Pharmacology of colloids and crystalloids.

We have attempted to review body fluid distribution by compartments so that the reader understands the physiology of ICF and ECF, and the relationship between interstitial and intravascular fluids. Crystalloids such as NS and RL are distributed to the ECF, whereas colloids primarily remain intravascular for longer periods. Although effective, crystalloids tend to require larger volumes for infusion, and edema remains a problem. Colloids as a group are extremely effective volume expanders, but none is ideal. Albumin, hetastarch, dextran, and the less commonly used colloids each have significant

Fluids and hyperosmolar agents in neurocritical care: an update.

PURPOSE OF REVIEW: To discuss recent updates in fluid management and use of hyperosmolar therapy in neurocritical care. RECENT FINDINGS: Maintaining euvolemia with crystalloids seems to be the recommended fluid resuscitation for neurocritical care patients. Buffered crystalloids have been shown to reduce hyperchloremia in patients with subarachnoid hemorrhage without causing hyponatremia or hypo-osmolality. In addition, in patients with traumatic brain injury, buffered solutions reduce the incidence of hyperchloremic acidosis but are not associated with intracranial pressure (ICP) alteration.

Fluid management in hemorrhagic shock.

PURPOSE OF REVIEW: Fluid management in hemorrhagic shock is a controversial topic, and there are evolving clinical guidelines and evidence-based practice. This review aims to highlight the physiological aspects in the light of current evidence on which volume replacement solution to use. RECENT FINDINGS: Current evidence and international guidelines are shifting from a liberal to a restrictive volume resuscitation strategy, emphasizing the potential risks associated with aggressive fluid therapy. The 'lethal triad of trauma' (hypoperfusion, acidosis, and coagulopathy) plays a crucial role in

Albumin kinetics, intravascular fluid volume, and respiratory function in pigs ventilated at different levels of mechanical power following crystalloid vs. albumin infusion.

BACKGROUND: The rationale of albumin use lies in its potential to increase oncotic pressure and optimize tissue perfusion. Randomized trials have not demonstrated a survival benefit, and the effects of albumin on volemia remain unclear. This study investigates, in healthy pigs, the effects of a 48-h albumin infusion on intravascular fluid volume, albumin kinetics, and its impact on respiratory function. METHODS: Thirty-nine healthy female pigs ventilated for 48 h were grouped according to mechanical power (high ~ 18 J/min vs. low ~ 6 J/min) and type of fluid (5% albumin solution vs. crystallo

[The supply of blood, albumin and infusion solutions during war].

Use of blood, albumin and infusion solutions is one of the essential measures in management of war casualties which significantly contribute … l of blood, 0.25 l of 5% albumin solution, 0.5 l of 6% clinical dextrane solution and one litre of crystalloid solutions. The availability of these quantities is directly proportional with the effectiveness of the systems for provision of blood, albumin and infusion solutions. The success of the systems depend on realisation of the definite prerequisites, especially o …

Fetal risks with dextrans during delivery.

Epidural analgesia for caesarean section is increasingly used and is gradually replacing general anaesthesia. Hypotension is one of the main risks: preloading of the maternal circulation is used to prevent maternal hypotension and its consequences. For this, various colloid and crystalloid solutions are used. We report a case of maternal anaphylactoid reaction with apparent death in a neonate after dextran administration to the mother. After 100ml of a dextran 40 solution administered intravenously, immediately before an epidural blockade, the mother fainted and developed urticaria and mild re…

Balanced Crystalloids versus Saline in Noncritically Ill Adults.

BACKGROUND: Comparative clinical effects of balanced crystalloids and saline are uncertain, particularly in noncritically ill patients cared for outside an intensive care unit (ICU). METHODS: We conducted a single-center, pragmatic, multiple-crossover trial comparing balanced crystalloids (lactated Ringer's solution or Plasma-Lyte A) with saline among adults who were treated with intravenous crystalloids in the emergency department and were subsequently hospitalized outside an ICU. The type of crystalloid that was administered in the emergency department was assigned to each patient on the ba

A randomised controlled trial of fluid pre-loading before low dose epidural analgesia for labour.

