Growth hormone neuroregulation and the clinical relevance of somatostatin.

The hypothalamus controls GH secretion from the anterior pituitary using two peptides; somatostatin inhibits GH, but physiologically the most important appears to GHRF, the structure of which has recently been discovered by two groups. This exciting development has not only given us further insight into the control of GH secretion, but also posed interesting questions as to the cause of the abnormal GH responses to various stimuli seen in patients with acromegaly. The other hypothalamic peptide controlling GH secretion, somatostatin has been the subject of intensive research in the last ten ye

[The clinical relevance of scintigraphic adrenal imaging (author's transl)].

Labelled steroid precursors have been found to be valuable in scintigraphic imaging of localized adrenal disease or even in adrenal hyperplasia. Scanning procedures are non-invasive diagnostic methods and, in the case of the adrenals the images reflect anatomical as well as functional changes in the organs, constituting an advantage over angiographic methods. However, careful preparation of the patient, sophisticated technical prerequisites and a case-oriented programme of the procedure favouring either dexamethasone suppression, comparsion with phantom measurements adrenal profiles and/or mul

[Hyperprolactinemic amenorrhea. Clinical relevance, endocrine features, therapy (author's transl)].

1. Hyperprolactinemia was found in 73 out of 445 patients as the underlying cause of primary or secondary amenorrhea, There was only in 65 cases both amenorrhea and galactorrhea. 2. Pituitary tumors were found in 21 cases. The causes of hyperprolactinemia in the other patients remained unclear. 3. Concentration of LH and FSH in serum was normal or lowered. Stimulation of LH and FSH by LHRH was possible only in part. 4. Regardless of the cause of hyperprolactinemia in all patients hPRL-levels were lowered significantly by 2.5 mg CB 154 within 4 hours. 5. Longtime suppression of hPRL by CB 154 n

Clinical relevance of antiphosphatidylserine/prothrombin antibodies in antiphospholipid syndrome.

Antiphospholipid syndrome (APS) is characterised by thrombosis and/or pregnancy morbidity associated with persistent antiphospholipid antibodies (aPL). Beyond the criteria antibodies, non-criteria aPL, particularly antiphosphatidylserine/prothrombin (aPS/PT), have emerged as clinically relevant aPL in the diagnosis of APS. aPS/PT antibodies, especially IgG, are strongly associated with thrombosis, obstetric morbidity, and LA positivity. Most importantly, aPS/PT identify high-risk aPL profiles and are detectable in seronegative APS. aPS/PT antibodies are among the most informative non-criteria

Emerging hypervirulent Klebsiella pneumoniae: Myth, reality, and clinical relevance.

The global emergence of hypervirulent Klebsiella pneumoniae (hvKp) has raised concern because of its association with invasive infections and reports of increased mortality. However, recent systematic reviews and meta-analyses reveal a critical paradox: genotypically defined convergent clones and strains carrying fewer virulence-associated markers exhibit mortality rates comparable to, rather than exceeding, those of multidrug-resistant (MDR) K. pneumoniae. In contrast, traditional phenotypic identification preferentially enriches for the classical hypervirulent lineages that remain uniquely a

Anatomical insights: meningo-orbital foramen and its clinical relevance.

OBJECTIVES: The meningo-orbital foramen (MOF) is an inconsistently present bony canal that connects the orbit with the cranial cavity, commonly the middle cranial fossa. It may transmit the meningolacrimal artery supplying orbital contents, making it clinically relevant in procedures involving the lateral orbital wall. This study aimed to investigate the morphometry and anatomical positioning of the MOF and discuss its surgical implications. METHODS AND ANALYSIS: A total of 127 adult dry human skulls (254 orbits) were examined. Gender was estimated morphometrically, while ethnicity was unknow

Appraising the visibility, relevance and impacts of clinical pharmacology.

