Neuropsychological Alterations in Narcolepsy with Cataplexy and the Expression of Cognitive Deficits.

OBJECTIVES: The objective of our study was to assess attention processes and executive function in patients with narcolepsy with cataplexy (NT1). To do so, we compared the results with those of a control group from the general population using an extensive neuropsychological test battery. METHODS: We studied 28 patients with NT1 and 28 healthy control participants matched for age, gender, and educational level. They all completed questionnaires on sleepiness, anxiety, and depression symptoms. In addition, they underwent neuropsychological tests. The ability to maintain attention was assessed …

GABA Cells in the Central Nucleus of the Amygdala Promote Cataplexy.

Cataplexy is a hallmark of narcolepsy characterized by the sudden uncontrollable onset of muscle weakness or paralysis during wakefulness. It can occur spontaneously, but is typically triggered by positive emotions such as laughter. Although cataplexy was identified >130 years ago, its neural mechanism remains unclear. Here, we show that a newly identified GABA circuit within the central nucleus of the amygdala (CeA) promotes cataplexy. We used behavioral, electrophysiological, immunohistochemical, and chemogenetic strategies to target and manipulate CeA activity selectively in narcoleptic (or…

An interesting case of late age at onset of narcolepsy with cataplexy.

The usual age at onset of narcolepsy with cataplexy is in the second or third decade. In cases with late onset narcolepsy with cataplexy, symptoms are usually mild with relatively less severe daytime sleepiness and less frequent cataplexy. Here we present a case of narcolepsy with cataplexy with onset of symptoms around sixty years of age. This case is unique, with severe daytime sleepiness both by subjective report as well as on objective Multiple Sleep Latency Test and having multiple cataplexy episodes in a day.

Hallucinations in narcolepsy with and without cataplexy: contrasts with Parkinson's disease.

BACKGROUND: Narcolepsy and Parkinson's disease (PD) are associated with hallucinations, excessive daytime sleepiness, REM sleep behavior disorder (RBD), as well as complete (narcolepsy with cataplexy) vs. partial (PD, narcolepsy without cataplexy) hypocretin-1 deficiency. OBJECTIVE: To compare the hallucinations associated with narcolepsy to those of PD. METHODS: One hundred patients with narcolepsy (with and without cataplexy) and 100 patients with PD were consecutively interviewed about their hallucinations (frequency, phenomenology, insight into unreality and association with sleep) as we…

Month of birth is not a risk factor for narcolepsy with cataplexy in the Netherlands.

The month of birth has been proposed as a risk factor for narcolepsy, suggesting a harmful influence during early development. Several autho … ulation data. Month and year of birth were noted for 307 patients with non-familial narcolepsy with cataplexy, born in the Netherlands between 1923 and 2001. The numbers of live births per month and per year from the entire Dutch population for the same period were used to calculate a virtual data set of expected births per month with exactly the number of …

Dopaminergic regulation of sleep and cataplexy in a murine model of narcolepsy.

STUDY OBJECTIVES: To determine if the dopaminergic system modulates cataplexy, sleep attacks and sleep-wake behavior in narcoleptic mice. DESIGN: Hypocretin/orexin knockout (i.e., narcoleptic) and wild-type mice were administered amphetamine and specific dopamine receptor modulators to determine their effects on sleep, cataplexy and sleep attacks. PATIENTS OR PARTICIPANTS: Hypocretin knockout (n = 17) and wild-type mice (n = 21). INTERVENTIONS: Cataplexy, sleep attacks and sleep-wake behavior were identified using electroencephalogram, electromyogram and videography. These behaviors were mo…

Neuraxial anaesthesia for caesarean section in a patient with narcolepsy and cataplexy.

We report the management of a morbidly obese parturient suffering from narcolepsy with cataplexy who presented for caesarean section. Her symptoms were controlled by modafinil and fluoxetine. We administered epidural anaesthesia successfully; surgery was uneventful and recovery was uncomplicated. We discuss some of the issues surrounding parturients with narcolepsy including drug therapy and choice of anaesthetic technique.

No persistent effect of intravenous immunoglobulins in patients with narcolepsy with cataplexy.

We report on four patients with narcolepsy with cataplexy (NC), who were treated with high-dose intravenous immunoglobulins (IVIg). Although in some patients transient effects were seen of both objective (multiple sleep latency test and maintenance of wakefulness test) and subjective symptoms (Epworth Sleepiness Scale and frequency of cataplexy), these effects lasted at the most for a few weeks and did not persist. Our report challenges the recent observations of a favorable and persistent effect of IVIg in NC patients.