Preloading with fluid is recommended before regional block in labour. Low dose epidurals may produce less haemodynamic disturbance than traditional stronger solutions of local anaesthetics. Our aim was to compare the incidence of hypotension in normal labouring women who received a low dose epidural (0.1% bupivacaine 15 mL with fentanyl 2 microg microg.mL(-1)) with and without an i.v. crystalloid preload. Women with normal labours were randomised to the intervention group: no i.v. crystalloid preload (n = 85) and the control group: 7 mL mL.kg(-1) i.v. crystalloid solution before epidural injec

[Fluid therapy for superior mesenteric artery occlusion shock in rats].

The efficacy of three kinds of fluid such as isotonic sodium chloride (0.9% NaCl), hypertonic sodium chloride (7.5% NaCl) and iso-oncotic (3% in saline) albumin solutions to maintain the hemodynamics was investigated in rats with superior mesenteric artery occlusion (SMAO) shock. Three kinds of fluid were infused so as to maintain the mean arterial pressure within 70% of the control values. Animals in the control group without fluid therapy died within three hours because of shock. Three regimens could maintain the mean arterial blood pressure over three hours. The hypertonic crystalloid solut

Effects of fluid resuscitation with colloids vs crystalloids on mortality in critically ill patients presenting with hypovolemic shock: the CRISTAL randomized trial.

IMPORTANCE: Evidence supporting the choice of intravenous colloid vs crystalloid solutions for management of hypovolemic shock remains unclear. OBJECTIVE: To test whether use of colloids compared with crystalloids for fluid resuscitation alters mortality in patients admitted to the intensive care unit (ICU) with hypovolemic shock. DESIGN, SETTING, AND PARTICIPANTS: A multicenter, randomized clinical trial stratified by case mix (sepsis, trauma, or hypovolemic shock without sepsis or trauma). A total balanced volume replacement strategy using a new balanced hydoxyethyl starch preparation (6% HES 130/0.42) in patients undergoing major abdominal surgery.

BACKGROUND AND OBJECTIVE: The kind of fluid for correcting hypovolaemia is still a focus of debate. In a prospective, randomized, controlled and double-blind study in patients undergoing major abdominal surgery, a total balanced volume replacement strategy including a new balanced hydroxyethyl starch (HES) solution was compared with a conventional, non-balanced fluid regimen. METHODS: In Group A (n = 15), a new balanced 6% HES 130/0.42 was given along with a balanced crystalloid solution; in Group B (n = 15), an unbalanced conventional HES 130/0.42 plus an unbalanced crystalloid (saline solut

Are lactated Ringer's solution and normal saline solution equal with regard to coagulation?

Источник пометил публикацию как отозванную

UNLABELLED: Crystalloids represent an attractive strategy to alleviate intravascular volume deficits. Crystalloid hemodilution was associated with hypercoagulability in in vitro and in vivo studies. The influence of different crystalloids on coagulation in the surgical patient is not well studied. In a prospective, randomized study in patients undergoing major abdominal surgery, we used either lactated Ringer's solution (RL) (n = 21) or 0.9% saline solution (SS) (n = 21) exclusively for intravascular volume replacement over 48 h to maintain central venous pressure between 8 and 12 mm Hg. The new generation gelatins. A review of their history, manufacture and properties.

Gelatin solutions of various kinds have been used as intravenous infusions since the First World War and belong to a heterogeneous group of fluids which tend to maintain the colloid osmotic pressure. They are produced by the breakdown of bovine collagen to yield solutions with a range of molecular weights. Anaphylactoid reactions are a rare but well-recognised complication of their use. The solutions help to fill a hiatus that exists between crystalloids and blood products.
