfusion pressure and abnormal intracranial pressure wave forms: their relation to outcome in birth asphyxia.

Intracranial pressure (ICP) studies were carried out in 14 infants with severe birth asphyxia and brain damage. A markedly low cerebral perfusion pressure (CPP) was noted in infants who died and in 1 infant who survived with cerebral palsy. The long-term ICP tracing revealed negative waves and plateau waves in 2 infants. Cushing response was noted in 2 infants who had elevated ICP. The value and significance of evaluated CPP and of abnormal waveforms are discussed.

Pathology of perinatal brain damage: background and oxidative stress markers.

PURPOSE: To review historical scientific background and new perspective on the pathology of perinatal brain damage. The relationship between birth asphyxia and subsequent cerebral palsy has been extensively investigated. The role of new and promising clinical markers of oxidative stress (OS) is presented. METHODS: Electronic search of PubMed-Medline/EMBASE database has been performed. Laboratory and clinical data involving case series from the research group are reported. RESULTS: The neuropathology of birth asphyxia and subsequent perinatal brain damage as well as the role of electronic fet

[Hypothermia after perinatal asphyxia reduces the risk of brain damage. But it's too early to recommend the method for routine treatment].

Two randomised controlled trials evaluating intervention with moderate hypothermia after perinatal asphyxia will probably be published this year. The first study, evaluating head cooling with a "cool cap" is already published, and preliminary data from the other study evaluating body cooling earlier have been presented. Both studies indicate that moderate hypothermia, initiated within 6 h after birth, for 72 h reduces mortality and survival with severe brain injury. Several issues must, however, be solved before postasphyctic cooling can be recommended as clinical routine. There are no data on

Varieties of deficit unawareness after brain injury.

The failure to recognize the existence of disease is known as anosognosia. This article provides a brief discussion of the evolution of this concept and reviews some qualitative differences in the manner in which the disavowal of neurogenic impairment is expressed. Theoretical explanations for the unawareness of deficit after neurologic illness include motivational-symbolic, cognitive subsystem, and supraordinate system theories. Observations from a clinically derived, structured awareness interview are presented, suggesting three factors that may underlie patients' apparent lack of awareness

[Maternal environment and developmental brain damages].

To estimate and to prevent the developmental brain damage caused by maternal environmental agents, the results were compared between epidemiological and experimental studies. I. Ethanol. The features of CNS involvement in human FAS (fetal alcohol syndrome) and FAE (fetal alcohol effects) were developmental delay and intellectual impairment. In rat FAS or FAE, a beneficial effect of supplementary zinc on the fetal cerebrum was observed, but was limited. The consistent dysformation of synapses during early brain development may be associated with the functional impairment of CNS in FAS and FAE I

[Significance of electroencephalography in the physical examination of children with encopresis].

At an one time transversal examination 28 of 36 children with encopresis exhibited abnormal findings, increased incidence of sharp waves, a back ground that was too slow for their age and subcortical dysfunctions was striking. The abnormalities are approached as constitutional risks and maturational retardation with increased environmental propensity. Increased frequency of enuresis and encopresis in premature craniosynostosis was found. Electroencephalography should be used in paediatric and child psychiatric examination as a not burdening screening method for clarifying the aetiologic condit

Diagnosis of rare dementia syndromes: an algorithmic approach.

The etiology of dementia can be diagnosed in most patients using a standard clinical approach consisting of physical, neurologic, and mental status examinations, and laboratory testing, lumbar puncture, and neuroimaging. In some cases, however, the clinical presentation or historical data are unusual, or the results of the workup are inconclusive or atypical. A rare cause of dementia may then be present and a complicated evaluation may be necessary to identify the specific disease process. A potentially useful approach to the diagnosis of rare dementing disorders consists of a series of diagno

Reading assessment post head injury: how valid is it?

The purpose of this study was to identify the passage dependency for three reading tests applied to the closed head injured (CHI) population and to determine whether reading the passage resulted in improved accuracy in responding to passage-related questions. Based on the performance of 10 CHI adults, passage dependency was low for all three tests. Only the Reading Comprehension Battery for Aphasia scores improved significantly when the subjects read the passages prior to responding. The results parallel those found with aphasic subjects and raise questions concerning the validity of these tes

Facial recognition 1990.

A review of recent studies of prosopagnosia suggests that the weight of evidence has shifted in favor of regarding it as a disability that can be produced by a right hemisphere lesion alone even though bilateral disease remains the more frequent anatomical basis. It is possible that prosopagnosia resulting from a right hemisphere lesion occurs only within the context of some atypical condition of the left hemisphere. "Types" of prosopagnosia continue to be postulated and the "identification of individuality" hypothesis continues to be advanced. Autonomic and covert recognition studies of proso

[Changes in somatosensory evoked potentials in carotid desobliteration. Diagnostic significance and possible consequences for therapy].

The cerebral function of 55 patients with stenosis and occlusion of carotid arteries was monitored intraoperatively by serial examination of somatosensory cortical evoked potentials (SEP). In 40 uncomplicated operations no significant alteration of the SEP was detected. In 13 examination series, however, reversible abnormalities were noted. Furthermore, in two cases irreversible SEP loss was documented. The nature and temporal development of SEP alterations enabled differentiation of possible etiological factors. Bilateral abnormalities were indicative of global hypoxemia whereas acutely arisi

Intensive training in chronic upper limb hemiparesis does not increase spasticity or synergies.

The authors assessed whether intensive training increases spasticity and leads to the development of "pathologic movement patterns," a concern often raised by Bobath-trained therapists. The authors used a baseline-control repeated-measures test to study 29 patients with chronic upper limb hemiparesis who received daily shaping training. Their results suggest that training has no adverse effects on muscle tone and movement quality.

[Mild and moderate hypothermia as a new therapy concept in treatment of cerebral ischemia and craniocerebral trauma. Pathophysiologic principles].

Hypothermia protects the brain and other vital organs during periods of ischaemia. We differentiate between mild (36-34 degrees C), moderate (33-29 degrees C), deep (28-17 degrees C) and profund (16-4 degrees C) hypothermia. During hypothermia, cerebral metabolic rate and cerebral blood flow decrease dependent on temperature. The relation between temperature and cerebral metabolism is expressed by the temperature coeffizient Q10, which is the ratio between two metabolic rates separated by 10 degrees C. The following factors contribute to decreases in cerebral blood flow seen during hypothermia