AIMS: Clinical pharmacology (CP) used to be a strong medical and scientific field, but during the last three decades it seems to have lost some of its appeal. We reviewed the visibility, relevance and impact of CP and clinical pharmacologists across the globe and suggest ways to strengthen the discipline to address future pharmacotherapeutic challenges. METHODS: Literature consultations and multiple survey approaches through personal contacts with leading experts from high-income countries, Asia and Africa and with CP scientific journal editors with presentation, discussion and recommendation

Clinical Relevance of Gastric Emptying Time in Functional Dyspepsia.

BACKGROUND/AIMS: This study evaluates the clinical significance of the gastric emptying time (GET) in patients with functional dyspepsia (FD). METHODS: This retrospective study included 89 patients who visited the clinic with indigestion between 2021 and 2022. FD was diagnosed and categorized into 3 subtypes: postprandial distress syndrome, epigastric pain syndrome, and overlap type, following the Rome IV criteria. GET was assessed using 99mTc scintigraphy. The stomach remnant was measured 1 and 2 hours after ingesting a technetium-99m labeled egg, and the time required for the remnant to dec

The clinical relevance of grip strength: A narrative exploration.

Hand grip strength (HGS) is a well-established measure of musculoskeletal function and integrity of the upper limb. Traditionally used in rehabilitation, its utility extends too many clinical conditions in primary care practice. Therefore, it is of interest to explore the diverse applications of HGS in medical practice. In osteoporosis, HGS predicts bone density and risk of fracture. In osteoarthritis, it serves as a benchmark for functional impairment. As a nutritional marker, HGS reflects malnutrition and predicts postoperative outcomes. In metabolic syndrome and chronic diseases, HGS relate

[Near-infrared Fundus Autofluorescence: Clinical Application and Diagnostic Relevance].

Near-infrared autofluorescence (NIA) is a non-invasive retinal imaging technique for examination of the retinal pigment epithelium (RPE) based on the autofluorescence of melanin. Melanin has several functions within the RPE cells, in one of them it serves as a protective antioxidative factor within the RPE cells and is involved in the phagocytosis of photoreceptor outer segments. Disorders that affect the photoreceptor-RPE complex result in alterations of RPE cells which are detectable by alterations of NIA. Therefore, NIA allows to detect early alterations in inherited and acquired chorioreti

The Genetics of Uveal Melanoma: Overview and Clinical Relevance.

Over the last ten years, much has been learnt about the genetic characteristics and genetic evolution of uveal melanoma. It has been shown that uveal melanoma differs fundamentally from non-uveal melanoma and is an independent genetic subtype. Compared to other tumours, uveal melanoma has a low mutational burden. There are recurring chromosomal aberrations with losses of 1p, 6q, 8p and 16q, gains of 6p and 8q, and the presence of monosomy 3. GNAQ, GNA11, PLCB4, CYSLTR2, MAPKAPK5, as well as mutations in BAP1, SF3B1, SRSF2 and EIF1AX, the latter being linked to a higher risk of metastasis, have

Biofilms in Diabetic Foot Ulcers: Significance and Clinical Relevance.

Foot infections are the main disabling complication in patients with diabetes mellitus. These infections can lead to lower-limb amputation, increasing mortality and decreasing the quality of life. Biofilm formation is an important pathophysiology step in diabetic foot ulcers (DFU)-it plays a main role in the disease progression and chronicity of the lesion, the development of antibiotic resistance, and makes wound healing difficult to treat. The main problem is the difficulty in distinguishing between infection and colonization in DFU. The bacteria present in DFU are organized into functionall

CD200 and Chronic Lymphocytic Leukemia: Biological and Clinical Relevance.

CD200, a transmembrane type Ia glycoprotein belonging to the immunoglobulin protein superfamily, is broadly expressed on a wide variety of cell types, such as B lymphocytes, a subset of T lymphocytes, dendritic cells, endothelial and neuronal cells. It delivers immunosuppressive signals through its receptor CD200R, which is expressed on monocytes/myeloid cells and T lymphocytes. Moreover, interaction of CD200 with CD200R has also been reported to play a role in the regulation of tumor immunity. Overexpression of CD200 has been reported in chronic lymphocytic leukemia (CLL) and hairy cell leuke
