Integrating Generative AI Into Clinical Reasoning Education for Medical Students: Mixed Methods Study.

BACKGROUND: Generative AI (GenAI) is increasingly integrated into clinical learning and practice. However, medical students often lack the competencies required for safe and critical use, including prompt design, output verification, and recognition of limitations. Educational interventions that integrate GenAI with clinical reasoning frameworks remain limited. OBJECTIVE: This study evaluated a structured, theory-informed workshop integrating GenAI, prompt engineering, and clinical reasoning education to enhance medical students' self-perceived AI literacy and collaborative learning attitudes

Evaluating reasoning-tuned large language models for clinical decision-making in spine surgery.

PURPOSE: Most clinical evaluations of large language models assess factual recall rather than the multi-step reasoning behind operative plans. Reasoning-tuned models, post-trained to generate explicit intermediate reasoning before answering, may better approximate surgical decision-making. We compared two such models from different development ecosystems, OpenAI o1 and DeepSeek R1, on complex spine cases. METHODS: Ten synthetic vignettes spanning deformity, degenerative, urgent, and cervical pathology were analysed; an eleventh was excluded owing to a survey error. Both models received identi

ProMem-agent: Procedural memory-augmented large language model agents for clinical trajectory reasoning.

BACKGROUND AND OBJECTIVE: Clinical large language model (LLM) agents can interpret current clinical context but have limited mechanisms for converting longitudinal experience into compact, reusable units. We developed ProMem-Agent, a procedural-memory framework that represents recurrent early intensive-care trajectories as provenance-linked observational patterns for retrospective mortality-risk estimation. METHODS: Adult ICU stays with at least 24 hours of observable data were represented as six consecutive four-hour state-action-response intervals. Memory candidates were extracted exclusive

Clinical Reasoning: A Woman With Parkinson Disease, Dyskinesia, Rhabdomyolysis, Subcutaneous Emphysema, and Pneumomediastinum.

A 47-year-old woman with Parkinson disease (PD) complicated by severe motor fluctuations had been treated with deep brain stimulation (DBS) for 11 years and intestinal carbidopa/levodopa infusion for 5 years. Three months after her most recent clinic visit, at which both the DBS and infusion pump systems were confirmed to be functioning appropriately, she presented to the emergency department with abnormal movements. On examination, she had continuous, generalized, involuntary hyperkinetic movements with an erratic, dance-like quality, consistent with levodopa-associated dyskinesia. Evaluation

Pilot investigation into the influence of racial implicit bias on physician clinical reasoning.

OBJECTIVES: Evidence for the impact of racial implicit bias and clinical reasoning remains conflicting. Our inability to characterize the relationship between racial implicit bias and clinical reasoning (CR) precludes development of comprehensive interventions seeking to address the impact of racial implicit bias on clinical encounters. To address this gap, we conducted a simulation-based investigation with clinical presentations with known health disparities, cognitive stressors common to clinical environments, and Black and White standardized patients (SPs). METHODS: We recruited 75 early-c

A clinical reasoning skills development plan for coronary care nurse: an action research.

BACKGROUND: Clinical reasoning (CR) skills are among the most important nursing competencies for providing safe and effective care in critical care units. Development of CR skills in nursing needs a well-designed interactive process for change to effectively support clinical competence promotion. The aim of this study was to develop CR skills among coronary care nurses through an action plan. METHODS: This participatory action research study was conducted in 2021 based on the framework of Hart and Bond (1995). Study setting was the coronary care unit of a leading heart center in Rasht, Iran.

Clinical Reasoning: A 59-Year-Old Man With Progressive Dysarthria and Gait Instability.

We report a 59-year-old man with a progressively deteriorating neurologic presentation characterized by dysarthria and gait instability. Neurologic examination revealed pseudobulbar palsy, cerebellar ataxia, and pyramidal signs. Neuroimaging demonstrated irregular brainstem and cerebellar lesions with minimal mass effect and partial enhancement. Bilateral heel swelling, an uncommon clinical presentation, prompted further diagnostic evaluation. This case highlights the importance of a systematic reasoning approach, emphasizing the differential diagnosis, further investigative modalities, and tr

Clinical Reasoning: Acute and Progressive Cognitive Decline in a 38-Year-Old Man.

