[Laparoscopic cholecystectomy: complications and prevention].

In this report, 140 cases underwent laparoscopic cholecystectomy (LC) with preoperative diagnosis of cholelithiasis in 134, noncalculous cholecystitis, and gallbladder polypus in three each. Ten patients had a history of abdominal surgery. Ten patients were converted to open surgery because of severe adhesions and hemorrhage. Complications were found in 6 including subcutaneous emphysema in 2, subhepatic infection in 1, bile fistula in 2, and postoperative intra abdominal hemorrhage in 1.

Partial cholecystectomy--safe and effective.

Patients undergoing surgical treatment for calculous disease were considered to have had a partial cholecystectomy performed when a part of the gall bladder wall was retained for technical reasons. Forty patients underwent partial cholecystectomy: for chronic cholecystitis (20), acute cholecystitis (4), Mirizzi's syndrome (14), portal hypertension or partially accessible gall bladder (one patient each). Four patients (10%) developed infective complications and two patients had retained common bile duct stones. In a mean follow up period of 13 months (range 1-36 mths), only 3 patients have ongo…

Intraoperative cholangiography for laparoscopic cholecystectomy.

Intraoperative cholangiography in the course of laparoscopic cholecystectomy is not only valuable to detect common bile duct stones, but also to delineate the anatomy of the biliary ducts, facilitate the dissection, avoid injuries to the biliary tract and identify other abnormalities, such as fistulas, cysts and tumors of the biliary system. Most surgeons use a variation of the transcystic injection of contrast. We describe herein a technique of cholangiography through the gallbladder, performed before starting any dissection in the cystic duct area, which has resulted in a rate of 92 percent …

Biliary complications of laparoscopic cholecystectomy.

Laparoscopic cholecystectomy has rapidly become the preferred treatment for symptomatic gallstones. Although this procedure has certain advantages over open cholecystectomy, concern has been expressed regarding the potential for bile duct injuries. In an effort to understand the scope of this problem, a retrospective review was performed of all patients referred to UCLA after having sustained biliary injuries during laparoscopic cholecystectomy. Over a 14-month period, 10 patients were referred to UCLA with 12 major bile duct injuries. One patient had a false positive cholangiogram leading to …

Laparoscopic cholecystectomy for acute cholecystitis.

Sixty-eight cases of acute cholecystitis managed by laparoscopic cholecystectomy (LC) are reviewed. Thirty-two patients were admitted up to 10 days after onset of symptoms and 31 were completed by LC. One patient was referred from intensive care with gangrenous acalculus cholecystitis and was completed by LC but required subsequent laparotomy to control a bleeding omental vessel. Five patients were admitted with recurrent attacks of pain and histology confirmed resolving acute cholecystitis. Thirty patients had LC on routine operating lists, having recently had pain within 10 days of admission…

Cost effectiveness of laparoscopic cholecystectomy.

An investigation was undertaken to determine whether hospital charges for laparoscopic cholecystectomy are higher than those for traditional open cholecystectomy. Thirty consecutive cases of successfully completed laparoscopic procedures in a single surgeon's experience were compared to 30 open cases performed within the previous calendar year. Patients undergoing open cholecystectomy were excluded if coexisting medical problems or complications prolonged hospitalization beyond 7 days. Mean patient age was comparable (open cholecystectomy = 47.3 +/- 2.9, laparoscopic cholecystectomy = 46.5 +/-…

Laparoscopic cholecystectomy in acute cholecystitis.

Between August 1989 and December 1990, twenty-five patients with a preoperative diagnosis of acute cholecystitis underwent laparoscopic cholecystectomy. Twenty-one patients (84%) had abdominal tenderness, 16 (64%) had leukocytosis, and 10 (40%) had fever. Eleven patients (44%) came to the hospital with only one of these previously mentioned clinical signs. Six patients (24%) had two clinical signs. Eight patients (32%) came to the physician with all three findings. The length of surgery correlated directly with the number of presenting clinical signs. The average operating time was 119 minutes…

Laparoscopic cholecystectomy in acute cholecystitis.

The experience of laparoscopic cholecystectomy in 79 patients with acute cholecystitis is described. This group is subdivided into acute and severe acute cholecystitis. These categories are defined. Six percent of our patients with acute disease and 30% of our patients with severe acute disease were converted to open cholecystectomy. Those patients who were converted to open cholecystectomy are discussed. The four port technique and decompression of the gallbladder is described. We conclude that acute cholecystitis should not be a contra-indication to the well-trained laparoscopic surgeon.

Laparoscopic cholecystectomy using intracorporeal lithotripsy.

