Use of computer-assisted design and manufacturing to localize dural venous sinuses during reconstructive surgery for craniosynostosis.

OBJECTIVE: Cranial vault remodeling surgery for craniosynostosis carries the potential risk of dural venous sinus injury given the extensive bony exposure. Identification of the dural venous sinuses can be challenging in patients with craniosynostosis given the lack of accurate surface-localizing landmarks. Computer-aided design and manufacturing (CAD/CAM) has allowed surgeons to pre-operatively plan these complex procedures in an effort to increase reconstructive efficiency. An added benefit of this technology is the ability to intraoperatively map the dural venous sinuses based on pre-operat

Incidental maxillary sinus findings on cranial computerized tomographic scan in a tropical setting.

BACKGROUND: Abnormalities in the maxillary sinuses are occasional findings on routine CT scans of the head. AIM: To study the incidental findings in the CT scans of the maxillary sinuses in patients with indications other than sinuses-related diseases. SETTING: Jos University Teaching Hospital, Jos, Nigeria. MATERIALS AND METHODS: Images of patients who underwent CT scan of the head over a 5-month period (January 2015- May 2015) were retrospectively reviewed with the exclusion of those whose indication for the scan was trauma or sinus related disease. RESULTS: A total of 121 CT scan images

[Congenital anosmia].

A 72-year-old woman with congenital familiar lack of smell is described. General ENT and otoneurological examination were normal. The ability to smell was totally absent. By MR scan normal conditions were found in the nose, paranasal sinuses, the cribriform plate in the anterior cranial fossa and in the brain especially in the olfactory bulbs. Microscopic examinations of biopsy specimens from the olfactory region showed no signs of olfactory epithelium.

REPRINTED FROM THE 2023 HYPERBARIC INDICATIONS MANUAL 15th Edition:Intracranial Abscess.

The term "intracranial abscess" (ICA) includes cerebral abscess, subdural empyema, and epidural empyema, which share many diagnostic and therapeutic similarities and, frequently, very similar etiologies. Infection may occur and spread from a contiguous infection such as sinusitis, otitis, mastoiditis, or dental infection; hematogenous seeding; or cranial trauma. Brain abscess usually results from predisposing factors such as HIV infection, immunosuppressive drug treatment, surgery, adjacent infection (i.e., mastoiditis, sinusitis, dental infection), or systemic infection causing bacteremia. [No-malignant lessions involving the paranasal sinuses and anterior skull base].

BACKGROUND AND OBJECTIVES: The lesions that involve the paranasal sinuses and the anterior cranial base at the same time are not unusual. These diseases have different features. The aim of this study is to set out the particularities of the non-malignant lesions involving both zones. MATERIAL AND METHODS: Retrospective study of 32 patients between 1986 and 2007 diagnosed with: non-malignant tumours (31.2 %), tumorlike lesions (3.1 %), fibrous-osseous lesions (12.5 %), congenital or acquired malformations (18.7 %) and infection disease (34.3 %). We analyse the diagnostic imaging, the treatment

Treatment of cranial dural arteriovenous fistulae by interruption of leptomeningeal venous drainage.

Cranial dural arteriovenous fistulae (AVF's) of the tentorial incisura or the dura of the middle fossa have a much higher incidence of draining via leptomeningeal veins than do AVF's of the transverse-sigmoid sinuses or the cavernous sinus. Such a drainage pattern is associated with an increased incidence of intracranial hemorrhage and progressive focal neurological deficits. Patients with cranial dural AVF's often undergo surgical excision and/or endovascular embolization for elimination of the AVF. Since these lesions are frequently large and involve the skull base or adjacent dural sinuses,

Neurilemmomas of the paranasal sinuses.

Within the head and neck region, neurilemmomas are rarely found in the nasal cavity or paranasal sinuses. A 70-year-old man presented with complaints of left-sided epiphora, rhinorrhea, epistaxis, and transient diplopia. Nasal endoscopy showed a large necrotic mass filling the left middle meatus. A computed tomography scan showed a large left ethmoid mass, with erosion of the medial wall of the orbit and the anterior cranial base. Multiple biopsies were nondiagnostic. Complete excision of the tumor was performed with endoscopic techniques. Small dural defects were repaired with a middle turbin

A Real Headache: Intracranial Extension and Epidural Abscess As Complication of Chronic Mucocele.

Mucoceles are benign lesions of salivary glands typically originating from the paranasal sinuses. Intracranial extension and superinfection of these lesions are rare but serious complications of chronic mucoceles. Here, we discuss a patient with a known mucocele, initially lost to follow-up, who presented three years later with headache, purulent rhinorrhea, and intracranial extension of his mucocele with development of an epidural abscess. This case highlights the potential complications of chronic, large mucoceles and emphasizes the importance of thorough evaluation in patients with facial a

Nonepithelial tumors of the nasal cavity, paranasal sinuses, and nasopharynx. A clinicopathologic study. XIII: Meningiomas.

Twelve cases of meningiomas involving the nasal cavity and paranasal sinuses are reported. Meningiomas only rarely involve the upper respiratory tract; these 12 cases were found among the 566,000 surgical pathology cases that have been studied in this laboratory. In this series, five meningiomas probably arose in the cranial cavity and secondarily extended into the nasal cavity and paranasal sinuses. Because radiographs demonstrated hyperostosis or bone destruction, three other tumors could have involved the cranial cavity, but this was never proven. In two other cases, the meningiomas appeare

Operational Analysis of Trautman's Triangle in Petroclival Region Approaches: Cone-Beam Computed Tomography Study.

