Percentage of coronary anomalies in a population of patients undergoing coronary angiography: a retrospective study.

Coronary artery anomalies, although less frequent than congenital anomalies of the heart chambers and valve morphology, should be considered in a wide range of ages, in both sexes and as a possible etiology in myocardial ischemia, infarction, and sudden death, as well as in the planning of heart surgery for coronary revascularization, correction of congenital heart malformations or valve replacement. Between January 1996 and June 2002 we reviewed our catheterization database and carried out a retrospective study of the 3660 angiographies performed in our cardiology department. The patients wer

Identification of anomalous coronary arteries and their anatomic course by magnetic resonance coronary angiography.

BACKGROUND: Anomalous coronary arteries are a rare but recognized cause of myocardial ischemia and sudden death. Identification currently requires x-ray angiography, which may have difficulty defining the three-dimensional course of the anomalous vessel. Magnetic resonance coronary angiography (MRCA) has been shown to image coronary artery anatomy noninvasively. We hypothesize that MRCA may be useful in the identification of anomalous coronary arteries and their anatomic course. METHODS AND RESULTS: Sixteen patients (9 men, 7 women, age 44 to 81 years) with anomalous aortic origins of the cor…

Coronary Dominance on Coronary CT Angiography: Prevalence and Potential Implications for Electrocardiographic Correlation.

Background/Objectives: Coronary dominance is a key anatomical variant in the interpretation of coronary artery disease and its correlation with electrocardiographic (ECG) findings. While right dominance is the most common pattern, left dominance may influence clinical and ECG presentations. This study aimed to determine the prevalence of coronary dominance using coronary CT angiography (CCTA) and evaluate its association with disease severity, gender, and ECG findings. Methods: A retrospective observational analytical study was conducted including 677 patients who underwent CCTA. Coronary domi

Coronary computed tomography angiography plaque and flow patterns in acute coronary syndrome lesions.

Acute coronary syndrome (ACS) risk stratification requires assessment across different time frames. This multicenter study investigated whether a single coronary computed tomography angiography (CCTA) scan could provide integrated short-term (≤7 days) and long-term (≥30 days) risk assessment. The study developed and validated dual-timeframe models combining population health data, stenosis severity, quantitative low-density plaque burden (LDP%), and the hemodynamic parameter ΔCT-FFR (CT-derived fractional flow reserve [CT-FFR] proximal to the lesion minus CT-FFR distal to the lesion). Key find

Association between obstructive sleep apnea and coronary inflammation: A coronary CT angiography study.

OBJECTIVE: To investigate the association between obstructive sleep apnea (OSA) and coronary inflammation measured by fat attenuation index (FAI), and to examine potential gender differences in this relationship. METHODS: We consecutively enrolled patients who underwent both sleep study and coronary computed tomography angiography (CTA). Multivariable linear regression models were constructed to assess the relationship between objective OSA severity metrics- including moderate-to-severe OSA, apnea-hypopnea index (AHI), and oxygen desaturation indices- and coronary FAI. Stratified analyses by

Assessment of coronary spasms with transluminal attenuation gradient in coronary computed tomography angiography.

PURPOSE: To evaluate the imaging features of coronary spasm, including transluminal attenuation gradient (TAG) on coronary computed tomography angiography (CCTA), in patients with vasospastic angina (VA). METHODS: A total of 43 patients with a high clinical likelihood of VA were included in the study. All the subjects underwent double CCTA acquisition: CCTA without a vasodilator ('baseline CT') and CCTA during continuous intravenous nitrate infusion ('IV nitrate CT'). A catheterized ergonovine provocation test was used to determine true VA patients. Coronary spasm is classified into focal- an

Preserved coronary flow reserve effectively excludes high-risk coronary artery disease on angiography.

UNLABELLED: Myocardial perfusion imaging has limited sensitivity for the detection of high-risk coronary artery disease (CAD). We tested the hypothesis that a normal coronary flow reserve (CFR) would be helpful for excluding the presence of high-risk CAD on angiography. METHODS: We studied 290 consecutive patients undergoing (82)Rb PET within 180 d of invasive coronary angiography. High-risk CAD on angiography was defined as 2-vessel disease (≥ 70% stenosis), including the proximal left anterior descending artery; 3-vessel disease; or left main CAD (≥ 50% stenosis). Patients with prior Q wave…
