Immediate Proximal Row Carpectomy for Severe Perilunate Dislocation Injuries: A Minimum 5-Year Follow-Up.

Background: In patients with perilunate injuries (PLI) with multiple ligamentous and bony injuries involving the proximal carpal row, open reduction and internal fixation (ORIF) can be difficult and lead to poor functional outcomes. Proximal row carpectomy (PRC) is an alternative procedure that has been used for severely comminuted fractures. The aim of our study is to evaluate the long-term functional outcome (minimum 5 years) of patients that underwent an emergency PRC for PLI. Methods: We conducted a retrospective study of patients who underwent PRC at our centre between 2001 and 2016. Only

New concept for total wrist replacement.

Wrist prostheses have never achieved the sort of clinical outcomes found with those of hips and knees. We have developed a novel uncemented modular wrist prosthesis with screw fixation, metal-on-metal coupling, and ball-and-socket articulation. Eight patients admitted for wrist arthrodesis to treat primary or secondary osteoarthritis (not rheumatoid) were operated on. The prosthesis reduced the amount of bone removed and spared the distal radioulnar joint. After 7 to 9 years we found that the fixed centre of the ball-and-socket articulation provided good stability and mobility, and relief of p

[Antelunar dislocation of the wrist. Apropos of 3 cases].

Volar perilunar dislocation of the carpus is rare. We report three cases here. The patients were 18, 23 and 31 years of age. The etiology was a traffic accident in all cases and palmar hyperflexion of the wrist was noted in all three. Palmar dislocation was associated with a scaphoid fracture in two cases. In the third case, there was an associated fracture of the radial styloid and capitate. The patients were operated on after a delay of 48 h in two cases and four weeks in one. The dorsal approach was used in two cases and the palmar approach in the other. Reduction, fracture osteosynthesis a

Transcarpal carpometacarpal dislocations, excluding the thumb.

During a 9-year period, we treated 13 patients with major fractures of bones of the distal carpal row creating instability of the corresponding carpometacarpal joints. Diagnosis of the injury was made early in only eight patients whose treatment consisted of open reduction, pin or screw fixation, and immobilization from 6 to 8 weeks. They showed no complications and had an excellent functional recovery. Diagnosis in five (38%) patients was delayed 1 week to 8 months after the accident. At initial presentation, symptoms included pain, loss of grip strength, and mild limitation of finger motion.

Physeal Growth Following In Situ Pinning for Slipped Capital Femoral Epiphysis and Hypothyroid Treatment: A Case Report.

Slipped capital femoral epiphysis (SCFE) is a common adolescent hip condition, most often seen during periods of rapid bone growth. Deficiency in thyroid hormone levels can lead to reduced bone turnover and altered epiphyseal plate activity, which may influence the outcome of SCFE pinning and other orthopedic interventions crossing the epiphysis of the femur. Our patient was a 12-year-old female child with a two-month history of atraumatic right hip pain who presented with bilateral slipped capital femoral epiphysis (SCFE) and underwent successful bilateral in situ pinning. An endocrinology wo

Pin placement in slipped capital femoral epiphysis.

Standard radiographs taken at right angles to each other during the pinning of a slipped capital femoral epiphysis may not demonstrate pins protruding into the joint. In this study roentgenograms of 110 patients (148 hips) with slipped capital femoral epiphyses were analysed by a three-dimensional mathematical model of the femoral head and pin position. A computer was used to determine the spatial distance between the tip of the pin and the subchondral bone of the femoral head. Pin protrusion was found in 41 hips. In 22, protrusion was sufficient to have transgressed the joint space. Pin protr

Excision of the Distal Pole of the Scaphoid and the Midcarpal Joint.

Background  Excision of the distal pole of the scaphoid is used to treat arthritis of the scaphotrapezial trapezoid (STT), radioscaphoid joint, and arthritis following scaphoid nonunion. Some patients develop midcarpal instability limiting utilization of this technique. Why some wrists develop postoperative instability while others do not, remains unclear. Purpose  To identify the wrists prone to developing midcarpal joint instability we evaluated the effect of midcarpal joint structure on force transfer through the wrist, we hypothesized that the force transfer will be further altered when a

Long-term results of vascularized bone graft for stage III Kienböck disease.

PURPOSE: Vascularized bone grafting (VBG) is one of the therapeutic approaches for treating advanced Kienböck disease; however, few reports on long-term outcomes are available for this technique. The purpose of this study is to evaluate long-term results by following up patients with stage III Kienböck disease for more than 10 years after VBG. METHODS: The study included 18 patients with advanced Kienböck disease (Lichtman stage IIIA, n = 10; stage IIIB, n = 8) who received VBG between 1996 and 2001 and were followed up for at least 10 years. Eleven patients received transplantation from the

Slipped capital femoral epiphysis in skeletally immature patients.

