Tuberculosis-induced arthritis (Poncet's disease): diagnostic challenges and a workflow for clinical decision-making.

INTRODUCTION: Poncet's disease (PD) is a rare form of TB-associated reactive arthritis, presents a major diagnostic challenge due to its close resemblance to rheumatoid arthritis and tuberculous septic arthritis. A clear diagnostic workflow is essential to support clinical decision-making to prevent misdiagnosis and inappropriate immunosuppressive treatment. This review synthesizes current evidence on the diagnostic characteristics of PD and propose a structured diagnostic workflow to improve clinical decision-making and reduce misdiagnosis. CONTENT: A narrative review was conducted using maj

Laboratory and functional tests in leprosy diagnosis: a practical guide for clinical decision-making.

Early diagnosis of leprosy remains primarily clinical, based on recognition of skin lesions with altered sensation and peripheral nerve involvement. However, complementary tests play crucial roles in confirming uncertain cases, guiding operational classification, detecting subclinical neural involvement, and distinguishing late reactions from relapse. This review synthesizes the practical application of diagnostic methods available to dermatologists. Slit-skin smear microscopy demonstrates high specificity but limited sensitivity in paucibacillary forms, while histopathology reveals the charac

Multimodal AI in high-grade serous ovarian cancer: integrated prediction and clinical decision-making.

Ovarian cancer remains the most lethal gynecologic malignancy, with high-grade serous ovarian carcinoma (HGSOC) accounting for the majority of deaths. It is typically diagnosed at an advanced stage, characterized by extensive peritoneal dissemination and high rates of recurrence, which contribute to persistently poor survival despite advances in cytoreductive surgery, platinum-based chemotherapy, and targeted therapies such as PARP inhibitors. Clinical outcomes are highly variable, reflecting substantial inter- and intratumoral heterogeneity at the genomic, cellular, and microenvironmental lev

Biomarkers in Clear Cell Renal Cell Carcinoma: From Biological Association to Clinical Decision-Making.

Therapeutic options in renal cell carcinoma (RCC) have expanded rapidly, including adjuvant pembrolizumab, HIF-2α-directed therapy, and multiple effective first-line combinations for metastatic clear-cell RCC (ccRCC), yet treatment selection remains largely clinicopathologic. This review evaluates biomarkers at three clinical decision points: characterization of an indeterminate renal mass; recurrence-risk assessment and adjuvant treatment selection after nephrectomy; and first-line regimen selection in metastatic ccRCC. DNA-methylation classifiers and carbonic anhydrase IX-targeted [89Zr]Zr-g

Restoring ankle dorsiflexion range of motion in athletes: an individualized clinical decision-making system.

BACKGROUND: Ankle dorsiflexion range of motion plays a pivotal biomechanical role within the lower limb with implications both in rehabilitation, injury risk reduction and athletic performance. However, clinicians often lack practical guidance on diagnosing and differentiating the various joints or structures that have been shown to have a role in ankle dorsiflexion range of motion restriction. CLINICAL QUESTION: To move beyond the "one size fits all approach" paradigm in musculoskeletal rehabilitation, we propose addressing the 2 following questions: (1) How can clinician utilize the weight-

Impact of Renal Mass Biopsy on Decision-Making Experience for Clinical T1 Renal Masses.

PURPOSE: Owing to uncertainty on best management, a subset of patients with small renal masses suspicious for kidney cancer experience elevated decisional conflict. We hypothesize that pathologic information from renal mass biopsy (RMB) may improve patient decision-making. In this study, we evaluate the impact of RMB on decisional conflict for patients with clinical T1 renal masses. MATERIALS AND METHODS: A comparative, nonrandomized clinical trial was performed at a large tertiary cancer center. Patients with new clinical T1 renal masses were self-assigned to standard-of-care biopsy. We used

Ventilation/perfusion SPECT in children: Feasibility, impact on clinical decision-making and interrater agreement.

INTRODUCTION: It remains to be established whether it provides any clinical or diagnostic benefits to perform ventilation/perfusion (V/Q) scintigraphy as a single photon emission computed tomography in children. Here, we report our experience with this modality, evaluate its impact on clinical decision-making and assess its interrater agreement. METHODS: All V/Q scintigraphies performed in children over a 1-year period were identified, and patient files were reviewed. Two nuclear medicine physicians, blinded to patient data and each other's readings, interpreted all scintigraphies. RESULTS:

Characterization of Disease Patterns in Children with Intracranial Abscesses for Enhanced Clinical Decision-Making.