OBJECTIVE: This study was conducted to evaluate the anatomical and clinical features of Trautman's triangle (TT) and to better understand the possible surgical corridor for other surgical approaches involving the petroclival region, especially the presigmoid retrolabyrinthine approach. METHODS: In this study, morphological analysis of structures related to TT was performed from cone beam computed tomography images of 134 female and 206 male individuals aged 18-65 years. RESULTS: The TT area was observed as 5.6% (n = 19) type I, 63.2% (n = 215) type II, and 31.2% (n = 106) type III. It was de

Balloon protection of the vein of Labbé during embolisation of a dural arteriovenous fistula.

We describe a novel technique of vein of Labbé preservation using balloon protection during embolisation of a transverse-sigmoid sinus dural arteriovenous fistula. A patient with refractory Cognard type IV fistula of the left transverse-sigmoid sinus and persistent pulsatile tinnitus underwent successful embolisation of the lesion via transarterial route. During embolisation, a dual lumen balloon was simultaneously inflated within the vein of Labbé, at its orifice in the transverse-sigmoid sinus junction, to prevent embolic reflux. This allowed for liquid embolic embolisation of the fistula vi

Transvenous Coil Embolization of a Transverse-Sigmoid Sinus Dural Arteriovenous Fistula: 2-Dimensional Operative Video.

This case is a 66-yr-old woman with a 2-mo history of left-sided tinnitus. Workup with magnetic resonance angiography showed early opacification of the left sigmoid sinus and internal jugular vein as well as asymmetric and abundant opacification of the left external carotid artery branches, suspicious for a dural arteriovenous fistula (dAVF). Diagnosis was confirmed with cerebral angiography, consistent with a left-sided Cognard type I dAVF.1 Initial treatment attempt was made with transarterial 6% ethylene-vinyl alcohol copolymer (Onyx 18) embolization of feeders from the occipital and middle

[Percutaneous Transluminal Sinus Stenting for Sigmoid Sinus Stenosis Associated with Transverse-Sigmoid Sinus Dural Arteriovenous Fistula:A Case Report].

We report a case of transverse sinus-sigmoid sinus dural arteriovenous fistula(T-S dAVF)with venous flow congestion, which was accompanied by sigmoid sinus stenosis and treated with percutaneous transluminal sinus stenting. A 76-year-old woman presented with dementia and disturbance of consciousness. Magnetic resonance imaging(MRI)on admission revealed subcortical edema in the left occipital lobe and angiography demonstrated a left T-S dAVF with right transverse sinus occlusion and sigmoid sinus stenosis. Hemodynamics of the shunt flow from several feeders demonstrated retrograde flow to the s

Trigeminal Function in Sino-Nasal Health and Disease.

The upper airway (nasal passages, paranasal sinuses, pharynx, and glottis) provides the sentinel portion of the human respiratory tract, with the combined senses of olfaction (cranial nerve I) and trigeminal sensation (cranial nerve V) signaling the quality of inspired air. Trigeminal function also complements the sense of taste (in turn mediated by cranial nerves VII, IX and X), and participates in the genesis of taste aversions. The ability of trigeminal stimulation in the upper aero-digestive tract to trigger a variety of respiratory and behavioral reflexes has long been recognized. In this

Serial neuroimaging in tolosa-hunt syndrome with acute bilateral complete ophthalmoplegia.

Tolosa-Hunt syndrome (THS) is a very rare, relapsing, and remitting painful ophthalmoplegia caused by nonspecific granulomatous inflammation in the cavernous sinus. To our knowledge, bilateral complete, simultaneous palsies of all 3 cranial nerves associated with extraocular movement have not been reported. We describe the first such patient with bilateral THS that responded quickly to corticosteroid therapy. A 54-year-old man presented with a periorbital and frontal headache with acute bilateral severe blepharoptosis and fixed eyes, which dramatically responded to corticosteroid therapy. He h

Rhinocerebral invasive mycosis: occurrence in immunocompetent individuals.

We report the computed tomographic appearance of invasive fungal disease of the paranasal sinuses in 13 patients. Coronal and axial computed tomographic images were obtained in each patient and data were analysed. Eight patients had Aspergillus flavus infection, four had Mucormycosis, and one had mixed Candida and Mucor. Our experience was different from that of other workers in many respects. All of our patients were immunocompetent. On radiological imaging by computed tomography, ethmoid sinuses were involved in 85% of our patients. In previously reported series maxillary sinuses were most f

[Frontobasal fractures in children].

In 1987, four children between 5 and 13 years of age with severe frontobasal trauma and dural tears were treated operatively. A CSF rhinorrhea was manifest clinically in only two cases. In addition high-resolution computerised tomography was essential in diagnosis and planning of the operation. In each of the cases an intracranial pneumatocele indicating dural laceration was shown. The fractures were confirmed during surgery in: Case 1. The roof of the ethmoid sinus and the roof of the orbit. Case 2. Both the sphenoid sinuses. Case 3. The roof of the ethmoid sinus and the posterior wall of the
