An unusual seat-belt injury in a doctor.

The personal experience of a general practitioner who sustained a fracture-dislocation of the atlanto-axial joint is recorded. The inefficient application of an unyielding (non-inertial) lap and diagonal seat belt permitted this injury, although one does not know what other injuries might have occurred had the belt not been worn.

Risk Factors for Fracture Nonunion and Transverse Atlantal Ligament Injury After Isolated Atlas Fractures: A Case Series of 97 Patients.

BACKGROUND: The management of atlas fractures is controversial and hinges on the integrity of transverse atlantal ligament (TAL). OBJECTIVE: To identify risk factors for atlas fracture nonunion, with and without TAL injury. METHODS: All isolated, traumatic atlas fractures treated at our institution between 1999 and 2016 were analyzed. Multivariable logistic regression was used to identify variables associated with TAL injury confirmed on MRI, occult TAL injury seen on MRI but not suspected on computed tomography (CT), and with fracture nonunion on follow-up CT at 12 weeks. RESULTS: Lateral

[Isolated fracture of the lateral mass of the atlas: a case report].

Isolated fracture of the lateral mass of the atlas is extremely rare. The authors report such a case because of its rarity and to emphasize the usefulness of computed tomography (CT) for its diagnosis. The case was that of a 63-year-old male, who had been hit on his left parietal region by a board falling from behind, and which forced him to hyperflex his neck. He complained of neck pain on arrival at our hospital without any resulting neurological deficits. Routine plain cervical spine films were normal, but CT scan revealed a vertical fracture of the lateral mass of the atlas. He was placed

Reduction of cervical dystonia after an extended course of chiropractic manipulation: a case report.

OBJECTIVE: The diminution of the signs and symptoms of cervical dystonia following an extended course of specific chiropractic manipulation is described. CLINICAL FEATURES: A 38-year-old man had gross anterior-lateral torticollis, focal dystonia of the head and neck, and radicularlike pains which failed to respond to physical therapy, medication, and injection. INTERVENTIONS AND OUTCOMES: Two specific spinal manipulative technique systems unique to the chiropractic profession (Applied Biostructural Therapy [ABT] and Atlas Coccygeal Technique [ACT]) were applied to the patient. The patient's

Persistent torticollis, facial asymmetry, grooved tongue, and dolicho-odontoid process in connection with atlas malformation complex in three family subjects.

Congenital clefts and other malformations of the atlas are incidental findings identified while investigating the cervical spine following trauma. A persistent bifid anterior and posterior arch of the atlas beyond the age of 3-4 years is observed in skeletal dysplasias, Goldenhar syndrome, Conradi syndrome, and Down's syndrome. There is a high incidence of both anterior and posterior spina bifida of the atlas in patients with metabolic disorders, such as Morquio's syndrome [Baraitser and Winter in London dysmorphology database, Oxford University Press, 2005; Torriani, Lourenco in Rev Hosp Clin

Ethnic differences in craniofacial and upper spine morphology between European and Asian children with skeletal Class III malocclusion.

INTRODUCTION: The aims of this study were to analyze differences in craniofacial and upper cervical spine morphology, including posterior cranial fossa and growth prediction signs between European and Asian skeletal Class III children, and to analyze associations between morphologic deviations in the upper cervical spine and craniofacial characteristics. METHODS: A total of 60 skeletal Class III children, 19 Danes and 41 Koreans, were included. Upper spine morphology, Atlas dimensions, and craniofacial morphology, including posterior cranial fossa and growth prediction signs, were assessed on

Developmental anatomy of the atlas and axis in childhood by computed tomography.

OBJECT: The CT modality plays a central role in the diagnosis of cervical spine fractures. In childhood, radiolucent synchondroses between ossification centers can resemble fractures, and they can be the sites of fractures as well. Recognition of cervical spine fractures in children requires familiarity with normal developmental anatomy and common variants as they appear on CT scans. METHODS: A convenience sample of 932 CT scans of the cervical spine accessible on the picture archiving and communications system (known as PACS) at a single children's hospital was examined. Scans were excluded

Direct osteosynthesis in the treatment of atlas burst fractures: a systematic review.

PURPOSE: The treatment of unstable atlas fractures remains a controversial topic. The study aims at assessing the prognosis and efficacy of osteosynthesis for unstable atlas fractures through a review of the current literature and additionally aims to compare outcomes between the transoral and posterior approaches. METHODS: A systematic review of databases including PubMed, EMBASE, Cochrane, Web of Science, CNKI, and Wanfang was conducted. Titles and abstracts were screened by two reviewers to identify studies meeting pre-defined inclusion criteria for comprehensive analysis. RESULTS: The sy

A biomechanical study of Jefferson fractures.

STUDY DESIGN: Fifteen specimens of the first cervical vertebra were tested by the application of pure tensile forces to failure. Seven specimens had intact transverse ligaments, and eight had transection of the transverse ligament before testing. Specimens were tested to failure by the rapid application of laterally directed tensile force to the ring; this force then was exerted through the lateral masses to simulate the mechanism of injury for this fracture as proposed by Jefferson. OBJECTIVES: To measure the biomechanical characteristics of the C1 ring, including the fracture patterns creat
