[Calciphylaxis: fatal complication of cardiometabolic syndrome in patients with end stage kidney disease].

UNLABELLED: Calciphylaxis characterized by schemic skin ulceration due to subcutaneous small arterioles calcification, is a rare disease but usually fatal. Disorders of calcium metabolism and vascular calcifications are common in dialysis patients but calciphylaxis prevalence is low in patients with end stage renal disease. So we proposed other emergent factors implicated in calciphylaxis development. METHODS: We studied retrospective 8 patients who developed calciphylaxis in our service from january 2001 to december 2006. RESULTS: All patients were female with mean age at diagnosis 68.5+/-6

Bone health in patients with inborn errors of metabolism.

Inborn errors of metabolism encompass a wide spectrum of disorders, frequently affecting bone. The most important metabolic disorders that primarily influence calcium or phosphate balance, resulting in skeletal pathology, are hypophosphatemic rickets and hypophosphatasia. Conditions involving bone marrow or affecting skeletal growth and development are mainly the lysosomal storage disorders, in particular the mucopolysaccharidoses. In these disorders skeletal abnormalities are often the presenting symptom and early recognition and intervention improves outcome in many of these diseases. Many d

In Transfusion-Dependent Thalassemia Children, Increased Iron Overload is Associated with Lower Serum Alpha-Klotho, Which is Strongly Associated with Lower Total and Ionized Calcium Concentrations.

BACKGROUND: Patients with transfusion-dependent thalassemia (TDT) show disorders in calcium metabolism. The α-Klotho protein is predominantly expressed in tissues that are involved in calcium homeostasis, and lowered levels are associated with bone disease. The aim of the study is to examine the associations between low α-Klotho status and calcium metabolism in relation to iron status in children with TDT. METHODS: Calcium, α-Klotho, parathyroid hormone (PTH), calcyphosin, vitamin D3, phosphorous, fibroblast growth factor receptor 2 (FGFR2), as well as iron and erythron biomarkers were measur

The calcium-sensing receptor in human disease.

The discovery of the calcium-sensing receptor (CaR), a G protein-coupled receptor, has led to the elucidation of the pivotal roles of the CaR in systemic calcium homeostasis. The receptor is situated on the chief cells of the parathyroid glands, where it senses the extracellular Ca2+ concentration and in turn alters the rate of secretion of parathyroid hormone (PTH). The intracellular signal pathways to which the CaR couples include, but are not limited to, phospholipase C (PLC), and mitogen-activated protein kinases. The receptor is widely expressed in various tissues and likely serves import

Noncalcemic actions of 1,25-dihydroxyvitamin D3 and clinical applications.

Vitamin D is absolutely essential for the maintenance of a healthy skeleton. Without vitamin D, children develop rickets and adults exacerbate their osteoporosis and develop osteomalacia. Casual exposure to sunlight is the major source of vitamin D for most people. During exposure to sunlight, ultraviolet B photons photolyze cutaneous stores of 7-dehydrocholesterol to previtamin D3. Previtamin D3 undergoes a thermal isomerization to form vitamin D3. Increased skin pigmentation, changes in latitude, time of day, sunscreen use, and aging can have a marked influence on the cutaneous production of

Rickets, osteomalacia, and renal osteodystrophy. An update.

Rickets, osteomalacia, and renal osteodystrophy are disorders of the mineralization of bone that result from a lack of available calcium, phosphorus, or both. The diseases that result from numerous mechanisms present with a symptom-sign-radiographic complex with such a high degree of stereotypy that laboratory investigation is often required to distinguish one form from another. The disorders in children, known as rickets, produce bowing and other deformities of the long bones and dwarfism. These disorders are principally related to the profound effect of the deficiency states on the epiphysea

Effect of aging and castration on the changes in the levels of bone gamma-carboxyglutamic acid-containing protein in bone and serum of female rat.

Using female rats from 2 to 64 weeks old, the changes in bone gamma-carboxyglutamic acid-containing protein (BGP) levels in bone and serum were studied in relation to the changes in calcium metabolism during aging. Intestinal calcium transport, serum phosphorus level, bone origin serum alkaline phosphatase activity and serum BGP content were high in rapidly growing animals and then decreased with aging. Serum BGP content correlated well to bone origin serum alkaline phosphatase activity. Bone density, bone ash content and bone BGP content increased during aging. Bone BGP content was correlated

Vitamin D deficiency: a culprit in metabolic bone disease.

Bone formation and bone remodeling are complicated processes regulated by systemic hormones and paracrine factors which regulate calcium and phosphate fluxes and cellular differentiation. The many actions of vitamin D reinforces its importance in the process of growth, maturation and aging of bone. An understanding of these important regulators of bone metabolism is important to understanding the clinical disorders as they are related to alterations in vitamin D metabolism and metabolic bone disease. Specific disorders of vitamin D metabolism can be related to clinical disease states of aging,

The functional status of the parathyroid in adult spasmophilia patients estimated by plasma parathormone assay.

