Cochlear implantation in children with single-sided deafness: does aetiology and duration of deafness matter?

For adult patients with single-sided deafness (SSD), treatment with a cochlear implant (CI) is well established as an acceptable and beneficial hearing rehabilitation method administered routinely in clinical practice. In contrast, for children with SSD, CI has been applied less often to date, with the rationale to decide either on a case-by-case basis or under the realm of clinical research. The aim of our clinical study was to evaluate the longitudinal benefits of CI for a group of children diagnosed with SSD and to compare their outcomes with respect to patient characteristics. Evaluating a…

[Psychogenic deafness - a rare problem in the differential diagnosis of the sudden deafness (author's transl)].

The sudden deafness is in our days a well known clinical picture, which incidence seems to augment. Already in 1950 Fowler described the frequent coincidence of psychosomatic disturbances with the sudden deafness. On these conditions a psychogenic deafness can be taken in some cases for a sudden deafness, if there is not carried out at least one simulation test. An additional hint at an "hysteric" deafness could be the otalgia, which we found in patients. The unilateral hearing disturbance is not a reliable sign against the psychogenic deafness as could be demonstrated in our cases as well as …

Defying Deafness: Remembering Jack Ashley.

After he sustained a profound hearing loss in his only hearing ear following a routine myringoplasty, Jack Ashley became Britain's first deaf politician. His story is an inspiring one, converting a postoperative complication into a driver for success and change in the lives of millions of deaf and disabled people around the world.

CI in single-sided deafness.

The cochlear implant (CI) as a treatment option for single-sided deafness (SSD) started with a clinical study looking in to the influence of cochlear implantation with a MED-EL device on incapacitating unilateral tinnitus in SSD. The study began in 2003 and was conducted by P. Van de Heyning and his team in Antwerp, Belgium. The first CI in SSD without tinnitus in Germany was implanted by J. Mueller and R. Jacob in Koblenz in 2005. Translational research activities took place since then to evaluate the CI as a treatment option for SSD not only in adults but also in children. They assessed the …

[Diagnostic approach to child deafness].

Diagnostic approach to child deafness. The identification of a hearing loss in a child must lead the practitioner to take two initiatives: first, he must characterize it as a conductive or perceptive hearing loss, and assess its severity; secondly, he must try to establish its origin because the management can depend on it. The etiological search relies on a careful physical examination and complementary tests oriented on the findings of the latter, the clinical history and not only personal but also prenatal medical history. Admittedly, genetic causes are majoritarian among perception deafnes…

Diabetes, deafness and renal disease.

Deafness, kidney disease and diabetes are not a usual association, neither is a family history of these diseases. We present the case of a 47-year-old woman with non-nephrotic proteinuria, no haematuria, normal renal function, sensorineural hearing loss, recently diagnosed diabetes and maculopathy. There was a maternal family history of deafness, diabetes and renal disease. Renal biopsy revealed focal and segmental glomerulosclerosis (FSGS), leading to the pursuit of an m.3243A > G mitochondrial mutation and diagnosis of maternally inherited diabetes and deafness. The association of FSGS with …

[The reclassification of sudden deafness].

Objective:Reclassified the total deafness and flat type of sudden deafness, identified the relationship between new classification and hearing prognosis.Method:To analyze 192 cases of patients with sudden deafness, especially for 159 cases of flat type and total deafness patients for further curve type classification, classified as rise, decline and consistent, analysis the possible factors with hearing in multiple factors using regression analysis.Result:According to age group of 45 years old,P>0.05,the difference has not statistically significant;Course of the disease according to the 0-7 days an…

A cardiac cause for deafness.

We describe a case of a 49-year-old diabetic man with a history of myocardial infarction, presenting with deafness for 2 weeks. Initial assessment by otorhinolaryngologists was suggestive of sensorineural hearing loss. Subsequently, the cardiac evaluation showed an apical clot in the left ventricle. Careful neurological reassessment and a cranial MRI yielded a diagnosis of cortical deafness with auditory agnosia secondary to bilateral temporal infarcts. Doppler and MRI ruled out carotid artery thrombosis. The temporal infarcts were therefore considered to be cardioembolic in origin. Cardioembo…

Vestibular dysfunction in DFNB1 deafness.

Mutations of GJB2 and GJB6 (connexin-26 and 30) at the DFNB1 locus are the most common cause of autosomal recessive, nonsyndromic deafness. Despite their widespread expression throughout the vestibular system, vestibular dysfunction has not been widely recognized as a commonly associated clinical feature. The observations of vertigo accompanying DFNB1 deafness in several large families prompted our hypothesis that vestibular dysfunction may be an integral, but often overlooked, component of DFNB1 deafness. Our aim was to define the prevalence of vestibular dysfunction in Cases of DFNB1 deafnes…

Cochlear implantation in otosclerotic deafness.

The otosclerotic process commonly involves the otic capsule and may cause quite widespread demineralisation which leads to a progressive and often profound bilateral sensorineural hearing loss. In this situation cochlear implantation may be the only effective treatment. This chapter considers the pathology of this hearing loss, the effects of cochlear obliteration on implantation, and the effects of demineralisation of the otic capsule on placement of the electrode and nonauditory stimulation. A study is reported from 4 cochlear implant centres in the UK and the Netherlands of 53 patients with