Therapies for narcolepsy with or without cataplexy: evidence-based review.

(1) Narcolepsy is a rare disorder of unknown aetiology characterized by excessive daytime sleepiness and typically associated with cataplexy. It is extremely incapacitating, and frequently results in impaired psychosocial functioning and reduced work performance. Currently there is no cure for narcolepsy, so treatment focuses on control of symptoms.(2) Although the sympathomimetic stimulants, such as amphetamines or methylphenidate, are effective in improving excessive daytime sleepiness in patients with narcolepsy, they have the potential for dependence, have sometimes disabling sympathomimet…

Daytime sleepiness with and without cataplexy in Chinese-Taiwanese patients.

BACKGROUND AND PURPOSE: Investigation of Chinese-Taiwanese patients with excessive sleepiness, but no association with other sleep disorders, and with the presence or absence of cataplexy. PATIENTS AND METHODS: Thirty-five patients, successively referred between 2002 and 2004, underwent polysomnography (PSG), repeat multiple sleep latency test (MSLT), and human leukocyte antigen (HLA) typing. Three patients without cataplexy also had cerebrospinal fluid (CSF) hypocretin measurements. RESULTS: DQB1*0602 was associated with cataplexy in over 90% of Chinese-Taiwanese cases. Absence of cataplexy…

Effect of age on MSLT results in patients with narcolepsy-cataplexy.

OBJECTIVE: To measure the effect of age on Multiple Sleep Latency Test (MSLT)characteristics, sleep latency, and number of sleep-onset REM periods (SOREMP) in two large populations of narcoleptic patients with similar genetic backgrounds. METHODS: Clinical and polygraphic information on the severity of the condition was obtained on 236 well-defined narcolepsy-cataplexy-human leukocyte antigen DR2-positive patients from Montpellier (France) and on 147 similar patients from Montreal (Canada). RESULTS: The results show a progressive decrease in the number of SOREMP with age and a progressive in…

Diagnostic ambiguities in a case of post-traumatic narcolepsy with cataplexy.

Narcolepsy arising from trauma can present particular problems of differential diagnosis. In this case study presentation the patient suffer … latency versus number of sleep onset rapid eye movement periods, the somewhat atypical features of cataplexy, the coexistence of sleep apnoea, and the mildness of the original head injury. It is argued that cases of post-traumatic narcolepsy should be considered in the context of their clinical development over time and that practitioners should be aware t …

Successful management of cataplexy with intravenous immunoglobulins at narcolepsy onset.

Hypocretin/orexin deficiency appears to be a consistent feature of narcolepsy with a putative autoimmune mechanism involved. We treated four hypocretin/orexin-deficient narcolepsy patients with intravenous immunoglobulins and assessed the efficacy by repeated polysomnographies and questionnaires. Three patients received the treatment within a few months after acute onset of narcolepsy. A clear improvement in the frequency and severity of cataplexy was obtained with a benefic effect up to 7 months without any anticataplectics drugs at follow-up. Our findings point to the importance of early dia…

[Accidents prevalence in a group of patients with the narcolepsy- cataplexy syndrome].

INTRODUCTION: Recent studies have shown statistically that the proportion of accidents at work, at home and on the roads is greater in narcoleptic patients than in the general population. OBJECTIVE: The object of this study is to show the high prevalence of the risk of accidents in patients diagnosed as having the narcolepsy-cataplexy syndrome. PATIENTS AND METHODS: We studied a group of 35 patients of both sexes, diagnosed in the Sleep Pathology Unit of our hospital between 1994 and 1998 as having the narcolepsy-cataplexy syndrome. They filled out a questionnaire to find the prevalence of a…

Validation of a cataplexy questionnaire in 983 sleep-disorders patients.

Our goal was to validate a self-administered narcolepsy questionnaire focusing on cataplexy. Nine hundred and eight three consecutive subjects entering the Stanford Sleep Disorder Clinic completed the questionnaire. Clinic physicians reported on the presence or absence of "clear-cut" cataplexy. Responses to 51 cataplexy-related questionnaire items were compared between subjects with clear-cut cataplexy (n = 63) and all other patients (n = 920). As previously reported, a large portion of the non-narcoleptic population was found to experience muscle weakness with various intense emotions (1.8% t…
