In addition to whole hCG, hCG beta-related molecular species are also present in the serum and urine of patients with trophoblastic diseases. We simultaneously measured whole hCG, free hCG beta and beta-core fragment in the serum and urine of patients with hydatidiform mole and choriocarcinoma by the highly specific and sensitive sandwich enzyme-immunoassay (EIA) systems using two kinds of antibodies. During the clinical courses of patients, although the whole hCG levels in serum and urine were closely correlated, the concentrations in the serum were almost twice as high as those in the urine.

Persistent ectopic syndrome is a complication of conservative surgery for tubal pregnancy. This study is directed toward the escalating conservatism in the treatment of tubal ectopic pregnancy and its possible sequelae. A 32-year-old nulliparous woman underwent hMG induction of superovulation, started on the third day of "menses." The presence of hyperstimulation confused the clinical picture of ectopic pregnancy conceived during the previous cycle. This case illustrates that (1) the clinical findings of hyperstimulation may mask those of ectopic pregnancy; (2) the ovary is not refractory to hMG in the presence of circulating hCG; and (3) an inappropriately high beta-hCG value is suggestive of gestation initiated during a previous cycle.

The current study was undertaken in an effort to identify the clinical characteristics and natural history of partial moles. Three cases recently managed at Tripler Army Medical Center and 52 cases collected from the medical literature were reviewed. The mean age of the women at diagnosis was 25.6 years. One-hundred subjects received vaginal suppositories containing prostaglandin analogues in order to terminate their pregnancies of 49 days gestation or less. This non-surgical method was successful in 61 subjects, failed in 35 subjects and four subjects were lost to follow-up. In this study, we conducted a retrospective single-centre study of 1664 singleton pregnancies derived from frozen-thawed blastocyst transfer between January 2017 and December 2018. Analysis showed that there were 596 early pregnancy losses and 1068 ongoing pregnancies. Fifty-nine women with early unruptured tubal pregnancy were treated by a single local injection of methotrexate at laparoscopy. All 59 patients underwent the procedure without any adverse reaction, 47 (80%) of them needing no laparotomy. Twelve patients required a laparotomy for reasons such as rising beta-hCG levels and abdominal pain with or without rising levels of beta-hCG. Only one patient ruptured the tube. The present paper reports on the treatment of 30 tubal pregnancies verified by laparoscopy by means of intratubal prostaglandin (PG) F2a and systemic PG E2. Tubal pregnancy was successfully treated in 25 patients; three patients exhibited side effects. The comparison with surgical management showed better results concerning the tubal patency in the PG-treated group. Further studies should clarify whether this technically simple procedure is superior to the existing methods.

Ten women with tubal ectopic pregnancy were treated by the injection of methotrexate into the gestational sac under direct laparoscopic vision followed by a course of intramuscular therapy including folinic acid rescue. One course of treatment induced resolution of the extrauterine pregnancy in eight women. Complete resolution (beta-hCG less than 10 miu/ml) was achieved within 6-47 days (mean 14.5 days). In 19 patients, the transvaginal (TVU) and in 24 patients, the transabdominal (TAU) ultrasound approaches were compared in their ability to identify by visualization the adnexal mass of ectopic pregnancy. An ectopic tubal pregnancy that undergoes repeated minor ruptures instead of a single episode of rapid bleeding frequently develops into a pelvic hematocele. The hematocele, which contains old blood, clots and gestational tissue, is surrounded by adhesions and is misleadingly called a "chronic" ectopic pregnancy. Early surgical intervention in ectopic pregnancy is desirable. However, occasionally some patients do not benefit from this approach, as the natural history of ectopic pregnancy is not always tubal rupture or any other catastrophic event. A retrospective as well as a prospective analysis of 144 cases of suspected ectopic pregnancies was performed. The results of this study support expectant management in cases where serum beta-HCG levels are declining, provided the patient is stable and has no acute abdominal symptoms.
