METHODS: The authors treated a 72-year-old woman with chronic prephthisical pain in the left eye secondary to graft rejection after penetrating keratoplasty and an 81-year-old woman with severe ocular pain in her left eye for neovascular glaucoma secondary to an ischemic central vein occlusion. Clonidine tablets have been used in the past for treatment of spasticity with some success. The use of clonidine, however, has been limited by adverse effects, mainly hypotension. Over a two-year period, 17 patients were started on clonidine transdermal delivery system. They were followed for up to 18 months. The aim of our study was to explore the effect of clonidine on the mouth to caecum transit time in healthy man. Hydrogen-breath test, using lactulose 10 g, was carried out in 10 healthy male volunteers, in double-blind conditions and in random sequences of medications, placebo or clonidine (0.3 mg per os). Clonidine increased the mouth to caecum transit time significantly (p = 0.013): from 85 +/- 12 min. (with placebo) to 139 +/- 16 min. (mean +/- SE). This study suggests that the antidiarrheal action of clonidine is due, at least in part, to effect on motility of the proximal gut.

The clinical records of 25 ambulatory patients who received clonidine (Catapres-TTS) for periods of one to 19 months were reviewed to determine the effectiveness and long-term patient tolerance of this transdermal antihypertensive medication. An experimental guinea-pig model was used to show that clonidine inhibited dose dependently the wheal and flare reaction to i.d. ovalbumin in sensitized animals. The effect of clonidine was counteracted by an alpha 2-adrenoceptor antagonist, yohimbine, but not by an alpha 1-adrenoceptor antagonist, prazosin. Clonidine and doxepin alleviate the symptoms of the opiate withdrawal syndrome.