BACKGROUND: Intracranial suppurative infections in pediatric patients, while rare, pose a significant risk to patient mortality. Early recognition and fast initiation of diagnosis and treatment are crucial to prevent fatal outcomes. Between December 2022 and May 2023, a significant cluster of nine cases emerged, each necessitating neurosurgical intervention. This series highlights an important trend in clinical outcomes and raises questions about underlying factors contributing to this pattern. The need for surgical procedures in all instances suggests a commonality in severity, warranting fur

Inconsistency in ferritin reference intervals across laboratories: a major concern for clinical decision making.

OBJECTIVES: Iron deficiency anemia is a significant global health concern, diagnosed by measuring hemoglobin concentrations in combination with plasma ferritin concentration. This study investigated the variability in ferritin reference intervals among laboratories in the Netherlands and examined how this affects the identification of iron-related disorders. METHODS: Ferritin reference intervals from 52 Dutch ISO15189-certified medical laboratories were collected. Ferritin, hemoglobin and mean corpuscular volume data of non-anemic apparently healthy primary care patients, measured by four lab

Molecular Subtypes of Bladder Cancer: Component Signatures and Potential Value in Clinical Decision-making.

Bladder cancer may be classified into "molecular subtypes" based on gene expression. These are associated with treatment response and patient outcomes. The gene expression signatures that define these subtypes are diverse, including signatures of epithelial differentiation, stromal involvement, cell cycle activity, and immune cell infiltration. Multiple different systems are described. While earlier studies considered molecular subtypes to be intrinsic properties of cancer, recent data have shown molecular subtypes change as tumors progress and evolve, and often differ between histologically d

Analysis of Clinical Utility of Functional MRI in Neurosurgical Decision-Making in Focal Epilepsy.

BACKGROUND: Functional MRI (fMRI) has proven valuable in presurgical planning for people with brain tumors. However, it is underutilized for patients with epilepsy, likely due to less data on its added clinical value in this population. We reviewed clinical fMRI referrals at the QEII Health Sciences Center (Halifax, Nova Scotia) to determine the impact of fMRI on surgical planning for patients with epilepsy. We focused on reasons for fMRI referrals, findings and clinical decisions based on fMRI findings, as well as postoperative cognitive outcomes. METHODS: We conducted a retrospective chart

Imaging of common coronary artery anomalies and imaging features important for clinical decision-making.

Anomalous coronary arteries are associated with sudden cardiac death, although only in a relatively small percentage. Although this has been a field of study for years and there are many ongoing studies of larger patient cohorts and longer term follow-up, important questions remain at present in clinical decision-making, particularly whether or not to have surgery. Advanced imaging techniques including coronary computed tomography (CT) and cardiac magnetic resonance (MR) play an important role in the workup of patients with known or suspected anomalies including delineating origin and course a

Prevalence of bladder cancer in Costello syndrome: New insights to drive clinical decision-making.

Costello syndrome (CS) is a rare disorder affecting development and growth characterized by cancer predisposition and caused by mutations in HRAS proto-oncogene. Somatic HRAS mutations drive bladder carcinogenesis. The aim of this study was to analyze prevalence and histological characterization of bladder cancer (BC) in a cohort of patients with CS to help clinicians plan effective management strategies. This study included 13 patients above 10 years of age with molecular diagnosis of CS. Screening cystoscopies (31 total procedures) were performed to exclude BC. Any lesion was analyzed throug

Exploration of a simplified clinical examination for scabies to support public health decision-making.

INTRODUCTION: In most settings, the diagnosis of scabies is reliant on time-consuming and potentially intrusive clinical examination of all accesible regions of skin. With the recent recognition of scabies as a neglected tropical disease by the World Health Organization there is a need for standardised approaches to disease mapping to define populations likely to benefit from intervention, and to measure the impact of interventions. Development and validation of simplified approaches to diagnose scabies would facilitate these efforts. METHODS: We utilised data from three population-based surv

Outcome of three screening questions for temporomandibular disorders (3Q/TMD) on clinical decision-making.

Patients with temporomandibular disorders (TMD) seem to go undetected and not adequately managed within dentistry. To identify these patients, three screening questions (3Q/TMD) have been introduced within dentistry in parts of Sweden. It is not known whether 3Q/TMD affects the clinical decision-making for these patients. The aim of this study was to evaluate the outcome of 3Q/TMD on the clinical decision-making and to analyse whether gender, age and the fee system the individual was assigned to were related to prescribed TMD treatment. This cohort study was carried out within the Public Denta
