A Frontal Route to Middle and Posterior Cranial Fossa: Quantitative Study for the Lateral Transorbital Endoscopic Approach and Comparison with the Subtemporal Approach.

BACKGROUND: Skull base lesions within the middle cranial fossa (MCF) remain challenging. Recent reports suggest that transorbital endoscopic approaches (TOEAs) might be particularly suitable to access the MCF and expose the lateral wall of the cavernous sinus and the Meckel's cave. METHODS: The present study was developed to compare the nuances of the subtemporal approach (STA) with those of the lateral TOEA (LTOEA) to the MCF and posterior cranial fossa (PCF) in cadaveric specimens. After orbital craniectomy, interdural opening of the cavernous sinus lateral wall (CSlw), exposure of the Gass

Are arachnoid cysts actually clinically mute in relation to neuropsychological symptoms? Cognitive functioning in children with AC of middle and cranial fossa.

Aiming at being part of the discussion about the cognitive functioning of patients with arachnoid cysts (AC) and the value of neuropsychological testing in these patients, we present our study in which we investigated the cognitive functioning of 32 children with ACs of the middle cranial fossa. We compared the Stanford Binet 5 (SB 5) results obtained by the patients with the population mean values and analysed the relation between the patients' clinical details and the results of SB 5. The main conclusions of this research are: (1) In SB 5 tasks, the tested group performed worse than the popu

Outcome on hearing and facial nerve function in microsurgical treatment of small vestibular schwannoma via the middle cranial fossa approach.

Encouraging results regarding hearing preservation and facial nerve function as well as increasing understanding of the natural behaviour of vestibular schwannomas have led to the recommendation of an early treatment in small VS. The aim of the present study was to evaluate current data on functional outcome of patients with small VS treated by middle cranial fossa (MCF) approach. A retrospective chart study of all cases treated by MCF approach between October 2007 and September 2011 was performed. Records were analyzed regarding demographical data, tumor size, hearing status, vestibular funct

Endoscopic Fenestration for Treating Galassi Type III Middle Cranial Fossa Arachnoid Cysts: Single- and Multiple-stoma have the Same Curative Effect.

BACKGROUND: For endoscopic fenestration of middle cranial fossa arachnoid cysts (MCFACs), the decisions on the location and number of stomas are key issues. However, research on this particular topic has been limited. Thus, this study aimed to compare single- versus multiple-stoma endoscopic fenestration for treating Galassi type III MCFACs. METHODS: This retrospective study included 86 patients with Galassi type III MCFACs treated with endoscopic fenestration. Single-stoma fenestration to the basal cistern was performed in 37 cases, whereas multiple-stoma fenestration to the basal cistern an

Pediatric orbital multifocal cavernous hemangiomas associated with bilateral arachnoid cysts of the middle cranial fossa. Case report and review of the literature.

Cavernous hemangiomas of the orbit are benign vascular growths that commonly occur in adults and account for 6% of all intraorbital tumors. Multifocal intraorbital cavernous hemangiomas are quite rare. The reported incidence of arachnoid cysts accounts for only 1% of lesions that occupy intracranial space, and they are nearly always sporadic and single. So far, the authors have not encountered any study reporting the coexistence of bilateral arachnoid cysts of the middle cranial fossa and orbital multifocal cavernous hemangiomas. In this report, they describe a 10-year-old boy with such a hema

Early clinico-radiological outcomes following neuroendoscopic cysto-cisternostomy for middle cranial fossa arachnoid cysts: a prospective cohort study with illustrative cases.

BACKGROUND: The dysmorphogenetic arachnoid cysts' pathomechanism is most favoured, and about 50% occur as middle cranial fossa cysts (MCFAC). Still being rare, management options are yet evolving. We described the clinico-radiological features, management and early outcomes of participants with MCFAC in our service. METHODS: This prospective cohort study involved 29 pediatric participants recruited (from electronic health records, using ICD G93.0 D016080 for arachnoid cysts) between 01/01/2023 and 31/06/2023, following informed consent according to the ethical approval. All participants had n

Endoscopic reconstruction of the middle cranial fossa through a subtemporal keyhole using a pedicled deep temporal fascial flap: a cadaveric study.

BACKGROUND: Reconstruction of the skull base is essential to prevent postoperative leakage of cerebrospinal fluid (CSF). However, a reliable method of reconstructing the middle cranial fossa via a subtemporal keyhole is not available. OBJECTIVE: To determine whether less invasive reconstruction of the middle cranial fossa under endoscopic guidance with a pedicled deep temporal fascia approach via a subtemporal keyhole is feasible and useful. METHODS: The middle cranial fossa in 4 fresh cadaver heads was reconstructed with a 4-mm 0° rigid endoscope. RESULTS: A subtemporal skin incision (subt

A Three-Dimensional Anterior and Middle Cranial Fossa Model for Skull Base Surgical Training with Two Layers of the Colored Dura Mater.