Over a 13-month period (November 1988 to December 1989), we performed our first 104 laparoscopic cholecystectomies using an intracorporeal ultrasonic lithotripsy technique. The procedure in three of these patients was converted to an open operation because of hemorrhage with unexpected findings of cirrhosis in two patients and dense subhepatic adhesions in a third. Endoscopic retrograde cholangiopancreatography was successfully used in three other patients in whom common bile duct stones were identified. We encountered only three postoperative complications out of all the laparoscopic cholecys

[Initial experiences with laparoscopic cholecystectomy].

Laparoscopic cholecystectomy was performed in 45 patients for treatment of symptomatic gallbladder disease. The age distribution was 23 to 79 years. In four patients the procedure was not completed and a laparotomy was performed to remove the gallbladder: once because of a defective instrument, once because of bleeding of an omental adhesion and twice because of disturbed anatomy of the cystic duct region by inflammatory tissue. In four cases a laparoscopic cholecystectomy was performed because of an acute cholecystitis and in three cases a laparoscopic cholecystectomy à froid was performed. T…

Laparoscopic cholecystectomy in anticoagulated patients.

Although minimally surgically invasive, laparoscopic surgery has yet to be proven safe in patients receiving anticoagulants. Retrospectively, the laparoscopic management of four patients requiring anticoagulation for cardiac valvular prostheses or chronic atrial fibrillation was reviewed with regard to potential hemorrhagic complications. Warfarin was discontinued preoperatively in all cases. Heparin anticoagulation was individualized according to each patient's risk for thrombosis. Laparoscopic cholecystectomy and intraoperative cholangiography were completed in each patient without resulting…

Laparoscopic cholecystectomy: a multicenter study.

Laparoscopic cholecystectomy has been developed as an alternative to open cholecystectomy for the treatment of symptomatic cholelithiasis. A collaborative study of 261 patients undergoing laparoscopic cholecystectomy at three Texas institutions was performed to determine the effectiveness and safety of this technique. There were 65 males and 196 females with a mean age of 49 years (range 17-94 years). Acute cholecystitis was present in 38 patients and chronic cholecystitis in 223 patients. Mean length of surgery was 80 minutes (20 min to 4 hr). Fifteen patients were converted to the open techn…

Laparoscopic cholecystectomy: 111 consecutive cases.

Laparoscopic cholecystectomy removes the gallbladder through three or four puncture wounds in the abdominal wall. The technique reduces the recuperative time to full activity, from as long as 4 wk to as little as 3 days, compared with conventional cholecystectomy. We herein present our initial experience with this procedure. In this series of 111 laparoscopic cholecystectomies, there were no mortalities and only one morbidity. Thirty-nine patients (35%) had a history of prior abdominal surgery. Fourteen underwent laparoscopic lysis of adhesions. Intraoperative cholangiograms were performed in …

[Cholecystectomy with celioscopy. 330 cases].

Laparoscopic cholecystectomy is possible. Under general anesthesia, the optic and the instruments are introduced through 4 trocarsheaths. The cystic duct and artery are dissected and clipped. The gallbladder freed by the retrograde way is removed through the umbilicus. 330 laparoscopic cholecystectomies were done with few and only benign complications. The advantages are the simple post-operative course, the short hospital stay and the minimal parietal traumatism. Some limits remains in case of too much peritoneal adhesions.

Concomitant cholecystectomy for asymptomatic cholelithiasis.

The outcome of 68 patients with asymptomatic cholelithiasis undergoing laparotomy for other conditions was reviewed to determine those most likely to become symptomatic postoperatively. Thirty-seven patients (54%) became symptomatic postoperatively. Eight patients (22%) required cholecystectomy within 30 days of operation or within the same hospitalization. These patients fasted for a longer period of time postoperatively (15 +/- 21 vs 4 +/- 3 days) than those undergoing later cholecystectomy. Significantly more of these patients required transfusion (38% vs 7%), mechanical ventilation (50% vs…

Acalculous cholecystitis: results of cholecystectomy.

The results in a small series of patients with clinical cholecystitis, but who had no stones, on whom operation was performed are compared with results in a larger series of patients who had stones. The results suggest that, if a clinical diagnosis is made and other causes of symptoms have been excluded, the results of operation are almost as successful as in those having stones. Patients should not be denied operative relief if the symptoms strongly suggest gall bladder disease even if no stones are shown on X-ray examination.

[Trouble after cholecystectomy (author's transl)].

Unsatisfactory results after cholecystectomy are mostly due to complications of the disease not realised under surgery and to technical blunders. Only intraoperative cholangiography, exploration of the common bile duct and inspection by choledochoscopy build the sound basis for an adequate surgical procedure.
