BACKGROUND: Total proctocolectomy with IPAA reconstruction is the surgical approach of choice in ulcerative colitis, indeterminate colitis, familial adenomatous polyposis, and selected patients with Crohn's disease. Pouch stricture is a common complication after IPAA.
OBJECTIVE: This study aims to identify surgical management options for pouch stricture and offer a treatment algorithm.
DATA SOURCES: A computer-assisted search of the online bibliographic databases MEDLINE and Embase from 1990 to 2021 was performed.
STUDY SELECTION: Randomized controlled trials, cohort studies, observational

AIM: Restorative proctocolectomy has been widely adopted as the procedure of choice for restoring gastrointestinal continuity following proctocolectomy. It is often associated with improved quality of life and high patient satisfaction; however, the development of a pouch anal fistula can cause significant morbidity. Pouch fistulas are notoriously difficult to treat and there is great heterogeneity in the management reported of these fistulas. A lack of classification, and the assumption that fistulas originating from completely different aetiologies will behave and respond similarly to a part

CASE 1: Following 2 years of rectal blood loss, a 31-year-old male was diagnosed with ulcerative pancolitis in 1978. Initial treatment consisted of both topical and systemic 5-aminosalicylic acids [5-ASAs], and remission was achieved. In both 1984 and 1986 he was hospitalised due to exacerbations necessitating treatment with intravenous corticosteroids. The following years went well, without disease activity, under treatment with 5-ASA. In 1997, at the age of 50 years, a surveillance colonoscopy showed a stenotic process with a macroscopic irregularity in the sigmoid region. Histology revealed

BACKGROUND: The impact of surgery in the treatment of inflammatory bowel disease (IBD) is considered as secondary whereas conservative treatment remains the first choice. Surgery is designated for a complicated or refractory disease course. Furthermore, there seems to be prophylactic aspects as well, such as ileocecal resection in Crohn's disease and prophylactic restorative coloproctectomy for intraepithelial neoplasia associated with ulcerative colitis.
AIM: This review evaluates the impact of surgery in the treatment of Crohn's disease and ulcerative colitis in view of existing data in the

Despite recent advances in the medical treatment of ulcerative colitis (UC), approximately 25-40% of patients will need surgery during their disease. The aim of elective surgical treatment of UC is to remove the colon/and rectum with minimal postoperative morbidity, and to offer a good long-term quality of life. There are several technical options for the surgical treatment of UC; at present, the most frequently offered is restorative proctocolectomy and ileal pouch-anal anastomosis. Both the surgeon and patient should be aware of the risks associated with a technically demanding procedure and

Restorative proctocolectomy with ileal pouch-anal anastomosis has become the surgical treatment of choice for patients with ulcerative colitis or indeterminate colitis who require surgery. A subset of patients with ileal pouches may develop Crohn's disease or a Crohn's disease-like condition of the pouch after the surgery. Diagnosis, differential diagnosis, and management can be challenging. A combined assessment of endoscopy, histology, radiography, and examination under anesthesia is often necessary for an accurate diagnosis. A multidisciplinary approach by a medical and surgical team is adv

A 39-year-old woman had a proctocolectomy, an ileo-pouch-anal anastomosis and a temporary intestinal stoma. The stoma discharged a tapeworm, Taenia saginata.

AIM: Cervicovaginal or vaginal agenesis with functioning endometrial tissue is rare. We report the construction of a colon conduit which is anastomosed to posterior uterine wall or upper vaginal pouch to allow menstruation.
MATERIALS AND METHODS: We report seven girls with cervicovaginal agenesis and four with lower vaginal agenesis (aged 12-20 years) who presented with painful cryptomenorrheoa. All the girls wanted to conserve their uterus and menstruate normally. A colon conduit was constructed for the egress of menstrual blood. The colon conduit was anastomosed to the posterior uterine wal

BACKGROUND: The ileal pouch represents a unique adaptation of the ileum to serve the dual roles of nutrient absorption, fecal storage, and evacuation in a new luminal microenvironment. However, the adaptation process produces early histological inflammation and therefore clues in the development of pouchitis may lie in how the ileal pouch adapts.
AIMS: This narrative review explored anatomical and luminal ileal pouch adaptations, with a goal of designing clinical strategies that can strengthen the resilience of the pouch mucosa.
METHODS: A comprehensive PubMed search was conducted, and findi

Prostate cancer is the fourth most frequently diagnosed malignancy worldwide. Since the inception of ileal pouch-anal anastomoses (IPAA) in 1978, the clinical situation of prostate cancer in the setting of an IPAA has arisen. We aimed to examine and synthesize the literature on oncologic, urologic and pouch outcomes after prostatectomy in patients with a pre-existing IPAA. We performed a systematic review according to PRISMA guidelines, searching MEDLINE, EMBASE, Scopus, PubMed, and Google Scholar for studies published from inception to June 2025. Studies not written in English or unable to be

A 37-year-old female patient had a past history of proctocolectomy for Crohn's disease, with ileal J-pouch-anal anastomosis. She was admitted for acute obstructive symptoms. CT scan revealed a 180 twisted ileo-anal anastomosis without signs of severe ischemia (Fig. 1and video), prompting urgent endoscopic treatment. Lower endoscopy showed a complete spiral of the J-pouch which was easily untwisted and passed. Aspiration followed by placement of a multiperforated Faucher tube allowed immediate symptoms relief. The tube was withdrawn 2 days later and the patient was discharged on the same evenin

Most patients with a restorative proctocolectomy with ileal pouch-anal anastomosis do well; however, properly identifying acute and chronic complications are paramount to managing and correcting these complications to allow for optimal pouch function and avoid pouch failure. Inflammatory conditions like pouchitis may require ongoing medical therapy, but surgical intervention may be needed to correct any underlying septic complication and to repair any structural disorders. Patients with signs of pouch failure may be candidates for pouch augmentation or redo pouch surgery and should be referred

Restorative proctocolectomy with ileal pouch-anal anastomosis is the procedure of choice to maintain intestinal continuity when a total proctocolectomy is a required. It is a technically challenging operation that may be burdened by several nuanced complications both in the immediate postoperative period and in the long term. Most patients with a pouch and any kind of complication will undergo radiological studies, thus multidisciplinary collaboration between surgeons, gastroenterologists, and radiologists is paramount to their timely and accurate diagnosis. When treating pouch patients, radio

BACKGROUND: In patients with ulcerative colitis or familial adenomatous polyposis who develop neoplasia or fail medical therapy and require colectomy, restorative proctocolectomy with IPAA is often indicated. Although often well tolerated, IPAA can be complicated by cuffitis or inflammation of the remaining rectal cuff. Although much has been published on this subject, there is no clear and comprehensive synthesis of the literature regarding cuffitis.
METHODS: Our systematic literature review analyzes 34 articles to assess the frequency, cause, pathogenesis, diagnosis, classification, complic