The differential diagnosis of multiple intracranial lesions is often perplexing and highly challenging, especially in the early stages. We present the case of a young man who initially presented with an acute headache and memory decline, which rapidly progressed to significant cognitive deterioration. Based on acute clinical manifestations, elevated CSF protein levels, and atypical MRI findings, the patient was initially misdiagnosed with idiopathic inflammatory demyelinating disease of the CNS. However, the rapid progression of symptoms and the enlargement of lesions on MRI raised further sus

Clinical Reasoning: A 6-Year-Old Girl With Right-Sided Pain and Weakness.

We outline the case of a 6-year-old girl presenting with a 2-week course of waxing and waning neurologic symptoms, including right-sided pain, weakness, dizziness, and difficulty walking. Her examination was notable for right-sided weakness, hyperreflexia, and dysmetria. Diagnostic evaluation was significant for MRI with numerous T2 hyperintense, T1 hypointense, and T1-enhancing lesions located in the juxtacortical and periventricular regions, corpus callosum, brainstem, and spinal cord; positive CSF oligoclonal bands; negative serum aquaporin-4 immunoglobulin G (IgG) and myelin oligodendrocyt

Clinical Reasoning: A 72-Year-Old Woman With Rapidly Progressive Bilateral Hearing Loss.

A 72-year-old woman presented with rapidly progressive hearing loss and neuropsychiatric symptoms without other focal neurologic symptoms. Progressive sequential sensorineural hearing loss (SNHL) was demonstrated on serial audiology. A diagnostic approach to SNHL is reviewed. Lumbar puncture revealed elevated protein, low glucose, and pleocytosis with poorly differentiated cells, and a differential diagnosis is discussed. MRI of the brain revealed gadolinium enhancement within the internal auditory canals bilaterally as well as the left cochlea. Zic4 antibodies were present in serum and CSF. A

Clinical Reasoning: A 65-Year-Old Woman With Cancer History and Wrist Drop.

Wrist drop is a common presentation in neurology. To localize the lesion, clinicians can focus on testing finger extension, elbow flexion with semipronated forearm, and elbow extension among other muscle groups and identifying dermatomes of numbness. Once the lesion is localized, electrophysiology or imaging can guide to an underlying etiology. Here, we describe a case that illustrates the importance of using a stepwise approach to diagnose the etiology of wrist drop in a patient with a cancer history. A 65-year-old woman with diffuse large B-cell lymphoma in remission presented with new onset

Lessons in clinical reasoning ‒ pitfalls, myths and pearls: a case of recurrent pancreatitis.

OBJECTIVES: Cognitive biases can result in clinical reasoning failures that can lead to diagnostic errors. Autobrewery syndrome is a rare, but likely underdiagnosed, condition in which gut flora ferment glucose, producing ethanol. It most frequently presents with unexplained episodes of inebriation, though more case studies are necessary to better characterize the syndrome. CASE PRESENTATION: This is a case of a 41-year old male with a past medical history notable only for frequent sinus infections, who presented with recurrent episodes of acute pancreatitis. In the week prior to his first ep

Neuralgic Amyotrophy Attributed Incorrectly to Block-Related Injury: Understanding Errors in Clinical Reasoning.

OBJECTIVE: We report a case of misdiagnosed neuralgic amyotrophy (brachial plexus neuritis, Parsonage-Turner syndrome). Our primary objective is to review the scientific basis for errors in clinical reasoning. CASE REPORT: We herein report a patient in whom signs and symptoms compatible with neuralgic amyotrophy presented after shoulder surgery. The patient's brachial plexopathy was attributed incorrectly as a complication of interscalene brachial plexus block. The true diagnosis was made only after the patient developed neuralgic amyotrophy in the contralateral upper extremity after a subseq