Fixation by a single screw is considered the current treatment of choice for a slipped capital femoral epiphysis. This approach promotes premature physeal closure. The use of a modified, standard, single, cannulated screw designed to maintain epiphyseal fixation without causing premature closure of the physis was reviewed in ten patients. The nine boys and one girl aged between 10.6 and 12.6 years with unilateral slipped capital femoral epiphysis (SCFE), were markedly skeletally immature (Tanner stage I, bone age 10 to 12.6 years). Clinical and radiological review at a mean follow-up of 44.3 m

Scapho-luno-capitate fusion with proximal lunate articular surface preservation for management of grade IIIA Kienböck's disease: a prospective case series.

BACKGROUND: Kienböck's disease is idiopathic lunate avascular necrosis, which may lead to lunate collapse, abnormal carpal motion and wrist arthritis. The current study aimed to assess the outcomes of treating stage IIIA Kienböck's disease by a novel technique of limited carpal fusion via partial lunate excision with preservation of the proximal lunate surface and scapho-luno-capitate (SLC) fusion. MATERIALS AND METHODS: We conducted a prospective study of patients with grade IIIA Kienböck's disease managed with a novel technique of limited carpal fusion comprising SLC fusion with preservatio

Aseptic necrosis of the hamate: a case report.

The case of a 25-year-old man with osteonecrosis of the hamate is reported. He had pain and swelling in his right wrist. The diagnosis was accomplished with plain radiographs as well as with MRI. The case was treated surgically that included resection of the necrotic bone. The occured cavity was filled with autogenous cancellous bone graft. In addition, capito-hamate arthrodesis was performed. Histopathological examination following the operation demonstrated avascular necrosis of the hamate. The arthrodesis was obtained four months after the operation.

Resurfacing capitate pyrocarbon implant as salvage procedure in several serious outcomes of carpal injuries. Clinical experience and follow-up.

BACKGROUND AND AIM: Resurfacing Capitate Pyrocarbon Implant has been introduced in the surgical practice as an alternative method to restore wrist motion, strength and functions in patients suffering from wrist osteoarthritis. It has already been well described in the literature as a treatment for advanced stages of degenerative wrist diseases which follow scaphoid's and lunate's injuries such as scapho-lunate advanced collapse, scaphoid non-union advanced collapse, and advanced stages of Kienböck disease. Authors extended the use of RCPI to other selected cases of complicated wrist injuries,

Homology of the adductor pollicis and contrahentes muscles: a study of monkey hands.

The deep palmar muscles in monkey hands were studied. The contrahentes muscles mainly arose from the capitate bone, descended palmar to the deep palmar branch of the ulnar nerve and the palmar metacarpophalangeal nerves, and attached to the proximal phalanges or wing tendons of the second, fourth and fifth fingers. In relation to the deep palmar branch of the ulnar nerve and the palmar metacarpophalangeal nerves, the contrahentes muscles are homologous with the adductor pollicis and flexor indicis radialis muscles. The contrahentes muscles occasionally gave off some accessory slips which blend…

Magnetic resonance imaging-assessed synovial and bone changes in hand and wrist joints of rheumatoid arthritis patients.

BACKGROUND/AIMS: Magnetic resonance imaging (MRI) is a sensitive and useful method for the detection of synovitis and joint destruction in rheumatoid arthritis (RA) patients. However, the patterns of MRI-detected bone erosion, bone marrow edema (BME), synovitis, and tenosynovitis have received insufficient attention. Therefore, this study evaluated the patterns of bone erosion, BME, synovitis, and tenosynovitis, and calculated the RA-MRI score (RAMRIS) of patients with RA at the carpal and metacarpophalangeal (MCP) joints using MRI. METHODS: MRI datasets from 43 RA patients were analyzed. All

Endocrine disorders and orthopedic problems in children.

Endocrine excesses or deficiencies frequently result in alterations of height as compared with age-matched children. Less clinically obvious are significant alterations in bone metabolism leading to osteoporosis, which can affect the child and may affect the patient as an adult. Slipped capital femoral epiphysis appears to be associated with a variety of endocrine disorders, particularly hypothyroidism. The hormonal treatment of various endocrine and nonendocrine disorders has been associated with alterations of bone metabolism, or of bone maturation, as well as with producing or aggravating o

Osteochondroma of the capitate causing rupture of the extensor digiti minimi: case report.

Osteochondromas usually arise from the metaphyses of long bones. Involvement of the carpal bones is extremely rare. We report a case of an osteochondroma originating from the dorsal capitate in a 51-year-old man. Attritional rupture of the extensor digiti minimi tendon resulted from mechanical irritation as it coursed over the mass. Surgical excision of the mass with tendon transfer of the extensor digiti minimi was subsequently performed.