BACKGROUND: Adequate epidural procedures and anatomical knowledge are essential for the technical success of skull base surgery. We evaluated the usefulness of our three-dimensional (3D) model of the anterior and middle cranial fossa as a learning tool in improving knowledge of anatomy and surgical approaches, including skull base drilling and dura matter peeling techniques. METHODS: Using a 3D printer, a bone model of the anterior and middle cranial fossa was created based on multi-detector row computed tomography data, incorporating artificial cranial nerves, blood vessels, and dura mater.

Supratentorial extracerebral cysts of the middle cranial fossa. A report of 23 consecutive cases of the so-called temporal lobe agenesis syndrome.

23 cases of supratentorial extracerebral cysts of the Sylvian fissure in children and adult patients are reported. There were 21 male and 2 female patients and the medial age was 15 years (range 2--60 years). The cysts were located in the left middle cranial fossa in 18 cases, in two cases there were bilateral cysts and three patients only had right-sided lesions. In 14 patients the presence of the cyst was uncomplicated. In two of these patients the cyst was detected only incidentally. Four patients had subdural haematomas in addition to the cyst. Two patients had subdural haematomas and were

Endoscopic Endonasal Dural Reconstruction for a Cerebrospinal Fluid Leak in the Middle Cranial Fossa of a Patient with Gorham-stout Disease with Skull Base Defect.

We report the use of endoscopic endonasal surgery for dural reconstruction following a cerebrospinal fluid leak in a 33-year-old patient with recurrent meningitis since at age of 2 years. Magnetic resonance imaging showed osteolytic changes in the left temporal and sphenoid bones, including the left pterygoid plate, a few left temporal encephaloceles, and cerebrospinal fluid-like fluid in the expanded Meckel's cave and the parapharyngeal space. After endoscopic endonasal surgery, Gorham-Stout disease was diagnosed. No recurrence of cerebrospinal fluid leakage or meningitis has been observed. T

Middle cranial fossa Sylvian fissure arachnoid cysts in infants younger than 1 year: clinical characteristics, preoperative growth patterns, and outcomes of endoscopic fenestration.

BACKGROUND AND PURPOSE: Middle cranial fossa Sylvian fissure arachnoid cysts represent the most common type of intracranial arachnoid cysts in children, yet their natural history and optimal management strategies remain controversial. In infants younger than 1 year, the number of reported cases is limited, and these patients are often analyzed together with older children, limiting our understanding of cyst growth patterns and treatment responses during infancy. This study was aimed at characterizing the clinical features and preoperative growth patterns of middle cranial fossa Sylvian fissure

Evaluation of sexual dimorphism in the foramina of the middle cranial fossa using high-resolution multi-slice computed tomography scan: A cross-sectional study.

INTRODUCTION: Sex estimation is a primary concern in forensic medicine. Although few studies have investigated sexual dimorphism in the middle cranial fossa, the obtained findings are still inconsistent. METHODS: A cross-sectional study was conducted to investigate the sexual dimorphism in three foramina [foramen ovale, foramen rotundum, and foramen spinosum] using a high-resolution 128 multi-slice three-dimensional (3D) computerized tomography scanner. Two hundred and forty adult Egyptians (55.4 % females and 44.6 % males) were included with a female: male ratio 1.2 and a mean age of 42. 87

Decompression of the geniculate ganglion and labyrinthine segments of the facial nerve through a middle cranial fossa approach using an ultrasonic surgical system: an anatomic study.

PURPOSE: The aim of this study is to evaluate the feasibility and the safety of a novel, alternative method for bone tissue management in facial nerve decompression by a middle cranial fossa approach. Several applications of Piezosurgery technology have been described, and the technique has recently been extended to otologic surgery. The piezoelectric device is a bone dissector which, using micro-vibration, preserves the anatomic integrity of soft tissue thanks to a selective action on mineralized tissue. METHODS: An anatomic dissection study was conducted on fresh-frozen adult cadaveric head

Arachnoid cyst with extraordinary extracranial extension in the skull base as a result of an iatrogenic defect of the middle cranial fossa floor: case report and literature review.

Arachnoid cysts are benign intracranial lesions that are typically diagnosed incidentally. They are divided into two types: congenital and acquired. Acquired arachnoid cysts are rare and usually arise after trauma, infection, or haemorrhage. In this report, a rare case of an iatrogenic multiloculated arachnoid cyst as an unusual complication of a skull base defect is presented. It extended extracranially into the sphenoid sinus, the ethmoid, the infratemporal fossa, the nasopharynx and the nasal cavity, as well as into the pterygomaxillary and retromaxillary space, appearing with a distinct cl

A Case of Subdural Hygroma due to a Ruptured Arachnoid Cyst in the Middle Cranial Fossa That Improved after Long-term Subdural Drainage: A Case Report and Review of the Literature.

Arachnoid cysts have the potential to rupture, leading to the development of a subdural hygroma following minor trauma. Although surgery may be considered in cases of increased intracranial pressure (ICP) or regional neurological symptoms, the optimal approach remains unclear. We report a case of subdural hygroma due to a ruptured arachnoid cyst (SHrAC) with elevated ICP successfully treated with long-term subdural drainage for over 1 month. A 26-year-old man with persistent headache was admitted to our hospital. Magnetic resonance imaging revealed an arachnoid cyst within the left middle cran
