OBJECTIVE: Fast track concepts are used to reduce the risk of perioperative and postoperative complications after total knee arthroplasty.
INDICATIONS: The described concepts are used for patients with indications for the implantation of a total knee prosthesis.
CONTRAINDICATIONS: Contraindications for fast track concepts are aged patients, dementia, American Society of Anesthesiologists (ASA) grade IV and implantation of large revision or tumor prostheses. BACKGROUND: Practice guidelines recommend anticoagulation therapy with warfarin for stroke prevention in patients with atrial fibrillation (AF). Despite this, warfarin is underused in older adults.
OBJECTIVE: The purpose of this study was to determine the prevalence of AF in nursing home (NH) residents and the use of warfarin or other antiplatelet medications in NH residents with AF who have indications for and no contraindications against warfarin use. The secondary objective was to determine the factors associated with warfarin use in NH residents with AF.

A 55-year-old man with a history of mild psoriasis was started on imatinib for chronic myeloid leukaemia. He developed new nail dystrophy after treatment with an exacerbation of his psoriasis. For neuropathic pain, the three medications to use are gabapentinoids, tricyclic antidepressants and serotonin norepinephrine reuptake inhibitors plus topical anesthetics. Beta-blockers, tricyclic antidepressants and anti-epileptic drugs are effective preventive medications for daily migraine headaches. The three FDA-approved drugs for fibromyalgia, pregabalin, duloxetine and milnacipran, are not robust. For osteoarthritis, nonsteroidal anti-inflammatories have good efficacy, and when gastritis contraindicates them, corticosteroid injections are helpful.

A case is reported of apparent podophyllum toxicity. The patient was a 17-year-old female Indian who had received some 3 to 8 cc of a 20 percent mixture of podophyllum resin in compound tincture of benzoin (approximately equal to 0.4 gm of podophylotoxin) as an application to her vulvar condylomata. Anti-epileptic drugs are commonly used in the management of neuropathic pain. Sodium valproate, however, is an anti-epileptic drug that is not commonly used. We report four patients with neuropathic pain who responded very well to the initiation of low-dose oral sodium valproate. Low-dose sodium valproate may have a role in managing neuropathic pain, especially when other first-line agents are unsuccessful or relatively contraindicated.
