Impact of biofilm production by Candida species and antifungal therapy on mortality of patients with candidemia.

BACKGROUND AND OBJECTIVES: Few studies have investigated the clinical outcomes of patients with candidemia caused by Candida species with different levels of biofilm formation. We aimed to investigate the impact of antifungal therapy on the outcome of candidemia caused by Candida species that were categorised as low biofilm formers (LBFs), moderate biofilm formers (MBFs), and high biofilm formers (HBFs). METHODS: Adults with candidemia caused by LBF and HBF/MBF Candida species that were susceptible to fluconazole and caspofungin were included to investigate the impact of treatment with flucon…

Persistent Candidemia in adults: underlying causes and clinical significance in the antifungal stewardship era.

To investigate the causes and the clinical significance of persistent candidemia (PC) in adults diagnosed in a tertiary hospital with an active antifungal stewardship program. Retrospective cohort including all adults with candidemia from 2010 to 2018. PC was defined as any positive follow-up blood culture (BC) obtained ≥ 5 days from the first BCs yielding the same Candida species. PC was detected in 35/255 (13.7%) patients. There were no differences regarding antifungal adequacy in PC vs. non-PC (94.3% vs. 82.3%, p = 0.084) and primary source control (63.3% vs. 76.4%, p = 0.172) at the time o…

Epidemiology and antifungal susceptibility patterns of candidemia from a tertiary centre in Western Australia.

Candidemia is a common invasive fungal infection with a high mortality rate. We performed a retrospective audit of candidemia at a tertiary centre in Western Australia, 2005-2014. There were 167 episodes of candidemia due to 173 isolates of Candida. Candida albicans (40.5%), Candida glabrata complex (30.6%), Candida parapsilosis complex (14.4%) were the most common species causing candidemia across the study. Of the tested isolates, 17.7% (11/62) were non-susceptible to fluconazole and 13.6% (9/66) non-susceptible to caspofungin. 22.8% (8/35) C. glabrata complex were fluconazole resistant and …

Assessment of the anti-biofilm effect of micafungin in an animal model of catheter-related candidemia.

In cases where catheter-related candidemia (CRC) must be managed without catheter withdrawal, antifungal lock therapy using highly active anti-biofilm (HAAB) agents is combined with systemic treatment. However, the activity of HAAB agents has never been studied in in vivo models using bioluminescence. We assessed the efficacy of micafungin using a bioluminescent Candida albicans SKCA23-ACTgLuc strain in an animal model of CRC. We divided 33 female Wistar rats into five groups: sham (A), infected nontreated (B), treated with lock therapy (0.16 mg/ml) (C), systemically treated only (1 mg/kg) (D)…

Antifungal tolerance is a subpopulation effect distinct from resistance and is associated with persistent candidemia.

Tolerance to antifungal drug concentrations above the minimal inhibitory concentration (MIC) is rarely quantified, and current clinical reco … w tolerance isolates. Furthermore, isolates recovered from immunocompetent patients with persistent candidemia display higher tolerance than isolates readily cleared by fluconazole. Thus, tolerance correlates with, and may help predict, patient responses to fluconazole therapy.

Evaluation of the possible influence of trailing and paradoxical effects on the clinical outcome of patients with candidemia.

OBJECTIVE: Paradoxical growth (PG) and trailing effect (TE) are frequently observed during antifungal susceptibility testing (AFST). These two phenomena interfere with the determination of the minimal inhibitory concentration (MIC). The aim of this study was to assess the clinical impact of TE and PG. METHODS: We analysed the frequency of TE and PG of 690 Candida isolates collected from a population-based study performed in Spain (CANDIPOP) and correlated the results with clinical outcome of the patients. RESULTS: Around 70% (484/690) of the isolates exhibited TE to azoles. Candida tropicali

Recommendations for the diagnosis of candidemia in Latin America. Latin America Invasive Mycosis Network.

Candidemia is one of the most frequent opportunistic mycoses worldwide. Limited epidemiological studies in Latin America indicate that incidence rates are higher in this region than in the Northern Hemisphere. Diagnosis is often made late in the infection, affecting the initiation of antifungal therapy. A more scientific approach, based on specific parameters, for diagnosis and management of candidemia in Latin America is warranted. 'Recommendations for the diagnosis and management of candidemia' are a series of manuscripts that have been developed by members of the Latin America Invasive Myco…

Time to positivity as a diagnostic tool for catheter-related candidemia: A species-specific analysis.

OBJECTIVES: Early identification of the source of candidemia is critical for catheter management. Time to positivity (TTP) is an accessible marker of catheter-related bloodstream infection in bacteremia, but its role in candidemia remains unclear. We evaluated the ability of TTP to predict catheter-related candidemia across major species and compared its performance with differential time to positivity (DTP). METHODS: Single-center retrospective study assessing the diagnostic utility and optimal TTP cut-offs for catheter-related candidemia. Predictors of catheter-related source were analyzed …

Impact of COVID-19 on candidemia: Risk factors and outcomes in a southeastern US cohort.

Candidemia cases increased during the COVID-19 pandemic, likely due to increased critical care interventions. However, the pandemic's impact on candidemia risk factors and outcomes remains understudied. The objective of this study was to evaluate changes in candidemia risk factors and in-hospital mortality before and during the COVID-19 pandemic, stratified by COVID-19 status. We retrospectively analyzed adult candidemia cases (defined by Candida species isolated from blood cultures) at a single center in Albany, Georgia, USA (January 2017-May 2023). Patients were grouped as (1) pre-pandemic, …

Evaluating predictors of ocular complications and mortality in candidemia: Are repeat blood cultures key?

Candidemia can lead to ocular complications, including endophthalmitis and chorioretinitis. Management of candidemia traditionally included ophthalmic exams. However, due to concerns regarding the utility of performing ophthalmic exams in all patients, there is a need for prognostic factors suggestive of ocular complications. We sought to evaluate if positive follow-up blood cultures (FUBC) are associated with ocular involvement in candidemia. We conducted a single-center, retrospective review of candidemia cases at an academic medical center (2017-2022). Data on demographics, risk factors, op…
