Diphtheria is an infectious disease caused by Corynebacterium diphtheriae, and is generally characterised by proliferation of the bacteria in the upper respiratory tract, formation of a pseudomembrane, and systemic diffusion of the diphtheria toxin throughout the body. We present the case of a young man with pseudomembranous plaques on the tongue and floor of the mouth, who received systemic and locoregional medical treatment, with a satisfactory outcome after 14 days.

We report on a 34-year-old homeless woman with polysubstance addiction, admitted with multiple skin ulcers initially misdiagnosed as insect bites during a dermatological teleconsultation. A wound swab revealed Corynebacterium diphtheriae. This case illustrates the danger of cutaneous diphtheria in vulnerable groups. It emphasizes the need for microbiological diagnostics in chronic wounds, as well as the importance of correct antibiotic therapy and strict isolation measures to prevent outbreaks.

OBJECTIVES: From September 2022 an increase in Corynebacterium diphtheriae (C. diphtheriae) infections was reported in Europe. Our study focuses on 31 adolescent and young adult refugees with cutaneous C. diphtheriae infections detected in Germany. We examined treatment regimens and outcomes to provide targeted insights into the management of this infection.
METHODS: We distributed a standardized survey, focused on children and adolescents presenting to paediatric clinics through the German Paediatric Infectious Diseases Society (DGPI) and additional professional contacts in Germany. Data wer…

From September 2015 to March 2018, CDC confirmed four cases of cutaneous diphtheria caused by toxin-producing Corynebacterium diphtheriae in patients from Minnesota (two), Washington (one), and New Mexico (one). All patients had recently returned to the United States after travel to countries where diphtheria is endemic. C. diphtheriae infection was not clinically suspected in any of the patients; treating institutions detected the organism through matrix-assisted laser desorption/ionization-time-of-flight mass spectrometry (MALDI-TOF) testing of wound-derived coryneform isolates. MALDI-TOF is…

With the global immunization programme of children there is a progressive decline in the number of diphtheria cases. It is a disease commonly affecting the children caused by Corynebacterium diphtheriae usually affecting the mucous membrane of the nose, pharynx or larynx. Cutaneous diphtheria is a rare entity. We present a rare case of cutaneous diphtheria in a 15-year-old boy with nasal pharyngeal and laryngeal involvement. The patient developed anaphylactic reaction to antidiphtheritic serum (ADS) during treatment, all of which were managed successfully.

BACKGROUND & OBJECTIVE: Diphtheria infections caused by the different toxigenic biotypes of Corynebacterium diphtheriae are endemic in Delhi. Information on biochemical identification, toxigenicity and antimicrobial susceptibility to this bacterium is scanty. This retrospective study was carried out to identify isolated Corynebacteria biochemically, determine their toxigenicity, drug sensitivity and some epidemiological characteristics of diphtheria cases from Delhi and adjoining States for the period 1998-2004.
METHODS: A total of 1118 throat and 585 nasal swabs were used to detect human pat…

INTRODUCTION: Diphtheria is a vaccine-preventable disease that has become uncommon in populations with high immunization coverage. However, breakthrough infections may still occur, particularly in resource-limited settings, posing diagnostic and public health challenges.
CASE PRESENTATION: We report a clinically suspected case of diphtheria with culture-confirmed Corynebacterium diphtheriae in a fully immunized three-year-old male child from Somalia who presented with a two-day history of high-grade fever, sore throat, and poor oral intake. Two siblings developed similar symptoms during the s…

Mycolic acids, the major lipid constituents of Corynebacterineae, play an essential role in maintaining the integrity of the bacterial cell envelope. We have previously characterized a corynebacterial mycoloyltransferase (PS1) homologous in its N-terminal part to the three known mycobacterial mycoloyltransferases, the so-called fibronectin-binding proteins A, B and C. The genomes of Corynebacterium glutamicum (ATCC13032 and CGL2005) and Corynebacterium diphtheriae were explored for the occurrence of other putative corynebacterial mycoloyltransferase-encoding genes (cmyt). In addition to csp1 (…

After 3 cases of Corynebacterium diphtheriae infection associated with intravenous drug use among persons experiencing homelessness (PEH) were reported to the Health Protection Authority in Frankfurt am Main, Germany, in 2023, we examined pathogen spread among PEH. Furthermore, we investigated a possible link with the 2022 outbreak of diphtheria in Europe. From swab samples collected during August-November 2023 from 36 PEH and cutaneous lesions, we detected 3 additional cases of cutaneous toxigenic C. diphtheriae. Sequence type 574 was identified in 5 case-isolates and is genetically associate…

624 Corynebacterium diphtheriae strains, newly isolated from patients and carriers, were studied with the use of the methods of gel immunodiffusion (Elek's test) and polymerase chain reaction (PCR). In the evaluation of 388 C. diptheriae strains, found to be toxigenic in PCR, the results of Elek's test coincided with those of PCR on 98% of cases. In 38 out of 143 strains (26.5%), nontoxigenic according to the results of Elek's test, the presence of the A-fragment of the tox-gene was established. Subculturing in nutrient media made it possible to determine the presence of toxin in 19 out of 38 …

A complex of measures carried out with a view of effecting the epidemiological surveillance of diphtheria infection has made it possible to study the level and intensity of immunity to diphtheria and tetanus, determined in the passive hemagglutination test, in the child and adult population and to reveal groups, least protected against diphtheria infection. Bacteriological studies on the biological properties of Corynebacterium diphtheriae have revealed the contamination of various groups of children with this infective agents and permitted its complete characterization with the determination …

Corynebacterium pseudodiphtheriticum is an established member of the normal flora of the respiratory tract. This organism is an emerging cause of respiratory tract infection, as well as infection of the skin and skin structures, urinary tract and other sterile sites. The syndrome of C. pseudodiphtheriticum exudative pharyngitis is a diagnostic challenge of particular relevance in recent times as this organism can be confused with Corynebacterium diphtheriae in the clinical setting and in the laboratory. We report a case of exudative pharyngitis, possibly due to C. pseudodiphtheriticum, in a 14…

INTRODUCTION: Corynebacterium ulcerans can produce diphtheria toxin and although still rare, is now the predominant cause of toxigenic diphtheria infection in the UK, making this organism of great clinical and public health importance. Here we describe a cutaneous case, likely secondary to domestic animal contact.
CASE PRESENTATION: A 60-year-old female presented with a slow-healing finger-burn wound. A skin swab cultured Corynebacterium ulcerans, which was confirmed to be toxin producing. She resided with her partner and two dogs, one of which had a chronic skin lesion. Her most recent dipht

A 31-year-old woman returned to her native Germany, after a two-year stay in Tanzania, suffering from cutaneous diphtheria which had spread to nose and throat. One of her three children, a four-year-old boy, also had Corynebacterium diphtheria in his throat, without any symptoms. Both mother and child had been immunized against diphtheria.
