Tramadol as an analgesic for mild to moderate cancer pain.

In most cancer patients, pain is successfully treated with pharmacological measures such as opioid analgesics alone or opioid analgesics combined with adjuvant analgesics (co-analgesics). Opioids for mild-to-moderate pain (formerly called weak opioids) are usually recommended in the treatment of cancer pain of moderate intensity. There is a debate whether the second step of the WHO analgesic ladder, which, in Poland, is composed of opioids such as tramadol, codeine, dihydrocodeine (DHC), is still needed for cancer pain treatment. One of the most interesting and useful drugs in this group is tr

Methadone use in cancer patients with pain: a review.

In recent years a better understanding of the pharmacologic and pharmacokinetic properties of methadone, including equianalgesic ratios has led to its increased use as a second line opioid for the treatment of pain in patients with cancer. Methadone may be an important alternative for those who have side effects related to the use of other opioids because it has no known active metabolites, is well absorbed by oral and rectal routes, and also has the advantage of very low cost. However, it has a long, unpredictable half-life, which can result in accumulation and toxicity in some patients. In a

Iatrogenic painful neuropathic complications of surgery in cancer.

It is estimated that at least one out of four patients with cancer complains of pain originating from nerve injury. Nerve injury may result from direct invasion/compression by tumour, or by remote effect of the cancer such as paraneoplastic polyneuropathy. In many cases, the nerve injury is caused by medical therapy, or surgical interventions. Pain generated by drugs or medical acts is called iatrogenic. A common iatrogenic neuralgia is chemotherapy induced painful polyneuropathy. This neuropathy typically affects mostly the small myelinated and unmyelinated nerve fibres. Surgical and anaesthe

An update on the clinical use of methadone for cancer pain.

Methadone is a synthetic opioid agonist considered a second choice drug in the management of cancer pain. Methadone has a number of unique characteristics including excellent oral and rectal absorption, no known active metabolites, high potency, low cost, and longer administration intervals, as well as an incomplete cross-tolerance with respect to other mu-opioid receptor agonist drugs. For these reasons, methadone has the potential of playing a major role in the treatment of cancer pain. However, its use is limited by the remarkably long and unpredictable half-life, large inter-individual var

Inappropriate use of naloxone in cancer patients with pain.

Opioid overdose is rarely the primary cause of altered mental status in cancer patients receiving opioid therapy. The inappropriate administration of naloxone to reverse an abnormal mental status can cause severe withdrawal symptoms and pain. To illustrate this problem, we report the case of a patient inappropriately treated with naloxone and the results of a retrospective review of the medical records of 15 consecutive patients with cancer treated with naloxone in the emergency department over a 5-month period. We offer guidelines for a more thoughtful approach to the management of patients w

Persistent Pain After Breast Cancer Treatment, an Underreported Burden for Breast Cancer Survivors.

BACKGROUND: Many patients who have undergone surgery experience persistent pain after breast cancer treatment (PPBCT). These symptoms often remain unnoticed by treating physician(s), and the pathophysiology of PPBCT remains poorly understood. The purpose of this study was to determine prevalence of PPBCT and examine the association between PPBCT and various patient, tumor, and treatment characteristics. PATIENTS AND METHODS: We conducted a questionnaire-based cross-sectional study enrolling patients with breast cancer treated at Máxima Medical Center between 2005 and 2016. PPBCT was defined a

Pain catastrophizing, pain acceptance, and pain intensity among patients with cancer in Oman and Lebanon: a comparative cross-sectional study.

BACKGROUND: Despite advances in pharmacological management, cancer-related pain remains highly prevalent. Psychological processes such as pain catastrophizing and pain acceptance may shape pain perception, yet their independent and cross-national contributions remain underexamined. OBJECTIVES: This study examined psychological, clinical, and contextual predictors of pain intensity among patients with cancer in Oman and Lebanon and explored the statistical mediating role of pain catastrophizing in the association between country and pain intensity. METHODS: A comparative cross-sectional study

Altered Functional Connectivity in Pain-Related Brain Regions and Its Correlation with Pain Duration in Bone Metastasis with Cancer Pain.

Bone metastatic pain is thought to be a severe type of cancer pain that has refractory characteristics and a long duration. This study is aimed at exploring the brain functional connectivity (FC) pattern in lung cancer patients with bone metastatic pain. In this study, 27 lung cancer patients with bone metastatic pain (CP+), 27 matched lung cancer patients without pain-related complaints (CP-), and 27 matched healthy controls (HC) were recruited. All participants underwent fMRI data acquisition and clinical assessments. One-way ANOVA or a Mann-Whitney U test was applied to compare clinical dat

Efficacy and Safety of Two Methadone Titration Methods for the Treatment of Cancer-Related Pain: The EQUIMETH2 Trial (Methadone for Cancer-Related Pain).

CONTEXT: In the European Association for Palliative Care recommendations for cancer pain management, there was no consensus regarding the indications, titration, or monitoring of methadone. OBJECTIVES: This national, randomized, multicenter trial aimed to compare two methadone titration methods (stop-and-go vs. progressive) in patients with cancer-related pain who were inadequately relieved by or intolerant to Level 3 opioids. METHODS: The primary end point was the rate of success/failure at Day 4, defined as pain relief (reduction of at least two points on the visual scale and a pain score

The unheard pain: why patient-related barriers remain the weakest link in cancer pain management.

Cancer pain remains a major challenge in oncology and palliative care despite the availability of clinical guidelines, effective analgesics, opioids, educational interventions, and non-pharmacological strategies. This Perspective argues that patient-related barriers remain one of the weakest links in cancer pain management because they are often treated as secondary obstacles rather than core determinants of treatment effectiveness. These barriers include fear of addiction, concerns about adverse effects, fatalistic beliefs, limited pain communication, low adherence, insufficient knowledge, an

Modern pain neuroscience in clinical practice: applied to post-cancer, paediatric and sports-related pain.

In the last decade, evidence regarding chronic pain has developed exponentially. Numerous studies show that many chronic pain populations show specific neuroplastic changes in the peripheral and central nervous system. These changes are reflected in clinical manifestations, like a generalized hypersensitivity of the somatosensory system. Besides a hypersensitivity of bottom-up nociceptive transmission, there is also evidence for top-down facilitation of pain due to malfunctioning of the endogenous descending nociceptive modulatory systems. These and other aspects of modern pain neuroscience ar

Correlation between mental health and pain-related impairment in patients with chronic cancer-related pain.

INTRODUCTION: Cancer-related pain (CRP) has a high prevalence in cancer patients, with almost one-third of patients experiencing moderate to severe pain. Together with psychological problems, CRP is among the most common issues faced by cancer patients. According to the biopsychosocial pain model, pain is closely related to anxiety and depressive disorders, as well as poorer health-related quality of life (QoL) in cancer patients. Due to scarce data on changes in CRP levels and pain impairment in relation to mental disorders over time, this cross-sectional study investigated differences in men

Interventions for myofascial pain syndrome in cancer pain: recent advances: why, when, where and how.

PURPOSE OF REVIEW: Pain is one of the most feared and most common symptoms of cancer, experienced by 38-85% of patients. Pain in terminally ill cancer patients is a multidimensional experience caused by a diverse array of factors - cancer itself, its treatment or other causes. Studies have shown a high incidence of myofascial pain syndrome (MPS) in patients with cancer and the knowledge of myofascial trigger points (MTrPs) is important to address and manage existing pain, and to prevent the recurrence of pain. This review aims to summarize recent advances in interventions for managing MPS in p