In 25 cases of spasmophilia the parathyroid function was assessed by complete exploration of the P-Ca metabolism and radioimmunoassay (RIA) of parathormone (PTH). Concomitantly the bone mineralization status was assessed by induced hypercalciuria tests and administration of 30 mg vitamin D2. RIA of PTH revealed increased values in 50% of the cases. The values fall within the range of secondary hyperparathyroidism, but this percentage varies with the values of the parathyroid function obtained by P-Ca metabolism explorations. The increased PTH values do no correlate with the level of plasma tot

Latest assessment methods for mitochondrial homeostasis in cognitive diseases.

Mitochondria play an essential role in neural function, such as supporting normal energy metabolism, regulating reactive oxygen species, buffering physiological calcium loads, and maintaining the balance of morphology, subcellular distribution, and overall health through mitochondrial dynamics. Given the recent technological advances in the assessment of mitochondrial structure and functions, mitochondrial dysfunction has been regarded as the early and key pathophysiological mechanism of cognitive disorders such as Alzheimer's disease, Parkinson's disease, Huntington's disease, mild cognitive

[Clinical significance of disturbed calcium metabolism].

The authors present literature data on the role of Ca-P metabolism and its regulators in the development of bone and cardiovascular pathology, intrauterine development, organogenesis, cell growth and differentiation. A large number of agents are currently available to modify Ca-P metabolism. (calcium and vitamin Dpreparations, PTH, calcitonin, stimulators of Ca-sensitive receptors, calcitriol receptor ligands, etc.) although their application is hampered by the dificulty of laboratory control and the lack of basic knowledge. It is maintained that raising awareness among practitioners about the

Calcium isotope ratio (δ44/42Ca) reflects kidney function and calcium excretion but not bone mineral density or bone turnover: a cross-sectional study.

UNLABELLED: Calcium isotope fractionation (δ⁴⁴/⁴²Ca) was evaluated as a biomarker of bone metabolism in 301 patients. δ⁴⁴/⁴²Ca correlated with urinary calcium excretion but not with bone mineral density or fracture status. Age, kidney function, and calcium intake influenced results, limiting its diagnostic utility for osteoporosis in clinical practice. Data on Calcium isotope fractions (δ44/42Ca) in isolated bone diseases are limited. Previously proposed as a diagnostic marker in metabolic bone disorders, we now extend our findings from monogenic bone diseases to patients with osteoporosis and

[Biochemical findings in osteoporosis. I. The significance of magnesium].

The authors examined in 60 patients with osteoporosis the serum and red cell magnesium and calcium content. Thirty-three patients suffered from senile osteoporosis, 18 patients from postmenopausal osteoporosis and nine had osteoporosis caused by corticoids. In the former two groups were signs of chronic magnesium deficiency, in the third groups there was a trend of low serum calcium levels. The results indicate the important role of magnesium in these disorders of bone metabolism.

Bone mineral disorder in chronic kidney disease: Klotho and FGF23; cardiovascular implications.

Cardiovascular factors are one of the main causes of morbidity and mortality in patients with chronic kidney disease. Bone mineral metabolism disorders and inflammation are pathological conditions that involve increased cardiovascular risk in chronic kidney disease. The cardiovascular risk involvement of bone mineral metabolism classical biochemical parameters such as phosphorus, calcium, vitamin D and PTH is well known. The newest markers, FGF23 and klotho, could also be implicated in cardiovascular disease.

Dynamic investigation for evaluation of calcium metabolism and parathyroid function.

Differently from other metabolic conditions, most of calcium metabolism disorders are diagnosed through simple detection of both serum and urinary excretion (24-h urine collection), levels of calcium, total and ionized form, and phosphate, and of calciotropic hormone serum levels, such as calcitonin, PTH and vitamin D metabolites. For the diagnosis and clinical monitoring of some metabolic bone diseases, such as osteoporosis and Paget's disease, the assessment of bone turnover is offering a useful tool for the evaluation of the therapeutic response in affected individuals. Serum Calcium as a Determinant of Fibroblast Growth Factor 23 Levels in Patients With Primary Hyperparathyroidism and Graves' Disease.

OBJECTIVE: Fibroblast growth factor 23 (FGF23) is a phosphate-regulating hormone with an emerging role in calcium homeostasis. However, its role in hypercalcemic conditions remains unclear. This study aimed to investigate the relationship between serum calcium and FGF23 levels in two hypercalcemic disorders: primary hyperparathyroidism (PHPT) and Graves' disease. METHODS: A cross-sectional analysis was performed in 98 patients with PHPT and 45 patients with Graves' disease. Clinical and biochemical parameters, including parathyroid hormone (PTH), phosphate, and 1,25-dihydroxyvitamin D3 (1,25(
